No. 3910: New York Avenue From Rosedale Rd. to E. Terrell St. CONTRACT 99JJ: Sewer and Water Line Replacements — Various Locations CONTRACT 99LL: Sewer and Water Line Replacements — Various Locations WHEREAS, it has become necessary to execute Amendment No. 2 to said Contract to include an increased scope of work and revised maximum fee; NOW THEREFORE, City and Engineer, acting herein by and through their duly authorized representatives, enter into the following agreement which amends the Contract: 1. Article I of the Contract is amended to include the additional engineering services specified in a proposal letter dated August 22, 2006, a copy of which is attached hereto and incorporated herein. The cost to City for the additional services to be performed by Engineer total $11,510.00 2. Article II of the Contract is amended to provide for an increase in the maximum fee to be paid to Engineer for all work and services performed under the Contract, as amended, so that the total fee paid by the City for all work and services shall not exceed the sum of $181,976.55. y ; • ', . 3. �'.l�i�,; __. _ S .. � , r . . The following is a description of the tasks that the ENGINEER will perform as part of Amendment No. 2. Work includes: Preparation of Plan and Pro�le sheets on 1"=20' Horizontal and 1"=4' Vertical scale for replacement of pavement, including curb, on Medina Avenue, Wosley Drive, and Wessex Avenue. Total length of improvements is approximately 4,955 LF. Station 0+00 to station 19+65 Medina Avenue: Provide P&P sheets showing existing top of curb, right of way and proposed top of curb profiles, proposed top of curb elevations at half and full stations in profile view and proposed driveway approach replacements in plan view. Maintain existing top of curb profile, where possible. Verify all driveway profiles for grade change not to exceed 15 %. Station 0+70 to Station 14+15 Wessex Avenue: Provide P&P sheets showing existing top of curb, right of way and proposed top of curb profiles, proposed top of curb elevations at half and full stations in profile view and proposed driveway approach replacements in plan view. Maintain existing top of curb proiile, where possible. Verify all driveway profiles for grade change not to exceed 15 %. Station 0+35 to Station 16+80 Wosley Avenue: Provide P&P sheets showing existing top of curb, right of way and proposed top of curb profiles, proposed top of curb elevations at half and Page 1 of 3 full stations in profile view and proposed driveway approach replacements in plan view. Maintain existing top of curb profile, where possible. Verify all driveway pro�les for grade change not to exceed 15 %. In addition, Plan and Profile sheets on 1"=20' Horizontal and 1"=4' Vertical scale for replacement of asphalt paving on Atkins Street and Riley Street. Total length of improvements is approximately 1,088 LF. Station 0+44 to station 7+30 Atkins Street: Provide P&P sheets showing existing and proposed asphalt centerline profiles, existing R.O.W. pro�les, proposed centerline elevations at half and full stations in profile view and typical cross section of paving with a straight cross slope, in plan view. Station 0+39 to station 4+41 Riley Street: Provide P&P sheets showing existing and proposed asphalt centerline profiles, existing R.O.W. profles, proposed centerline elevations at half and full stations in proiile view and typical cross section of paving with a crown (City of Ft. Worth Street Standards, Alternative (ETJ) Local - Option A Figure 11 and Residential Streets Figure 12.), in plan view. Verify type and condition of 36-inch existing drainage pipe, far reuse in this project. The following fee proposal is based on the current approved hourly rate schedule on City contracts. Amendment No. 1 was administratively approved on February 28, 20�5 in the amount of $23,732.00 for additional sanitary sewer and water line design requested by the Water Qepa�ment. This amendment, No. 2, provides for design of plans and profiles for various streets under Water Department Contracts 99JJ and 99LL requested by the Water Department for paving rehabilitation to be constructed under street maintenance program after the sanitary sewer and the water lines are constructed. These services were not included in the , original scope. City staff considers the proposed fee of $11,510.00 to be fair and reasonable for the scope of additional services � to be performed. This project is located in COUNCIL DISTRICTS 3,6,8 and 9, Mapsco 77K, T, X and 90E, J, L , U and 61T and 91 B, C. FISCAL INFORMATION/CERTIFICATION: The Finance Director certifies that funds are available in the current capital budget, as appropriated, of the Sewer Capital Projects Fund.
