FOR ARCHITECTURAL DESIGN SERVICES

WHEREAS, The City of Fort Worth and Perkins Will (Architect) made and entered into City Secretary
Contract No. 32411 (The Contract) which was authorized by the City Council by M&C C-20926, the 30th
day of August, 2005; and
WHEREAS, further amendment is necessary to compensate the Architect for reimbursable expenses
relating to the new Nashville Neighborhood Policing District No.6 and Traffic Division Facility located at
1100 Nashville Avenue.
NOW THEREFORE, City and Architect, acting herein by and through their duly authorized
representatives, enter into the following agreement which amends the Contract:

Article 1.1, SECTION I —SERVICES OF THE ARCHITECT, a paragraph is added to read as follows:
"Provide additional design and construction administration services for the new Nashville NPD #6
and Traffic Division Facility, to resolve the "Bump Out" at the south wall of the building. The not-to-
exceed compensation of additional design fees and reimbursable expenses are $2,475.00 and
$200.00, respectively. The scope of services is described more fully in the Perkins+Will letter
dated June 17, 2010 subject: Additional Services Proposal—"Bump Out" at South Wall—NPD#6,
which is attached hereto and as Attachment"A" and made a part of this Agreement."

Paragraph 1 of SECTION V-COMPENSATION TO ARCHITECT, is amended as follows:
"The total compensation for all of the assignments to be performed by Architect as described in
CHARACTER AND EXTENT of ARCHITECT'S SERVICES hereof shall be a fixed fee of
$835,400.00 plus up to $56,948.31 in reimbursable expenses."

All other provisions of this Contract which are not expressly amended herein shall remain in full force and
effect.

Re: Additional Services Proposal — "Bump Out" at South Wall — NPD6
Dear Alfonso:
Perkins+Will, Inc. is pleased to provide the City of Fort Worth with this proposal for additional
professional services. Per your email on 6-2-10, we are submitting a formal proposal for your use
to initiate the next amendment. Please find below our description of additional scope and
proposed compensation.
SCOPE OF PROJECT
Regarding the issue with the "Bump Out" at the South wall:
We propose to do the following scope of work as indicated below to detail a solution to repair the
issue along the south wall where the corrugated metal panels do not align:
Ten (10) hours of labor by Perkins+Will that shall be completed by Kippen Schecht, Project
Manager at the hourly rate of $210.00/hr as established in the owner/architect contract for
additional services. The work shall include drawing modifications at 1/2" = 1'-0" scale for wall
section 1/A-406 (which includes the overall coping, corrugated metal wall panel, and the head
and jamb detail of the window condition) as well as providing modified section details of 07/A-
424, 08/A-424, 09/A-424, and 11/A-424. The section details will include 3" = 1'-0" scale
drawings of the modified coping condition, the head detail at the window condition, the sill detail
at the window condition, the soffit condition underneath, and the modified detail of the overhang
where it meets the "bump out" area. In addition, the ten (10) hours of labor shall include time
spent by the architect to coordinate with the structural engineer, time spent at the site to observe
and approve installation, coordination with the general contractor, and the issuing of an ASI with
the modified details and an explanation indicating how to correct the non-conforming work.
Three (3) hours of time will be included for Renate Woods and Associates to review the
architectural details and run calculations to verify that the conditions of the work as detailed are
acceptable regarding the structural integrity. The hourly rate for Renate Woods, per the additional
services section of the owner/architect agreement, will be calculated at $125.00/ hr.

ADDITIONAL SERVICES FEE
The total additional services will be at a rate not to exceed $2,475.00 for the services of the
architect and structural engineer [(10x210.00) + (3x125.00) =$2,475.001.
REIMBURSABLE EXPENSES
Reimbursable expenses shall be for items as agreed to in the original contract and shall be
reimbursed at their actual cost. Reimbursable expenses for the additional services as outlined in
this proposal shall not exceed the amount of $200.00.
APPROVAL
Please review this proposal and if it is acceptable, please authorize Perkins+Will, Inc. to proceed
by signing below and returning a copy to me. We appreciate the opportunity to be of service to the
