(No M&C Needed) s WHEREAS, the City of Fort Worth (City) and Brown & Gay Engineers, Inc., (Engineer) made and entered into City Secretary Contract No. 34091, (the Contract) which was authorized by the City Council by M&C G-15409 on the 26�' day of September 2006; and WHEREAS, the Contract involves engineering services for the following project: Engineering Design Services: Street Reconstruction, Water and Sewer Replacement on Merritt Street (White Settlement to 150' N. of Melbourne), Priddy Lane (White Settlement to Melbourne) and Westwind Drive ( Lake Country Dr to Skylake Dr), 2004 CIP Contract Year 4& 5, Contract 50, Project No. 00452; DOE No. 5401 WHEREAS, it has become necessary to execute Amendment No. 1 to said Contract to include an increased scope of work and revised maximum fee; NOW THEREFORE, City and Engineer, acting herein by and through their duly authorized representatives, enter into the following agreement which amends the Contract: 1. Article I of the Contract is amended to include the additional engineering services specified in a proposal letter dated December 22,2006, a copy of which is attached hereto and incorporated herein. The cost to City for the additional design services to be performed by Engineer total $13,158.00. 2. December 22, 2006 Page 2 Assuming that the average garbage truck represents 2.3 ESALS, we are designing our neighborhood streets so that they can handle approximately 200 garbage trucks a week. Based on experience from CONTRACT 35, the ESALS break point between a 6-inch and 7-inch concrete pavement section with a Drainage Coefficient of 0.7 is approximately 240,000 ESALS over the 25-year design life of the pavement or 9,600 ESALS per year which represents approximately 80 garbage trucks a week. For comparison purposes, it takes approximately 10 delivery trucks or 750 passenger cars to equal the ESAL load of one garbage truck. We propose that to perform one (1) 8-hour traffic count on Merritt Street (to serve as similar data on Priddy Lane) and one (1) 8-hour traffic count on Westwind Drive to determine an appropriate traffic volume and weight information utilized in determining ESALS for the design. Once the traffic count with vehicle information data is collected, we can provide our recommendations for ESALS over the design life of the paving. The City can then decide to use the more conservative minimum values from the City's Pavement Design Standard Manua12005 Edition, or the recommendation we provide. The proposed cost to perform the traffic data collection data and provide the ESAL recommendation is as follows: $1, 800.00 Traffic Count Data Collection (Two (2) 8-hour counts) $ 250.00 ESAL Recommendation $2,050.00 Total Proposed Amount for Traffic Data Collection As outlined in this proposal, we are proposing to provide these additional services for a total fee of $15,208 ($13,158 for geotechnical investigations and pavement design and $2,050 for traffic count data to determine ESAL). Krogness, P.E. Dear Mr. Krogness: PROPOSAL FOR: GEOTECHNICAL ENGINEERING INVESTIGATION AND PAVEMENT DESIGN CONTRACT 50 FORT WORTH, TEXAS INTRODUCTION CMJ Engineering, Inc. (CMJ) is pleased to submit a proposal for providing geotechnical engineering services in conjunction with the above-referenced project. We prepared this proposal based on the preliminary scope submitted to this office by Mr. Carl Krogness. This geotechnical investigation will be performed in accordance with the City of Fort Worth's 2005 Pavement Design Standards Manual. The project, as currently planned, wi(I consist of refurbishing specified portions of three streets in Fort Worth including: Merritt Street (White Settlement Rd. to Blackstone St.), Priddy Lane (White Settlement Rd. to Melbourne St.) and Westwind Drive (Skylake Dr. to Lake Country Dr.). Specific refurbishment lengths range from approximately 1,20Q to 1,500 linear feet with an approximate combined total project length of 4,100 linear feet. For purposes of this proposal, it is assumed that all borings are accessible to truck-mounted drilling equipment. ln addition, it is assumed that underground utilities at boring locations will be coordinafed by CMJ. Proposal No. 06-1756 December 19, 2006 Page 2 SCOPE OF SERVICES I. BASIC SERVICES A. SUBSURFACE EXPLORATION Experienced drillers and technicians will evaluate subsurface conditions with a total of eleven (11) continuously sampled borings distributed as follows: Merritt Street — Four (4) borings to 10 feet Priddy Lane — Three (3) borings to 10 feet Westwind Drive — Four (4) borings to 10 feet The field personnel will drill the borings using truck-mounted equipment. Cohesive and non- cohesive soil samples will be continuously obtained using 3-inch diameter Shelby tube sampler$ and 2-inch diameter standard split-spoon samplers, respectively. In additian, rock encountered will be evaluated by use of Texas Department of Transportation (TXDOT) cone penetration tests. A soils logger will extrude the samples in the field, check the samples for consistency with a hand penetrometer, carefully wrap them to preserve