Home My WebLink AboutContract 34801-A4� STATE OF TEXAS b COUNTY OF TARRANT S AMENDMENT NUi�ER 4 CITY SECRETARY CONTRACT NO. � —�� WHEREAS, the City of Fort Worth (CITY) and Teague Nall and Perkins, Inc., (ENGINEER) made and entered into City Secretary Contract No. 34801, (the CONTRACT) which was authorized by the City Council by M&C C-21918 on the 9th day of January, 2007; and subsequently revised by Amendment #1 which was authorized by the City on the 19th day of June, 2007; and subsequently revised by Amendment #2 which was authorized by the City Council on the 18th day of March, 2008 (M&C C-22712); and subsequently revised by Amendment #3 which was authorized by the City Council on the 3rd day of February, 2009 (M&C C-23325); WHEREAS, the Contract involves engineering services for the following project: Keller-Hicks Road from 750 feet west of Ridgeview Street to Park Vista Boulevard and North Beach Street from Golden Triangle Boulevard to Keller-Hicks Road; WHEREAS, it has become necessary to execute Amendment #4 to said Contract to include an increased scope of work and revised maximum fee; NOW THEREFORE, CITY and ENGINEER, acting herein by and through their duly authorized representatives, enter into the following agreement, which amends the Contract: l. Article I of the Contract is amended to include the additional engineering services specified in the proposal letter dated November 18, 2010, copies of which are attached hereto and incorporated herein. The cost to City for the additional design services to be performed by Engineer total $50,000.00. The following is a brief description and cost for the additional services provided on this project: Additional Services to provide right of way staking for UNIT 2: Keller Hicks Road (Alta Vista to Park Vista Boule�vard); Engineering Services related to Big Bear Creek Crossing options, FEMA map modification for UNIT 2:North Beach Street (Golden Triangle Boulevard to Keller Hicks Road) As requested, the right of way along Keller Hicks (Phase 2) was staked for the franchise utility relocation phase of construction. The additional engineering services include the evaluation of changes to the flood plain and hydraulic model based on the FEMA Map Modification effective September 2009. In addition to the Map Modification issues, multiple crossing options were evaluated and modeled to identify a solution that was in the City's best interest. The amendment is for $50,000.00, and it was my understanding that anything $50,000.00 and over requires Mayor and Council approval.
