WHEREAS, the City of Fort Worth (City) and Stream Water
Group, Inc., (Engineer) made and entered into City Secretary
Contract No. 34090, (the Contract) which was authorized by the
City Council by M&C C-21721 on the 26th day of September 2006; and
WHEREAS, the Contract involves engineering services for the
following project: �
Engineering Design Services: Street Reconstruction, Water and
Sewer Replacement on NE 37�' Street (Grover Ave to Deen Rd),
Decatur Avenue (Terminal Rd to Altamont Dr and Altamont Dr to
Elaine Pl), Hardy Street (Elaine Pl to Mineola Ave) and Selene
Street (Decatur Ave to Weber St) , 2004 CIP Contract Year 4& 5,
Contract 41, Project No. 00441; DOE No. 5396
WHEREAS, it has become necessary to execute Amendment No. 1
to said Contract to include an increased scope of work and revised
maximum fee;
NOW THEREFORE, City and Engineer, acting herein by and
through their duly authorized representatives, enter into the
following agreement which amends the Contract:
1.
Article I of the Contract is amended to include the
additional engineering services specified in a proposal letter
dated January 18,2007, a copy of which is attached hereto and
incorporated herein. The cost to City for the additional design
services to be performed by Engineer total $14,201.00.

Article II of the Contract is amended to provide for an
increase in the maximum fee to be paid to Engineer for all work
and services performed under the Contract, as amended, so that the
total fee paid by the City for all work and services shall not

We are pleased to submit this pr�pos�l to provide a geotechnical evaluation for the �E�oue-
referEnced projec#. This proposaf includes a de�cription of the project, our proposed s�ope of
work, and our proposed fee for completing our services,
pRQJECT Q��CRIPTION
The project wili cansist af Wat�r and Sanitary Sewer Replac�ment on Selena Street (Decatur
Avenue #� Weber Street), N. E. 37th Street (Grover Street to Den Road), H�rdy Street {Elaine �I
#o Mineola Avenue) and DEcatur Avenue (Termin�l Road to �laine PI) in Fort Worth, Texas. All
water and sewer lines will be less than 8 feet deep except the �2-inch water fine crossing under
the box culvert along Decatur Avenu� which wil( be about 15 feet deep below th� currant site
grades.
SCOPE 4F 1NORK
Field Investiqation
Four borings to a depth of 11 fee# will be drilled along Selena Street, four barings to a depth of
10 feet will be drilled along H2rdy Street, two b�rings to a depth of 10 feet will be drilled along
N.E. 37th Street and two borings to a depth of � 1 feet and one boring to a depth of 2Q feet at
box culverk crossing will be drilled alang Decatur Avenue. The borings will extend 3 feet below
the �lanned wate� and sewer lin� depth. The borings will be drilled through the existin�
ro�dway. The borings will be continuously sampled to a depth of 6 feet, and from 7 to 8 fe�t, 9
ta 10 feet, 11 to 12 feet, 14 fo 15 feet, 17 to 18 and 19'to 20 feet. Soil sart�ptes wilf be obtained
with thin-wa!!ed tub� sampfers. Rock will be tested in-pl�c� with a cone penetrometer, if
encountered: The borings-will be backfilled with soil cutting�.

It is assumed thafi it will be possible #o drill the borings using a conventional truck-mounted drill
rig. It is also assumed that the client will provide right-of-entry if �cr,ess through priva#e
praperty is required. (t is assurned that flagmen will nat be requi�ed to complete the field
in�estigation. We have assumed that signs and con�s will be required for traffic control_
Terra-Mar wi11 coardinate underground utility line Clearance with the Texas Excav�tion Saf�ty
System and the Ciry af Fort Worth. However; we wifl nat be responsible for damage fo
underground u#ility lines thaf are not identified by others prior to mobiliz�tion of drilling
equipment to the si#e.
Laboratory Investiqation
Th� sampfed soils will be tested in our faboratory to �valuate their pertinent engineering
properties. The type and number af faboratary tests r�quired to compfete aur evaluation will be
a function of the oonditions that are encount�red in the borings. However, it is envisioned that
the folfowing tests will be required as per the Pavement Design 5tandards Manual for the City
of Fort Wvrth:

The results of th� field investig�tion and labaratory testing programs will be evaluated to
provide recommendations for th� design and construction of the pr�pased water and sewer
lines and pavement. The repart will include the following informa#ion:
1. Boring (ags, laboratory test resul#s, water IEvel observations and boring location plans.
2. Summary of the paw�ment thickness at ea�h boring Ioca#ion,
3. Recommenda#i4ns for open trench excavation and lateral earth pres�ure envelopes for
design of trench shoring:

SCHEDULE
1N� are prepared t4 commence work on this project within a few days of authorization ta
pro�eed. It is anticipated that approximately 15 to ZO working days will be required to compiete
our evalu�tion. HQwever, we can adjust our schedul� if required to mee# your project
deadlin�s.
BUDGET
We propose to provide the above-described services on a time and materials basis. It is
estimated th�t the tat�l cost of our services will be on the order of $�2,910.00. A breakdown of
this estimated cost is presented on Table �. We will not �xceed #his estimated cost without yQur
prior written approval. Any addition�l services not included within the above scope can be
provided in accardance with the attached fee schedule.
