TO CITY SECRETARY CONTRACT No.
WHEREAS, the City of Fort Worth (CITY) and Freese and Nichols, Inc. ,
(ENGINEER) made and entered into City Secretary Contract No. 34445, (the
CONTRACT) which was authorized by M&C C-21884 on the 12th day of
December, 2006 in the amount of $ $263, 346 . 00; and
WHEREAS, the CONTRACT was subsequently revised by: Amendment Number
1 in the amount of $576, 320 . 00 which was authorized by M&C # C-22439 on
October 9, 2007, Amendment Number 2 in the amount of $53, 210 . 00 which was
authorized by M&C # C-24149 on April 14, 2010, and Amendment Number 3 in
the amount of $216, 662 which was authorized by M&C # C-25311 on December
14, 2011,
WHEREAS, the CONTRACT involves engineering services for the
following project :
Precinct Line Road from Hurst Boulevard (State Highway 10) to Concho
Trail, Project No. 00147;
WHEREAS, it has become necessary to execute Amendment No. 4 to the
CONTRACT to include an increased scope of work and revised fee.
NOW THEREFORE, CITY and ENGINEER, acting herein by and through their
duly authorized representatives, enter into the following agreement,
which amends the CONTRACT:

Article I of the CONTRACT is amended to include the additional
engineering services specified in a proposal dated October 24, 2012, a
copy of which is attached hereto and incorporated herein. The cost to
City for the additional design services to be performed by Engineer
totals $10, 600. 00. (See Attached Funding Breakdown Sheet, Page B4A)

Article II of the CONTRACT is amended to provide for an increase in
the fee to be paid to Engineer for all work and services performed under
the Contract, as amended, so that the total fee paid by the City for all
work and services shall be an amount of $1, 120, 138. 00.

All other provisions of the Contract, which are not expressly
amended herein, shall remain in full force and effect.

Amendment#4—Additional Services—Proposal to revise engineering design for proposed 12-
inch Waterline along Precinct Line Road.

Dear Mr. Ingram:
Based upon our initial meeting with the City on February 24, 2012 and several follow-up meetings and
discussions regarding the Precinct Line Road project waterline plans,we are submitting a proposal to
perform additional services to prepare a full re-design of a 12"WL and addition of two (2)offsite
redundant waterline connections to the existing 33"WL, due to recent modifications from original scope
and design provided by the City.
Previously, FNI had submitted 90%plans to the City and were almost complete with the 12"WL design
when we received some major changes to the 12"WL design and plans at our February 20 meeting
with the City's Water Department. The changes set us back to a 30%level on the 12"WL plans,and we
will now be working towards a 60%and 90% level next with the Water Department to receive City
approval.
We propose performing the additional services listed below for$10,600.00.We have attached a
detailed scope of services as well as a fee estimate in the subsequent pages of this proposal.
Project Scope—Precinct Line Rd 12"WL Re-design
FNI will revise the design for relocating 1,915 linear feet of 12"WL from the south parkway of Precinct
Line Road to the eastbound traffic lane and raising the 12"WL profile per recent City comments due to
the fact that the roadway grading for Precinct Line Rd will now be completed before the WL is installed
by TxDOT. In addition to the original scope,the City has now asked that FNI provide two (2)offsite
redundant waterline connections;one (1)to the 8"WL that serves the Bell Helicopter site along Trinity
Blvd, and one (1)to the 10"WL along S. Precinct Line Rd. This will allow the 33"WL to be drained and
tapped by the new 12"WL within Precinct Line Rd,whereas before we were approved to provide a HOT
tap to the 33"WL, and now we are not by the City. This plan will keep water service to these two
existing offsite customers that are currently connected to the 33"WL within Trinity Blvd.

Task No. 1—Proiect Management
This task will include managing the team,communications and reporting. Managing the team will
include internal team meetings and QA/QC.Communications and reporting will include project update
meetings, design submittal review meetings, baseline schedule, monthly progress reports, schedule
updates and invoicing.
Task No. 2—Final Design
This task will include preparation of final draft construction plans and specifications, preparation of final
plans and specifications, updating the project decision log, and providing a construction estimate at the
60%,90%and final submittal.
Please note that this project will be resubmitted as part of the Precinct Line Road construction plans to
the Water Department at the 60%level,and will be designed in time to meet overall project and bid
schedule for the TxDOT Letting in December 2012.
Compensation
The compensation for these services shall be a lump sum fee of$10,600.00

We will be utilizing existing survey for the project for the revised scope in Amendment No 4.
With the additional services for waterline design the MWBE percenage will change from 32.6%to 32.3%.
