HomeMy WebLinkAboutContract 42723-A4 CITY SECRETARY CONTRACT NO._ �a7a 3-hq AMENDMENT No.4 TO CITY SECRETARY CONTRACT No. 42723 WHEREAS, the City of Fort Worth (CITY) and James DeOtte Engineering, Inc. , (ENGINEER) made and entered into City Secretary Contract No. 42723, (the CONTRACT) which was authorized by M&C C-25189 on the llth day of September, 2011 in the amount of $ 526, 623; and WHEREAS, the CONTRACT was subsequently revised by Amendment Number 1 in the amount of $149, 866 which was authorized by M&C C-27778 on June 21, 2016, Amendment 2 in the amount of $39, 300 which was administratively authorized on April 22, 2017, Amendment 3 in the amount of $35, 300 which was administratively authorized on November 2, 2017, and Amendment 4 in the amount of $49, 438 which was authorized by M&C C-29094 on April 16, 2019; and WHEREAS, the CONTRACT involves engineering services for the following project: Greenfield Acres Storm Drain Improvements; and WHEREAS, it has become necessary to execute Amendment No. 4 to the CONTRACT to include an increased scope of work and revised fee. NOW THEREFORE, CITY and ENGINEER, acting herein by and through their duly authorized representatives, enter into the following agreement, which amends the CONTRACT: 1 . Article I of the CONTRACT is amended to include the additional engineering services specified in a proposal dated March 11, 2019, a copy of which is attached hereto and incorporated herein. The cost to City for the additional design services to be performed by Engineer totals $49, 438 . 00 . (See Attached Funding Breakdown Sheet, Page -3-) 2 . Article II of the CONTRACT is amended to provide for an increase in the fee to be paid to Engineer for all work and services performed under the Contract, as amended, so that the total fee paid by the City for all work and services shall be an amount not to exceed $800, 557 . 3 . (JDEI) will perform the work outlined in the following items: Additional services shall include the re-design of approximately 1,000 LF of storm drain improvements along a portion of South Ridge Road as referenced and depicted as Segment A in the City exhibit "Greenfield Acres Drainage Improvements (Rev. 1/31/2019)". The design for the storm drain shall be based on the Greenfield Acres Neighborhood Drainage Improvements progress submittal plans dated August 14, 2015, with revisions based on previous comments and updated information. The specific tasks to be accomplished with this Amendment are outlined within the attached Level of Effort spreadsheet. JDEI anticipates the fees listed above for the various elements of the project to be a not to exceed amount of $49,438.00. All costs, associated with the identified scope of work, will be lump sum for the scope provided and will be billed as a percentage of completed work as determined by JDEI. This scope of work assumes any services not specifically listed as part of this contract will be treated as an additional service.
