Amendment #6 to Exhibit A - Statement of Work 141
(City Secretary Contract No. 32401)
for City of Fort Worth
THIS AMENDMENT#6 ("Amendment') is made to Exhibit A— Statement of Work ("SOW"), which is incorporated
into CSC No. 32401, executed September 8, 2005 between City of Fort Worth (the "City" or "Client') and
Solutionary, Inc. The Effective Date of this Amendment is the date the second of the two parties signs below.
The parties agree as follows:
1. The term of the SOW for eV3 Subscription Services is extended for an additional 12 months. The new term
will end on September 24, 2010.
2. The term of the SOW for ActiveGuard Subscription Services is extended for an additional 12 months. The
new term will end on September 24, 2010.
3. Upon execution of this Amendment by the parties, any subsequent reference to the SOW between the parties
shall mean the SOW as modified by this Amendment.
4. Except as expressly modified herein, the terms and conditions set forth in the SOW shall remain in full force
and effect.
