S�P� (No M&C Needed) COUNTY OF TARRANT S WHEREAS, the City of Fort Worth (City) and CH2M Hill, Inc. , (Engineer) made and entered into City Secretary Contract No. 31312, (the Contract) which was authorized by the City Council by M&C C-20499 on the 25th day of January 2005; and WHEREAS, the Contract involves engineering services for the following project: Engineering Design Services: Water and Sewer Replacement on Southland Terrace Drainage Improvements, 2004 CIP Major Drainage, Project No. 00090; DOE No. 4740 Water Project No. P264-6091700090 Sewer Project No. P274-7091700090 WHEREAS, it has become necessary to execute Amendment No. I to said Contract to include an increased scope of work and revised maximum fee; NOW THEREFORE, City and Engineer, acting herein by and through their duly authorized representatives, enter into the following agreement which amends the Contract: 1. Article I of the Contract is amended to include the additional engineering services specified in a proposal letter dated August 17, 2006, a copy of which is attached hereto and incorporated herein. The cost to City for the additional design services to be performed by Engineer total $10,552. 50. 2. Article II of the Contract is amended to provide for an increase in the maxum-1m fee to be paid to Engineer for all work and services performed under the Contract, as amended, so that the total fee paid by the City for all work and services 014=126 -f"W y;1 'Jf exceed the sum of $160,315. 50. �' �;? We have received a proposal from ANA to perform this work. We feel that the ANA proposal is fair and will result in quality data that will enable us to proceed with the final design. Please review the enclosed estimated breakdown of cost for completing the work Also included is ANA's proposal for the scope of services. We request a budget increase in our Basic Services contract in the amount of$10,552.50 to complete the revisions requested. In order to expedite the final design of the project,please approve this proposal at your earliest convenience. Project Manager c: Carlon Small/COFW Brad Hudgens/CH2M HILL File ATTACHMENT A Subcontract No. 2 STATEMENT OF WORK City of Fort Worth Southland Terrace Drainage Improvements Project Additional Survey Services A.1 Background The City of Fort Worth(CITY)is planning to design and construct storm drainage improvements in the existing Southland Terrace subdivision located in the southern area of the City(PROJECT).Preliminary investigations define the project area as the area bounded by Cole Street on the east,Evans Avenue and Felix Street on the West,Hammond Street to the north,and Brown Drive and Cole Court to the south. A.2 Objectives The objective of this Subcontract is to provide survey data for existing sanitary sewer lateral lines in the project area. A.3 Scope SURVEYOR will perform field surveys to collect horizontal and vertical elevations and other information needed by ENGINEER in design and preparation of plans for the project. Information gathered during the survey shall include top of pipe elevations of sanitary sewer lateral lines at,or near,the property line at nine (9)locations as specified in Table 1. TABLE 1 Sanitary sewer lateral connection survey,locations Location Address 1 1053 Cole Ct. 2 4921 Cole St. 3 4909 Cole St. 4 4809 Cole St. 5 4769 Cole St. 6 4521 Cole St. 7 1009 Cardinal Ridge Ave. 8 1029 E.Spurgeon St. 9 1029 E.Boyce Ave. All field survey activity shall be performed under the direct supervision of a Professional Land Surveyor licensed in the State of Texas and shall adhere to Minimum Standards of Practice,TEXAS ADMINISTRATIVE CODE Part 29,as approved by the Texas Board of Professional Land Surveying..All work shall be conducted using equipment,personnel,and procedures that will insure compliance with the accuracy standards as defined below.It is the responsibility of the supervising Land Surveyor to ensure that all work under this agreement complies with all state and local regulations.All documents submitted shall bear the surveyor's seal,signature,and a certificate that all work was done under the surveyor's supervision and that all information contained in the document is true and is accurately shown. Horizontal Control work can be done using either standard surveying techniques or Global Positioning System(GPS) techniques meeting the specification requirements outlined in this scope. All horizontal control work shall comply with Second Order-Class 11 standard of accuracy for geodetic control as established by the FGDC