WHEREAS, the City of Fort Worth (City) and Halff Associates, Inc.,
(Engineer) made and entered into City Secretary Contract No. 31529, (the
Contract) which was authorized by the City Council by M&C C-20557 on the
8th day of March 2005; and
WHEREAS, the Contract involves engineering services for the
following project:
Engineering Design Services: Street Reconstruction, Water and Sewer
Replacement on Robin-Denver Storm Drain Reconstruction, 2004 CIP Major
Storm Drain Projects, Project No. 00086; DOE No. 4895
Water Project No. P253-6071700086
Sewer Project Noe P258-7071700086
Paving Project No. C200-2074000086
WHEREAS, it has become necessary to execute Amendment No. 1 to said
Contract to include an increased scope of work and revised maximum fee;
NOW THEREFORE, City and Engineer, acting herein by and through their
duly authorized representatives, enter into the following agreement
which amends the Contract:

Article I of the Contract is amended to include the additional
engineering services specified in a proposal letter dated February
27,2006, a copy of which is attached hereto and incorporated herein. The
cost to City for the additional design services to be performed by
Engineer total $4,180.00.

Article II of the Contract is amended to provide for an increase in
the maximum fee to be paid to Engineer for all work and services
performed under the Contract, as amended, so that the total fee paid by

the City for all work and services shall not exceed the sum of
$143,370.00.
3.
All other provisions of the Contract which are not expressly amended

In response to your request, Halff Associates is pleased to present this Amendment far additional services for
design of the Robin-Denver Storm Drain Rehabilitation. Amendment No. 1R shall include the following
additional services:
• Additional design survey for approximately 9S0 LF of new alignment
A summary of the proposed budget for this revised amendment is as follows:
Desien Survey (Gorrondona & Associates + 10%) $4,180
TOTAL PROPOSED AMENDMENT NO. 1R $4,180
In accordance with the City of Fort Worth M/WBE participation goal, the anticipated M/WBE utilization for
Amendment No. 1R is 91%. Attached are Summary of Total Project Fees and Estimated Construction Costs
(formatted in accordance with E�chibits B-3 and B-3A of the original contract).

Dear Ms. Visintainer:
Gon•oiidona & Associates, Ine. is pleased to submit this proposal for professional land surveyiug
seivices for the above referenced project. The following iteuuzed surveying services are to be
provided by Gortondona & Assoc. Inc.:
1.) Establish additional horizontal and vertical control for the new route.
2.} Provide a topographic survey showing all existing features that would affect proposed pz-oject
route as defined by engineers map dated 2/24/2006. These features include: telephone poles,
power poles, all other utilities located on or above the ground, fences, retaining walls, water
meters, manholes, property corners, vaults, spri.nkler heads {visible} and structures in the close
proximity to the limifs of construction. Also locate all buildings, trees, steps and other
topographical features wbich would be of intarest to the property owner in discussing the plans
with the City must be shown accurately and drawn to scale. The topographic surveys shall
extend from right-of-way to right-of way 'vi streets and alleys or being 30 feet wide when the
route is nat with stzeet limits. Also, locate the above ground structures of the e�cisting storm
drain in the project area. The length of the suxvey is approximately 9501inear feet.
3.) Deliverables shall include an Autocad digital f�le of the existing features located, propert�
owner data, an ASCII file of the points located and a hardcopy of the field notes and
coordinates of points located,

Progress Report No. 10
2004 CIP Project — Robin-Denver Storm Drain Reconsti-uction Project
I. Progress to Report to Date:
A. KICKOFF MEETING — Held April 14, 2005. City Representatives Richard
Argomaniz, Joseph Bergeron, and Steve Eubanks met with Mike Moya and Ronda
Visintainer at the City of Fort Worth — meeting inoved to the site for informational
review/update.
B. SITE VISIT — April 14, 2005. Representatives from COFW (Argomaniz, Bergeron,
Eubanks) and representatives froin Halff Associates, Inc. (Moya, Visintainer) met at
Robin Street and walked/drove the reconstruction alignment.
C. CONCEPT PLANS —Concept report was submitted to the City of Fort Worth June
16, 2005.
D. DESIGN SURVEY — Gorrondona and Associates completed the field surveys April
29`�,
E. PRELIMINARY PLANS — Base maps with design survey information and existing
utilities are being compiled in an effort to ensure work on Preliminary Plans can begin
immediately upon notification of approval of the Concept Plan.
F. SITE VISIT — September 29, 2005. Bergeron and Visintainer visited the site to
investigate Alternative 2 as presented in second Concept Repoi-t.
G. CONCEPT PLANS — Revised Concept Report was submitted to the City of Fort
Worth October 4, 2005.
H. CONTRACT AMENDMENT — A contract amendment for additional design survey
was submitted to the City of Fort Worth November 11, 2005. The requested design
survey follows the route of Alternative 2 in the second Concept Report. The contract
amendment was revised and will be forwarded to the City March 1, 2006.
L PROJECT STATUS MEETING — Representative from Halff Associates (Visintainer,
Grimmett) met with Joe Bergeron in his office February 22. During that meeting, it
was generally agreed that Halff Associates will proceed with design of the storm drain
in the alley between Denver and Gould, south of Park Street. It was also agreed that
Halff will submit preliminary plans to the City the week of March 6, 2006.
