AMENDMENT NO. 2
TO CITY SECRETARY CONTRACT NO. 30145
CONTRACT FOR PROFESSIONAL SERVICES
This Amendment to C�ntract for�g ssional Services (the "Amendment") is
made and entered into this `� day of -E3��e�-, 200�'� by and between the City of
Fort Worth ("City"), a home-rule municipal corporation situated in Tarrant, Denton and
Wise counties, Texas, acting by and through its duly authorized Assistant City Manager,
and McGriff, Seibels & Williams of Texas, Inc., a Texas corporation, (Broker).
WITNESSETH:
WHEREAS, City and Broker have entered into a certain Contract for Professional
Services, ("the Agreement") effective as of July 7, 2004 ("Effective Date"), same being
City of Fort Worth Secretary Contract ("CSC") No. 30145; and
WHEREAS, City and Broker subsequently amended CSC No. 30145 by entering
into Amendment No. 1, which is styled as CSC No. 33180 to modify the payment
schedule and certain services;
NOW, THEREFORE, in consideration of the foregoing and mutual covenants and
agreements contained herein and for other good and valuable consideration, the receipt
and sufficiency of which are hereby acknowledged, City and McGriff hereby agree to
amend the Agreement as follows:
SCOPE OF SERVICES
Section 1 of CSC No. 30145 is hereby amended to add Section 1.9 to read as follows:
1.9 Broker shall provide consultation services for City in developing bid requests to
secure a Workers' Compensation Health Care Network under the provisions of
the Texas Insurance Code, Chapter 1305, or an alternative Health Care Provider
arrangement under the provisions of the Labor Code, Chapter 504, Section
504.053. Services from Broker shall include assistance in developing Requests
for Quotes in accordance with the City's process through the Purchasing
Department, reviewing bid quotes and making a recommendation to the City.
The objective is to secure a health care network that is in compliance with
Insurance Code Chapter 1305 and Texas Department of Insurance certification
requirements, or other similar health care services agreement authorized by the
Labor Code Chapter 504 that allows for breakout services related to the
administration of Workers' Compensation claims.
2. TERM AND RENEWAL
Section 2 of CSC No. 30145 is hereby amended to read as follows:
2.2 City and Broker both mutually agree to extend the original Contract period as
stipulated by the original Contract under the same terms and conditions for the
period of January 1, 2007 through December 31, 2007.

2.3 This contract provides a one (1) year rate guarantee upon renewal at a 15%
maximum cap with the carrier reserving the right to review the City's Self-Insured
Retention (SIR) level.
3. PAYMENT FOR SERVICES
Section 4 of CSC No. 30145 is hereby amended to add Section 4.5 to read as follows:
4.5 The total of all payments and obligations made and incurred by City hereunder,
in consideration for consultation services rendered by Broker in developing bid
requests to secure Workers' Compensation Preferred Provider Network shall not
exceed the amount of Fourteen Thousand Nine Hundred Ninety Nine Dollars and
Ninety Nine cents ($14,999.99) as a Consultation Fee in addition to the Broker
services fee and Excess Workers' Compensation policy premium.
All other terms and conditions of City of Fort Worth City Secretary Contract No.
30145 that are not amended herein continue in full force and effect.