Geospatial Positioning Accuracy Standards,Part 4: Standards for Architecture,Engineering,Construction (A/E/C),and Facility Management. Vertical Control work shall be Second Order-Class II,as outlined in the FGDC Geospatial Positioning Accuracy Standards,Part 4: Standards for Architecture,Engineering, Construction(A/E/C) and Facility Management. Horizontal control shall be referenced to the Texas State Plane Coordinate System NAD 83, using the appropriate zone for the region to be surveyed. Vertical control shall be referenced to North American Vertical Datum,(NAVD) 1988. Surveying will be performed to meet the City of Fort Worth standards,using GPS equipment and electronic total stations. SURVEYOR will locate and use Control Points previously established for the PROJECT. The SURVEYOR will be responsible for having underground utilities located prior to excavation. The SURVEYOR will be responsible for avoiding marked utilities,and will maintain a safe distance from all above-ground utilities. Prior to conducting survey,the SURVEYOR will notify all affected residents of the project in writing.The notification letter shall be on company letterhead and shall include the following:project name,SURVEYOWs Project Manager and phone number,scope of survey work,and schedule of survey activities. If it is necessary to collect the required survey data,the SURVEYOR will attempt to obtain permission for surveying through private property. SURVEYOR will immediately notify ENGINEER if not able to obtain permission. The SURVEYOR will comply with all federal,state,and local laws,statutes,and ordinances relating to the execution of the work. This requirement includes,but is not limited to, applicable regulations for minimum wage rates,nondiscrimination in the employment of labor,protection of public and employee safety and health,environmental protection,the protection of natural resources,fire protection,burning and non-burning requirements, permits,fees,and similar subjects. T i f The SURVEYOR will perform all work necessary to move personnel and equipment in and out,set up and remove equipment, and clean up the site. SURVEYOR will not demolish or remove existing sidewalk or curbs without prior approval from the ENGINEER and the CITY. The SURVEYOR will be responsible for repairing or replacing above-grade damaged property,and providing surface restoration within reason. SURVEYOR will complete restoration of excavation holes before close of business on the same day of the excavation. SURVEYOR will ensure that trafficability and property access is maintained at all times within the project area. SURVEYOR will ensure that no waste,rubbish,or other similar materials is left on the ground in the project area. A.4 Deliverables All plans,field notes,plats,maps,legal descriptions,or other specified documents prepared in conjunction with the requested services shall be provided in a digital format compatible with the electronic data collection and computer aided design and drafting software currently in use by the City of Fort Worth Department of Engineering.All text data such as legal descriptions,coordinate files,cut sheets,etc.,shall be provided in the American Standard Code for Information Interchange (ASCII)format,and all drawing files shall be provided in Autocad(DWG or DXF) format(currently Release 12),or as otherwise approved in writing by the CITY,and all data collected and generated during the course of the project shall become the property of the CITY. The minimum survey information to be included in the Autocad Drawing and/or delivered to the ENGINEER shall include the following: • A copy of original field notes and sketches and hard copy plots of the work map on a scale of 1"=40' (or as specified). • An ASCII file including survey point information. The format shall be Point Number, Northing,Easting,Elevation,Description separated by spaces or commas. • A unique layer in the Autocad drawing showing all survey points used or set while performing the survey; • Point numbers,point symbols and point descriptions shall be on separate levels conforming to the City's standards. A.5 Schedule The SURVEYOR shall complete the survey and provide all deliverables to the ENGINEER within two(2)weeks after Notice to Proceed(NTP). A.6 Compensation Compensation for the services set forth in this scope of work shall be as follows: A) Sanitary Sewer Lateral Survey 9 EA.@$975.00/EA = $8,775.00 Total = $8,775.00 The method of compensation shall be Fixed Unit Price,per the attached contract.
