S(,°I -AX (MSC Not Required) COUNTY OF TARRANT S WHEREAS, the City of Fort Worth (City) and CH2M Hill, Inc. , (Engineer) made and entered into City Secretary Contract No. 35696, (the Contract) which was authorized by the City Council by MSC C-22312 on the 14t' day of August, 2007 and subsequently revised by one previous amendment; and WHEREAS, the Contract involves engineering services for the following project: M-244B Emergency Sanitary Sewer Rehabilitation. WHEREAS, it has become necessary to execute Amendment No. 2 to said Contract to include an increased scope of work and revised maximum fee; NOW THEREFORE, City and Engineer, acting herein by and through their duly authorized representatives, enter into the following agreement which amends the Contract: 1. Article I of the Contract is amended to include the additional engineering services specified in a proposal letter dated September 271, 2010, a copy of which is attached hereto and incorporated herein. The cost to City for the additional design services to be performed by Engineer total $22,508.15. 2 . Article II of the Contract is amended to provide for an increase in the maximum fee to be paid to Engineer for all work and services performed under the Contract, as amended, so that the total fee paid by the City for all work and services shall not exceed the sum of $314,658.15. Because there is$13,603.75 of unauthorized funds remaining on the project,the requested additional contract funding is$22,508.15. The additional funding is needed to address additional design services. Project History The M-244B Emergency Sanitary Sewer Rehabilitation project has been successfully designed,bid and awarded. During construction,a portion of existing reinforced concrete pipe (RCP) failed during sliplining operations with high density polyethylene pipe (HDPE). The City has requested CH2M HILL evaluate the failure and provide additional design services for supporting the HDPE piping. Request for Additional Funding The following outlines our request for additional funding. Additional Meetings Since October,2008,over five additional meetings were held with the City to discuss options for supporting the HDPE piping. Additional geotechnical information was required to facilitate the design.CH2M HILL will deliver signed and sealed drawings for the selected option of drilled pier supports and concrete encasement of the piping. However,since there was$38,251.75 of unused funds left in the original contract, the actual cost of Amendment No. 1 was deducted leaving a balance of$13,603.75 b The actual cost of Amendment No.2 is$36,111.90. However,since there is$13,603.75 left in the contract,the actual cost of Amendment No. 2 has been deducted necessitating $22,508.15 in additional funding.