their condition, and return them to the laboratory for testing. A log of each boring will be prepared to document field activities and results. CMJ's personnel will stake the boring locations using normal taping procedures. Approximate locations of the borings will be shown on the plan of borings. Precise surveying of boring locations and elevations is not included in the cost estimate. These services may be provided as Additional Services upon request. At the completion of drilling operations, boreholes will be backfilled with cement grout and the upper two feet plugged with bentonite pellets. B. LABORATORY SERVICES Considering the planned improvements, anticipated soil conditions and geology, laboratory tests will be required for classification purposes, and to determine strength characteristics. The following types of tests are therefore recommended: � moisture content and soil identification • liquid and plastic limit determinations • percent passing No. 200 sieve • unconfined compression tests on soil • unit weight determinations • absorption pressure and/or one-point pressure swell tests • lime series test . soluble sulfate tests CMJ ENGINEERING, INC. Proposal No_ 06-1756 December 19, 2006 Page 3 • unconsolidated undrained triaxial compression tests The specific types and quantities of tests will be determined based on geologic conditions encountered in the borings. C. ENGINEERING SERVICES An engineering report will be prepared to present the results of the field and laboratory data together with our analyses of the results and recommendations. We will provide three copies of the report. The report will address: • general soil and ground-water conditions • earthwork recommendations • guidelines for pavement subgrade stabilization • pavement thickness design Items other than those specified above, which are revealed by these studies or are necessitated by a change in project scope, may require revised field, laboratory, and engineering services. These services, if required and requested, will be performed as Additional Services. Additional Services are described in Section II. D. COMPENSATION FOR BASIC SERVICES It is proposed that the Basic Services described above be performed on a unit price basis, in accordance with the attached Basic Services Cost Estimates. Based on the anticipated scope and the attached Basic Services Cost Estimates, the total cost of Contract 50 is $11,962. This cost for Basic Services will not be exceeded without prior authorization. The estimated costs shown in this proposal are based on the anticipated soil conditions. The final invoice will be based on the specific quantities drilled and tested. If unanticipated conditions are encountered during drilling, we will notify you accordingly. E. SCHEDULE FOR BASIC SERVICES Weather permitting, we plan to initiate these studies within seven days of receipt of notice to proceed, and anticipate that two working days will be required to complete the subsurface exploration for the sites (weather conditions permitting). You will receive the final report approximately three to four weeks following the completion of the field phase. We will make preliminary design data available sooner if necessary. AUTHORIZATION AND SCOPE Additional Services will be performed only if specifically requested and authorized by Client. Additional Services may consist of the following: • Additional subsurface exploration, including quantities or items other than described in Basic Services. • Bulldozer or other equipment services required to achieve access to boring locations. • Stand-by time or time in excess of one-half hour required for travel between boring locations. • Additional laboratory services, including quantities or items other than described in Basic Services. • Additional engineering services, including personnel time and expenses for items not specifically described in Basic Services. This may include, but is not limited to, additional meetings requested by Client or Client's other consultants, assistance to Client in dealing with regulatory agencies, preparation and engineering assistance in legal proceedings, and evaluation of alternative designs for the project or relocation of structure, following initial submittal of the geotechnical report. • Additional copies of the report, other than the number described in Basic Services. • Any other required or requested services authorized by Client, other than those specifically described in Basic Services. B. COMPENSATION AND SCHEDULE FOR ADDITIONAL SERVICES Additional Services, when authorized by Client, will be in accordance with our Schedule of Fees. Additional Services will be performed at reasonable times and within reasonable schedules as requested by Client. Authorized Additional Services will be billed as a separate item on invoices and a description of the Additional Services will be provided. III. TERMS AND CONDITIONS The scope of services will be performed pursuant to the attached CMJ Terms for Geotechnical Engineering Services, which is incorporated into this proposal. CMJ ENGINEERING, INC.
