HomeMy WebLinkAboutContract 33939-A3 AMENDMENT NO. 3 TO AGREEMENT FOR ARCHITECTURAL DESIGN SERVICES GUINN ELEMENTARY SCHOOL RENOVATION STATE OF TEXAS CITY SECRETARY CONTRACT NO.33939 -A3 COUNTY OF TARRANT WHEREAS, The City of Fort Worth and Komatsu Architecture, Inc. (Architect) made and entered into City Secretary Contract No. 33939(The Contract)which was authorized by the City Council by M&C C-21619, the 15th day of August,2006; and WHEREAS, further amendment is necessary to reimburse the Architect for interiors build out part of the project as part of the design services agreement; and NOW THEREFORE, City and Architect, acting herein by and through their duly authorized representatives,enter into the following agreement which amends the Contract: 1. Article 1.1, SECTION l —SERVICES OF THE ARCHITECT, the following paragraph is added to read as follows: "Provide additional design and construction phase services to include Civil Engineering design services for a new primary electrical service to the building as a separate part of the project. The cost of this additional service is $13,470.00($12,970.00 plus$500.00 reimbursable). The scope of services is described more fully in the Komatsu letter dated March 12, 2008, subject: Guinn Elementary School which is attached hereto as Attachment "A" and made a part of this Agreement." 2. Paragraph 1 of SECTION V-COMPENSATION TO ARCHITECT, is amended as follows: "The total compensation for all of the assignments to be performed by Architect as described in CHARACTER AND EXTENT of ARCHITECT'S SERVICES hereof shall be a fixed fee of $235,379.62 plus up to$6,418.00 in reimbursable expenses. Project No. 04.149C February 25, 2009 Revised March 23, 2009 SCHEDULE A STATEMENT OF WORK FOR ADDITIONAL SERVICES For New Electrical Primary Service Guinn Elementary School Phase I Komatsu Architecture ("Consultant") shall perform additional Design Services for the City of Fort Worth ("Client") as outlined in this proposal. CONSULTANT and CLIENT sometimes are collectively referred to herein as the"Parties"and individually as a"Party". Execution of this statement of work will serve as Noticeto Proceed. Re: Guinn Elementary Building Primary Electrical Service Design Purpose: Consultant to provide civil and electrical design services consisting of the design of the electrical primary service bored in the easement per survey attached to this statement of work dated 01/26/09 to serve Elementary Building. The Consultant will additionally provide coordination among the other consultants involved in the current Phase I of the Guinn Elementary School as required to ensure the proper coordination of this new Primary Electrical Service Design with the current sitework related design issues of the Phase I scope. Assumptions: 1. The service disconnect will be engineered by Electrical Engineer providing the building interior electrical design. 2. All electrical service grounding will be provided by Electrical Engineer providing the building interior electrical design. 3. Building electrical requirements, with service disconnect location and sizing, will be provided by others. 4. City of Fort Worth and ONCOR have agreed to preliminary design approach. 5. City of Fort Worth will provide the location and sizing of the bore pits and the boring direction from each pit since they have indicated they wish to bore this utility to minimize surface disruption. A. Scope of Services 1. Design and production of the Construction Documents will be provided as outlined below. Consultant will provide Construction Documents at 100%completion for the Client's and City of Fort Worth review and approval on a date agreed to by both parties. Consultant will address all comments received from the Client's and City of Fort Worth review. 2. The electrical engineering design will include the following: • Electrical site utility from existing service point of connection to new utility transformer continuing to line side of service disconnect to building • Coordination of design with Oncor Utility Standards 1 of 3 Guinn Elementary School Phase One Komatsu Architecture, Inc. Project No. 04.149C February 25, 2009 Revised March 23, 2009 • Electrical Site Plan • Electrical Details • Technical specifications for electrical items designed • One meeting/site visit as required. 3. The civil engineering design will include the following: • Transformer pad location • Drainage/Grading Plan at new location • Erosion control plan • Boring location plan showing the approximate size, location of the pits and direction of bore. 4. The Consultant will provide coordination among consultants relevant to • The above stated scope for the new electrical service • The current Phase I scope related to sitework, landscaping work, site amenities. 5. Construction Administration will be provided on an hourly basis as need and as requested by the Client. Hourly rates for Consultants are attached hereto. Our proposed professional services DO NOT include the following: • Additional ONCOR design requirements outside of the proposed easement route or location • Design of any portion of the underground bore including bore pit size, boring direction, bore depth or utility location. • If the City either chooses not to bore this utility, or cannot bore this utility for whatever reason, the time and effort to create a paving plan, paving details or other required plan sheets will be completed on an hourly basis or under a separate contract as determined at that time. • Additional landscaping design pursuant to enclosure or otherwise screening the new transformer and pad site.
