HomeMy WebLinkAboutContract 49932-A2 RECEIVED 72019 AMENDMENT NO-2 CONTRACT NORI/ 7 20- 49 9, a-�a FEBF CIN OF FORT WORTH CITY SECRETARY TO CITY SECRETARY CONTRACT No. 49932 WHEREAS, the City of Fort Worth (CITY) and Dunaway Associates, LP, (ENGINEER) made and entered into City Secretary Contract No. 49932, (the CONTRACT) which was authorized on the 17 day of November, 2017 in the amount of $ 58, 825 . 00; and WHEREAS, the CONTRACT was subsequently revised by: Amendment Number 1 in the amount of $ 21, 700 . 00 which was authorized October 10, 2018; and WHEREAS, the CONTRACT involves engineering services for the following project : HAMMOND STREET DRAINAGE IMPROVMENTS - FINAL DESIGN AND CONSTRUCTION PHASE SERVICES; and WHEREAS, it has become necessary to execute Amendment No. 2 to the CONTRACT to include an increased scope of work and revised fee . NOW THEREFORE, CITY and ENGINEER, acting herein by and through their duly authorized representatives, enter into the following agreement, which amends the CONTRACT: 1 . Article I of the CONTRACT is amended to include the additional engineering services specified in a proposal dated December 18, 2018, a copy of which is attached hereto and incorporated herein. The cost to City for the additional design services to be performed by Engineer totals $19, 350 . 00 . (See Attached Funding Breakdown Sheet, Page -3-) 2 . Article II of the CONTRACT is amended to provide for an increase in the fee to be paid to Engineer for all work and services performed under the Contract, as amended, so that the total fee paid by the City for all work and services shall be an amount of $99, 875 . 00 . 3 . All other provisions of the Contract, which are not expressly amended herein, shall remain in full force and effect . (Dunaway) is pleased to submit for your consideration this proposal for additional professional engineering services on the above-referenced project. Based upon your request for the extension of existing proposed storm sewer in Timothy Road south of Spurgeon St, Fort Worth, Texas, we believe the following scope of professional services will address the City of Fort Worth (Client) needs for this project. Executive Fee Summary 1. Topographic Survey..............................................................$4,250.00 Hourly Not to Exceed 2. Construction Documents.......................................................$6,490.00 Hourly Not to Exceed 3. Subsurface Utility Exploration (SUE).....................................$8,610.00 Hourly Not to Exceed Total: Hourly Not to Exceed Services $19,350.00 FEE Dunaway proposes to provide the scope of work described below for a fee as shown above. All administrative and application fees required by review authorities will be paid by the Dunaway and are included in Dunaway's proposed fee. Please find attached to this proposal our Standard Terms & Conditions for professional services, which is also a part of this proposal. DETAILED SCOPE OF WORK 1. Topographic Survev Dunaway will provide a topographic survey of Timothy Road from Right of way to Right of way One-foot interval contours will be developed based on the results of the measurements taken. Existing visible utility facilities will be located and depicted on the face of the survey along with other visible improvements situated within the defined area. Construction Documents Dunaway will prepare construction documents for the public infrastructure improvements required for this project. The anticipated improvements consist of two inlets, laterals, and extension of the existing proposed stormwater trunk line in Timothy to connect to the proposed laterals. Design will include basic hyrologic and hydraulic calculations and review Existing proposed inlets See attached exhibit. As part of design, one (1) alternate configuration of junction boxes and inlets will be considered. These construction documents will be signed and sealed by a Registered Professional Engineer licensed in the State of Texas and will be processed with the City of Fort Worth for approval and construction.A tentative list of drawings anticipated to be created follows: Storm Drain Layout(Revision to Sheet No. 17) Storm Drain Profiles(Revision to Sheet No. 20) POL Sheet showing new extents of pavement(Revision to Sheet No. 23) Dunaway proposes to address up to one (1) round of review comments from City staff. If additional comments are made by City staff that are not indicative of design deficiencies caused by the design team, the time required to revise and resubmit the study will be tracked and billed on an hourly basis and is not a part of this proposed scope of work. 3. Subsurface Utility Exploration (SUE)—Dunaway has solicited a bid for a subsurface utility exploration (SUE) for this project by SurvWest. The SUE will identify franchise & public utility locations and potential conflicts with the proposed project. This work will be coordinated with the topographic survey to minimize utility conflicts during construction and increase the accuracy of the construction documents. Construction documents will show utilities in their relocated state. The scope of this proposal includes Quality Level "B"SUE along the proposed stormwater alignment. Utilities will be designated from right of way to right of way within the proposed street alignment. Designating of utilities on the private/city property adjacent to this proposed roadway extension is also included in the scope of work of this proposal. Utilities to be designated include both city (water, sanitary sewer, storm drains) and franchise (telecommunications, electric, gas, etc.) utilities. Quality Level "A" SUE is also included in the scope of work. For the purpose of this proposal, we have assumed that three (3) test holes will be required. Only those services specifically mentioned in the Scope of Work section are offered as a part of this proposal. ADDITIONAL SERVICES The following is a partial list of services that are not anticipated to be required for your project and are not proposed to be provided by Dunaway. Some of these services can be provided if they are desired or if it is later determined that they are necessary. In general, additional services will be authorized either by a contract amendment or a separate proposal. Boundary Survey beyond the right-of-way of Timothy Road 2. Survey services such as as-built surveys, platting and easements by separate instrument. 3. Revisions to drainage area map, Infoworks modeling revisions or verifications. 4. Franchise utility relocation coordination 5. ISWM revision and/or resubmittal 6.
