WHEREAS, the City of Fort Worth (City) and Wade & Associates,
Inc. , (Engineer) made and entered into City Secretary Contract No.
28404, (the Contract) which was authorized by the City Council by
M&C C-19437 on the 21st day of January 2003, and subsequently
revised by one previous amendment; and
WHEREAS, the Contract involves engineering services for the
following project:
Engineering contract for the design of Sanitary Sewer
Rehabilitation Contract LVI (56) .
WHEREAS, it has become necessary to execute Amendment No. 2
to said Contract to include an increased scope of work and revised
maximum fee;
NOW THEREFORE, City and Engineer, acting herein by and
through their duly authorized representatives, enter into the
following agreement which amends the Contract:
1.
Article I of the Contract is amended to include the
additional engineering services specified in a proposal letter
dated July 18, 2005, a copy of which is attached hereto and
incorporated herein. The cost to City for the additional design
services to be performed by Engineer total $22, 979 .40 .

Article II of the Contract is amended to provide for an
increase in the maximum fee to be paid to Engineer for all work
and services performed under the Contract, as amended, so that the
total fee paid by the City for all work and servi all not

All other provisions of the Contract which are not expressly amended
herein shall remain in full force and effect.

Mr.Domenech:
Please find attached, revised Contract Amendment No. 2 to the Contract 56 Rehabilitation
and Improvements Project. This amendment has been revised to clarify the sanitary sewer
designs associated with the Hams Hospital area improvements that have been constructed.
Amendment No. 2 addresses the recovery of labor and expenses associated with the
following items:
1. L-39/L-44: Additional Design Services: Additional topographic survey data was
collected and processed for the Weatherford Street alignment revision at the direction of
the City. Additional topographic survey data was also collected and processed for the
portion of L-44 that was abandoned in Henderson St. Additional design changes to the
proposed sanitary sewer improvements(L-39 &L-44)in the Henderson and W.Belknap
Street area were also recently made to the plans. The design changes were made to
facilitate the construction of these improvements at the request of the Department of
Engineering and the Water Department. The changes included revising several sanitary
sewer service re-route plans and converting a portion of L-39 into a private service to
serve 929 W. Belknap Street using trenchless rehabilitation (CIDP). These changes
required the re-submission of the plans and right of entry documents multiple times. We
are seeking to recover the cost of processing the 845 LF of additional survey data
associated with Henderson and Weatherford Streets. Additionally, we are seeking to
recover the cost of two service re-route plans and 90 LF of trenchless sewer design,plus
the revision and re-submission of 7 temporary right of entry documents and the entire set
of design plans on mylar(7 sheets).
Harris Hospital (M-35): Additional Design Services: The original project scope
required the replacement of 85 LF of M-35 in 6N Street. However, significant scope of
work changes was ultimately required for the sanitary sewer improvements located in 6'h
Street around Harris Hospital that affected existing M-35 and M-442R. The City
directed us to completely evaluate all of the sanitary sewer system in this area and to
recommend system changes. The system evaluation process required manhole

inspections, additional dye testing and pipeline condition assessment analysis through
CCTV tape reviews. Ultimately the sewer system changes amounted to 419 LF of sewer
system and large service lateral designs. Additionally, the system improvements
included abandoning approximately 126 LF of M-35 and diverting it to M-442R in 6ffi
Street as well as replacing the last section of M-35. The revised project scope also
included replacing the three primary 8-inch sewer service lines for Harris Hospital that
cross through the hospital's maintenance tunnels and connected to M-442R and M-35.
These sewer service crossings of the maintenance tunnels required the preparation of
special design details. At the request of Harris Hospital, Wade provided limited
inspection services during the construction of these sewer crossings.
Additionally, we re-routed a sewer force main and several other sewer services that
serve various Harris Hospital facilities as part of this project. The design of sewer
improvements in this area required a number of subsurface utility
investigations/inspections, additional surveying and included some construction
inspection services in order to complete this project. We were also required to attend
weekly coordination meetings discussing all of the ongoing water and wastewater
projects in the area. We are seeking labor and expenses associated with the increase in
design footage and extra services provided to see the successful construction of these
improvements.
The table below breaks down the extra design footage that was performed and itemizes
the other supplemental services associated with this design location.

Therefore, we are requesting Contract Amendment No. 2 to the Contract LVI (56)
Rehabilitation and Improvements Agreement in the amount of$22,979.40. This amendment
represents approximately a seven and one-half percent (7.36%) increase in the total contract
value. For your convenience, a summary of the total project fees are as follows:
