RENEWAL OPTION NO. 2 TO CITY SECRETARY
CONTRACT NO. 30803
THIS Contract is entered into by and between the City of Fort Worth ("City"), a
home-rule municipality in the State of Texas, acting through Joe Paniagua, its duly
authorized Assistant City Manager, and Mental Health Mental Retardation of Tarrant
County ("Contractor"), by and through Susan Garnett, its duly authorized Chief
Operating Officer.
WHEREAS, the City and Mental Health Mental Retardation of Tarrant County have
entered into that certain agreement identified as City Secretary Contract No. 30803 for
a period of one (1) year, ending September 29, 2007; and
WHEREAS, said City Contract provides for three (3) consecutive one (1) year
renewal options;
WHEREAS,it is the mutual desire of the parties to exercise its second renewal option
for said Contract.
NOW, THEREFORE, THIS CONTRACT FURTHER WITNESSETH:
City Secretary Contract No. 30803,currently on file in the office of the City Secretary
of the City of Fort Worth is hereby amended to allow the term of the Contract to
extend to September 30, 2006 through September 29, 2007. Contractor's
compensation for the renewal period shall be four hundred twenty-seven thousand
eight hundred nine dollars and no cents ($427,809.00).
All other terms and conditions of City Secretary Contract No. 30803 remain in full
force and effect.

Exec to an Annual Evaluation Service Contract Funded by a Grant from the U.S. Department of
Health and Human Services for a Child Mental Health Initiative (CMHI) Co-operative Agreement
RECOMMENDATION:
It is r commended that the City Council authorize the City Manager to execute an annual evaluation service
contr;ct with the Department of Mental Health and Mental Retardation of Tarrant County (MHMRTC) in the
amount of $699,536 for evaluation services for the period September 30, 2004, through September 29,
2005 ?ontingent upon receipt of third year funding for this grant.
DISCUSSION:
The goal for the CMHI, "Children's Voices, Family Choices, Community Solutions: Building Blocks for
Healthy Families", is to create a new way of providing mental health services for a child with severe
emotional disturbances. The evaluation contract is one that is required by the funding agency (the
Substance Abuse and Mental Health Service Administration, or SAMHSA). MHMRTC fulfilled this role in
the last fiscal year as approved on October 21, 2003 on M&C C-19821 and is proposed to continue this
service. The evaluation will assess the effectiveness of a new system that is created to provide mental
health services. These services will be evaluated on accessibility, responsiveness to the target audience
and the extent in which services are driven by the child and family seeking care.
MHM TC was included and accepted by the granting agency as an approved participant during the grant
applic tion process. All funds for this contract are provided by the grant from the U.S. Department of Health
and H- man Services, which was first approved by the City Council on April 23, 2002 (M&C C-19063).
The scppe of work for this evaluation service contract is as follows:•
1. Conduct funded research designed to measure consumer outcomes and systems change;
2. Ensure that evaluation research projects are on time and on budget unless modified by funding source;
3. Aspess the impact of involvement in community partnerships in providing services to clients when
fu ded for evaluation;
4. Design and modify questionnaires and forms as needed;
5. Follow-up on clients who were interviewed during last fiscal year; and
6. Communicate research results to colleagues through journals, periodicals and other media and present
papers at conferences and institutes (may author articles, papers, and reports for publication
or in-house distribution).
In the n w process, families will play key roles in working with case managers in determining the care to be
provide . The same Wraparound facilitator (case manager) will stay with the family during the time the
child re eives mental health services and will assure that other service providers collaborate in providing
care to the child. The target population for this project is any Fort Worth Independent School District
school-a�ge youth between the ages of 4 and 18, who have been diagnosed wi a severe emotional
disturbance. These youth are further defined as being at-risk for removal from sch ol_or. home due to the
sympto s of their mental illness. r

Providers in mental health, education, juvenile justice, health, substance abuse and child welfare will
parti4ipate in developing this system change.
FISCAL INFORMATION/CERTIFICATION:
The Finance Director certifies that upon approval of the above recommendations and receipt of the
SAMHSA grant contract, funds will be available in the Fiscal Year 2004-2005 operating budget of the
Grants Fund.
