(the "Architect") made and entered into City Saor taiy Contract No. 45275 (The Contract)which was administratively ox*culedin |heamountof$27.220,00. WHEREAS, the City administratively executed Amendment No. 1 on July 31. 2014 in the amount of *22.373�00, WHEREAS, the City axo:uhed Amendment No. 2 on October 27, 2015 by Council approval of M&C C- 27517 in the amount of$147,099,00. WHEREAS, the City administratively executed Amendment No. 3 on yWooh 1` 2017 in the amount of $5,570.00, WHEREAS, The City administratively executed Amendment No, 4 on October 6, 2017 in the umountof $20,800,00, WHEREAS, The City administratively executed Amendment No. 6on July 26. 2018 in the amountof $72,000,00. WH EREAS, The City executed Amendment No. 6 on September 11. 2018 by Council approval of M&C C- 28847intheomountof$27`500.00. VVHEREAG, further amendment is necessary to reimburse the Architect for professional services for the project oopart of the design services agreement and NOW THEREFORE. 'IX 76102 Re: Proposal for A/E Servi;:;es Tridical Censer-.-Additional Services 05 Additional CA Services Sept 201£1 —Dec 2018 (closeout) First Floor Kennel/Lab Space-SD, CD and CA Pho-,e Services Dear Ronnie; Elements is pleased to provide to you this proposal for services associated with providing Architectural and Engineering services for the modificolions to the area of the first floor of the existing Tactical Center to accommodate the change in scope with install of dog kennels for the K-9 unit and the modificaliams for Lab functions. We understand that this work is being separated from the existing controc'uai work and completed thru.IOC contract outside of the existing proj=ucl schedule. In addition to the services for the Kennel/Lob orea, the existing project for construction Eras exceeded the timeframe as noted in our Additional Services 03 which defined construction thru August 2018. We now believe that the project will not complete until December 2018 which would include our closeout of 11-10 project. Based on our understanding of the project we anticipate the following services as defined below: SCOPE OF SERVICES EXISTING ELRICH CONTRACT Construction Administration Phase-- Elrich Contract(Original Contract) 1. CA services through December 2018 2. Provide record drawings based on the As-Builts from the C:ontroctor. 3. Closeout of the project under this contract. KENNEL/LAB PROJECT Schematic Design Phase 1. Review existing conditions on site and document cis needed for the scope of this project. 2. Tour example city kennel facility with Owner. 3. Develop Payout to illustrate generally the proposed modifications to include basic architectural elements. MEP and Structural to he a narrative of scope items at this phase. 4. Provide on opinion of probable construction cost for the proposed modificaliorls. 5. Submit SD package to COFW for review and approval. 6. Attend 1 Meeting with the COFW-FWPD to review. Design Development/Construction Documents Phase 1. Develop construction documents. We have assumed specifications on the drawings and not a separate project manual. 2. Allend 2 Meetings with the COFW-FWPD to review progress documents and obtain review comments during the course of this phase of the project. 4. Modify documents cis rnduired and final Construction Documents and provide to the city fordistribution cmd perrnitting. 5. Subrnit documents to TDI.R as is now outside of the scope of the original project. Construction Administration Phase We have assumed that construction will be vio JOG and as such our services during this phase are limited, I. Respond to any RFI's for clarification Io the docurrtents. 2. Review of Shop drawings and other submitlals as aii:rulatr;d in*-he amstrodion documents. 3, We have anticihaied 2 site visits during conslwction of the project by Architect's represerdalive and no site visits by the MEP engineers and Structural engineer_ If neecled/requested,site visit by Ihese disciplines can be provided as cin additional service. 4. Attend I punch visit with the;Gwner prior to final turnover ancr acceptance by Architect's representative only. 5. Provide Record Documenis hosed or-,os-buills provided by the contractor(JOC). Exclusions and Qualifications to This Proposal. I. Services requested in addition to those specifically outlined in this proposal will be identified and o fee proposal provided with approval required before continuing with those services, 2. We have not included any Owner/Architect/Contractor meetings during the construction phase of the project based on JOC delivery. 3. Any modifications aflef the layouts are approved in the SD Phose will be considered additional services. 4. We have not included providing an update to the opinion of probable construction cost developed in the SD Phase. 5. We have included TAS submittal with the assumption that permit submittal will be by City staff. 6. No other areas of the facility will be impacted or upgraded within the scope of this project. We have assumed that this facility is code compliant and that the city will not require other modifications. Expenses Reimbursable expenses cis outlined in the Master Conlract are not included in our labor fee and are estimated at$2,800. DELIVERABLE Deliverable for this project will include layouts for proposed solution and construction documents per a link for download. PROJECT TEAM Services will be by TES, Frank Neal,and Elemfmts. ILS and Elements are SBL certified firms. S(','HEI)UI.E A definitive;schedule for the deliverobles of the project will be provided upon execution of a delivery order.
