L S" WHEREAS, the City of Fort Worth (CITY) and Freese and Nichols, Inc. , (ENGINEER) made and entered into City Secretary Contract No. 34445, (the CONTRACT) which was authorized by the City Council by M&C C-21884 on the 12th day of December, 2006 in the amount of $263, 346. 00; and WHEREAS, the CONTRACT was subsequently revised by Amendment Number 1 in the amount of $576, 320 . 00 which was authorized by M&C C-22439 on the 9th day of October 2007, Amendment Number 2 in the amount of $53, 210 . 00 which was authorized by M&C C-24149 on the 14th day of April 2010, Amendment Number 3 in the amount of $216, 662 . 00 which was authorized by M&C C-25311 on the 14th day of December 2011, Amendment Number 4 in the amount of $10, 600 . 00 which was administratively authorized on the 30th day of November, 2012, Amendment Number 5 in the amount of $49, 166. 00 which was authorized by M&C C-26754 on the 8th day of April, 2014; and WHEREAS, the Contract involves engineering services for the following project : Precinct Line Road from Hurst Boulevard (State Highway 10) to Concho Trail (Project No . 00147 ) ; and WHEREAS, it has become necessary to execute Amendment No. 6 to said Contract to include an increased scope of work and revised fee. NOW THEREFORE, CITY and ENGINEER, acting herein by and through their duly authorized representatives, enter into the following agreement, which amends the CONTRACT: 1 . Article I of the CONTRACT is amended to include the additional engineering services specified in ATTACHMENT "A" Scope of Services, copies of which are attached hereto and incorporated herein. The cost to City for the additional design services to be performed by Engineer totals $48, 141 . 00 . (See Attached Funding Breakdown Sheet, Page -3- . ) 2 . Article II of the CONTRACT is amended to provide for an increase in the maximum fee to be paid to Engineer for all work and services Page -1- OFFICIAL RECORD CITY SECRETARY FT.WORTH, TX performed under the CONTRACT, as amended, so that the total fee paid by the City for all work and services shall be an amount of $1, 217, 445 . 00 . 3 . All other provisions of the CONTRACT, which are not expressly amended herein, shall remain in full force and effect . EXECUTED and EFFECTIVE as of the date last written by a signatory, below. Construction Phase Services—Precinct Line Road &Trinity Boulevard Task 2. Subsurface Utility Exploration at Lhoist Task 3. Redesign of Storm Drain Line F Task 4. Channel Design Revision Task 5. Emergency Services for Bridge Approach Water Line Design Task 5. Design Revisions of Water Line during construction TxDOT Roadway Construction TASK 1. CONSTRUCTION PHASE SERVICES. As the number of meetings provided in the previous amendment will exceed the 18 meetings authorized previously, we propose to attend an additional 36 meetings. This will provide for FNI to be able to attend most weekly meetings to keep up to date and provide necessary input on the latest developments in the project. TASK 2. SUBSURFACE UTILITY EXPLORATION FNI will subcontract to perform subsurface utility exploration to identify the water service line within the acquired Right-of-Way from Lhoist. TASK 3. REDESIGN OF STORM DRAIN LINE F In order to improve the constructability of storm drain line F, the contractor requested in RFI 37. to relocate the storm drain line. FNI reviewed the acceptability of this proposal and will redesign storm drain line F further north and with two outfalls into the creek. Storm drain sizing calculations, plan and profile layout and outfalls will be redesigned. Additionally, a custom junction box structure requiring structural design is required with this revision. TASK 4. CHANNEL DESIGN REVISION During construction, the channel lining material was requested to be revised. This task will include providing TxDOT guide details, review of material submittals and review of quantity adjustments. TASK 5. EMERGENCY SERVICES FOR BRIDGE APPROACH This task includes one site visit, one conference call, preparation of proposed remediation, preparation of traffic control plans and additional coordination regarding the undermined bridge approach on the south side of the existing bridge. ADDITIONAL SERVICES: Additional Services per previous contract and subsequent amendments are still valid. Water Line Design TASK 6. DESIGN REVISIONS OF WATER LINE DURING CONSTRUCTION Several revisions to the water line design have been requested after the bidding of the project. These design revisions include the following: • Water line relocation for waterline crossing at approximately Station 38+50. This water line was installed after the final plans were completed for the project and was installed in conflict with the proposed driveway. • Water line revision with Storm Drain Line F.Along with the contractor's request for the storm drain line F revision, the water line ring connection is to be revised north of the existing pavement to improve traffic control phasing. • Water line conflict with utility pole. A new utility pole was placed on top of the proposed design for the water line creek crossing. The design will be revised to split the distance between this pole and the nearby storm drain manhole. This will require revisions to the plan and profile design of the water line. o Effort also includes revision of one (1) easement document.
