HomeMy WebLinkAboutContract 35352-A4 CITY SECRETARY CONTRACT NO. 1`5 LQ A AMENDMENT No.4 TO CITY SECRETARY CONTRACT WHEREAS, the City of Fort Worth (CITY) and Dunaway Associates, L.P. , (ENGINEER) made and entered into City Secretary Contract No. 35352, (the CONTRACT) which was administratively authorized on the 4th day of June, 2007 in the amount of $24, 500. 00; and WHEREAS, the CONTRACT was subsequently revised by: Amendment Number 1 in the amount of $165, 573.00 which was authorized by M&C C-23497 on May 5, 2009, and Amendment Number 2 in the amount of $25, 400. 00 which was administratively authorized on February 3, 2011; and Amendment Number 3 in the amount of $13. 527. 00 which was administratively authorized on December 22, 2011; and WHEREAS, the CONTRACT involves engineering services for the following project: Randal Mill Culvert Improvements, Project No. 00702; and WHEREAS, it has become necessary to execute Amendment No. 4 to the CONTRACT to include an increased scope of work and revised fee. NOW THEREFORE, CITY and ENGINEER, acting herein by and through their duly authorized representatives, enter into the following agreement, which amends the CONTRACT: 1 . Article I of the CONTRACT is amended to include the additional engineering services specified in a proposal dated February 28, 2012, a copy of which is attached hereto and incorporated herein. The cost to City for the additional design services to be performed by Engineer totals $3,959. 00. 2. Article II of the CONTRACT is amended to provide for an increase in the fee to be paid to Engineer for all work and services performed under the Contract, as amended, so that the total fee paid by the City for all work and services shall be an amount of $232,959. 00. 3. The additional work includes the following task: 1. Soil Boring for Temporary Retaining Wall — The phased construction of the proposed culvert will create the need for a temporary retaining wall to stabilize the embankment of the existing road. One 20-foot deep soil boring is required to provide soil characteristics for the temporary retaining wail design. The boring will be performed by Gorrondona & Associates for a fee of $3,599.00 in accordance with the attached scope of services. (G&AI) is pleased to offer this proposal for a geotechnical investigation for the referenced project. We prepared this proposal based on information provided. Project Location: The site is located along Randol Mill Road, approximately 0.15- miles east of its intersection with Goldeneye Lane, in Fort Worth,Texas. Proiect Description: The project consists of the following proposed improvement: • A culvert crossing. G&AI Proposal No.P12-0036 January 27,2012 Page 2 of 4 FIELD INVESTIGATION The proposed field investigation includes: No. of Depth per Location Borings Boring (feet)* 1 20 In the area of the proposed culvert crossing. (*} Note: Borings will be drilled to depth indicated or refusal, whichever is shallower. One boring totaling 20 feet of drilling is included in this scope of work. Field personnel will drill the borings using truck-mounted equipment. Cohesive and non-cohesive soil samples will be obtained using three-inch diameter Shelby tube samplers {ASTM D-1587} and two-inch diameter standard split-spoon samplers {ASTM D-158G}, respectively. An engineering geologist or soils technician will extrude the samples in the field, check the samples for consistency with a hand penetrometer, carefully wrap them to preserve their condition, and return therm to the laboratory for testing, A log of each boring will be prepared to document field activities and results. G&AI will stake the boring locations using normal taping procedures. Locations will be shown on the plan of borings. Precise surveying of boring locations and elevations is not included in the cost estimate. These services may be provided upon request, at additional cost. At the completion of drilling operations, bore holes will be backfilled and plugged with soil cuttings. LABORATORY INVESTIGATION Laboratory tests will be required for classification purposes, to determine strength characteristics, and to evaluate both the short- and long-term deformation/swell properties of the materials encountered. Testing will be in accordance with our standard procedures which include moisture content and soil identification, liquid and plastic limit determinations, strength tests on soil, and unit weight determinations. The specific types and quantities of tests will be determined based on soil conditions encountered in the borings. ENGINEERING SERVICES The engineering report will be prepared by a registered engineer and will present the results of the field and laboratory data together with our analyses of the results and recommendations. We will provide a digitally signed and sealed report in electronic PDF format. The report will address: • sail and groundwater conditions encountered at the baring locations; • culvert-related recommendations; Mr.Stephen James,P.E. G&A1 Proposal No.P12-0036 January 27,2012 Page 3 of 4 + earthwork recommendations, including material and compaction requirements; and • construction considerations related to soil and groundwater conditions at the borings. COST OF SERVICES Based on the scope of services described above, we propose a lump sum fee of $3,599.00 for the geotechnical investigation phase of the project. A detailed cost estimate is attached. This fee will not be exceeded without prior authorization. Items other than those specified above, or changes in drilling requirements, which are revealed by these studies or are necessitated by a change in project scope, may require revised field, laboratory, and engineering services. These services will be discussed and negotiated on an individual basis. The final invoice will be sent to you with our report. PROJECT SCHEDULE We plan to initiate these studies within five working days of receipt of notice-to-proceed and anticipate that one working day will be required to complete the field investigation (weather conditions permitting). You will receive the final report approximately 10 working days following completion of field operations. If you require a delivery time frame other than that presented above, please advise us at the time of completing the agreement so that we may make the appropriate adjustments in our schedule to accommodate your needs. Items to be provided by the client include the right-of-entry to conduct the exploration and information regarding the location of any utilities on the subject site. Any restrictions or special project requirements should be brought to our attention before we commence fieldwork. Should weather or other factors result in unforeseen changes in site accessibility, G&AI will contact the client to discuss accessibility options and associated fees. Please sign and return one copy of the proposal where indicated below as your authorization to proceed. By execution of this proposal, the undersigned Client acknowledges and agrees that the document entitled "Terms and Conditions" has been provided or made available to Client and Client agrees that such Terms and Conditions shall be applied to the present Proposal and shall be fully binding upon Client.
