WHEREAS, the City of Fort Worth (City) and Teague Nall and
Perkins, (Engineer) made and entered into City Secretary Contract
No. 35554, (the Contract) which was authorized by the City Council
by M&C C-22181 on the 12th day of June 2007; and
WHEREAS, the Contract involves engineering services for the
following project:
Quail Run Drainage Improvements, Project No. 00629
WHEREAS, it has become necessary to execute Amendment No. 2
to said Contract to include an increased scope of work and revised
maximum fee;
NOW THEREFORE, City and Engineer, acting herein by and
through their duly authorized representatives, enter into the
following agreement which amends the Contract:
1.
Article I of the Contract is amended to include the
additional engineering services specified in a proposal letter
dated September 9, 2010, a copy of which is attached hereto and
incorporated herein. The cost to City for the additional design
services to be performed by Engineer total $39,385.00.
2.
Article II of the Contract is amended to provide for an
increase in the maximum fee to be paid to Engineer for all work
and services performed under the Contract, as amended, so that the
total fee paid by the City for all work and services shall not
exceed the sum of $93,661.00.

Dear Mr. Simmons:
Teague Nall and Perkins, Inc. is submitting this Amendment to the above referenced contract based
on additional services requested. The following is a brief description and cost for the additional
services provided on this project:
Additional Services for Constructability Review
Prior to the 60 percent review meeting with the CITY, the ENGINEER shall schedule and
attend a project site visit with the CITY Project Manager and Construction personnel to walk
the project. The ENGINEER shall summarize the CITY's comments from the field visit and
submit this information to the CITY in writing.
For this project an additional review meeting is required with CITY Project Manager and
Engineering personnel to walk the project and assess potential design alternatives.

Additional Services for Traffic Control Plan
The original project scope was written around the T/PW specification that provides for the
contractor to prepare a traffic control plan as a non -pay item. Since that time the
specifications have been updated to require a traffic control plan developed by the Engineer
as a part of the project. The following scope will provide for the inclusion of traffic control
plans and construction phasi ng necessary to construct the bridge.
Engineer will develop a traffic control plan utilizing standard traffic reroute configurations
posted as "Typicals" on the CITY's Buzzsaw website as applicable. The typicals need not
be sealed individually, if included in the sealed contract documents.
Engineer will develop supplemental traffic control drawings as needed for review and
approval by the Traffic Division of the Transportation and Public Works Department. These
drawings shall be sealed by a professional engineer registered in the State of Texas. The
following sheets are anticipated:

Additional Services for Bid Phase Services
ENGINEER will support the bid phase of the project as follows.
The ENGINEER shall upload all plans and contract documents onto Buzzsaw for
access to potential bidders. Contract documents shall be uploaded in .pdf files and
the plans in .pdf and AM files.
The ENGINEER shall sell contract documents and maintain a plan holders list from
documents sold and downloaded from Buzzsaw.
The ENGINEER will develop and implement procedures for receiving and answering
bidders' questions and requests for additional information. The procedures shall
include a log of all significant bidders questions and requests and the response
thereto. The ENGINEER will provide technical interpretation of the contract bid
documents and will prepare proposed responses to all bidders questions and
requests, in the form of addenda. The ENGINEER shall upload all approved
addenda onto Buz zsaw and mail addenda to all plan holders.
Attend the bid opening in support of the CITY.
Tabulate and review all bids received for the construction project, assist the CITY in
evaluating bids, and recommend award of the contract.
Incorporate all addenda into the contract documents and issue conformed sets.
Provide 10 full size and 10 half size sets of conformed drawings to the City and 20
sets of specifications.

Additional Services for Construction Phase Services
ENGINEER will support the construction phase of the project as follows.
The ENGINEER shall attend the preconstruction conference.
After the pre -construction conference, the ENGINEER shall provide project exhibits
and attend public meeting to help explain the proposed project to residents. The
CITY shall select a suitable location and mail the invitation letters to the affected
customers.
The ENGINEER shall visit the project site at requested intervals as construction
proceeds to observe and repor t on progress.
The ENGINEER shall review shop drawings, samples and other submittals
submitted by the contractor for general conformance with the design concepts and

general compliance with the requirements of the contract for construction. Such
rMew shall not relieve the Contractor from its responsibility for performance in
accordance with the contract for construction, nor is such review a guarantee that
the work covered by the shop drawings, samples and submittals is free of errors,
inconsistencies or omissions. The ENGINEER shall log and track all shop drawings,
samples and other submittals.
As requested by the CITY, the ENGINEER shall provide necessary interpretations
and clarifications of contract documents, review change orders, and make
recommendations as to the acceptability of the work.
The ENGINEER shall attend the "Final" project walk through and assist with
preparation of final punch list.
■ No Additional Fee Required. These Services are included in the original contract.
Additional Services for Record Drawings
The ENGINEER shall prepare record drawings from information submitted by the inspector
and/or Contractor. The drawings shall be submitted as full size (22" x 34") mylar drawings
with an estimated total of 20 sheets.

Additional Services for Storrn Water Pollution Prevention Plan
For projects that disturb an area greater than one (1) acre, ENGINEER will prepare the
Storm Water Pollution Prevention Plan (SWPPP) required for the Project for use by the
Contractor during construction. ENGINEER will prepare drawings and details for proposed
SWPPP improvement that the Contractor must use during construction. Contractor will be
responsible for filing the SWPPP with appropriate regulatory agencies.
■ No Additional Fee Required. These Services are included in the original contract.
Additional Services for Subsurface Utility Engineering
The ENGINEER shall provide Subsurface Utility Engineering (SUE) to the Quality Levels, as
described below. The SUE shall be performed in accordance with Cl/ASCE 38-02. The
original contract provided for a Quality Level C for this project.
Level B (includes Quality Level C work performed to date)
1. Select and apply appropriate surface geophysical methods) to search for and detect
subsurface utilities within the project limits, and/or to trace a particular utility line or
system.
2. Based on an interpretation of data, mark the indications of utilities on the ground surface
for subsequent survey. Utilize paint or other method acceptable for marking of lines.
3. Unless otherwise directed, mark centerline of single -conduit lines, and outside edges of
multi -conduit systems.
4. Resolve differences between designated utilities, utility records, and surveyed features.

5. Recommend additional measures to resolve differences if they still exist.
Recommendations may include additional or different surface geophysical methods,
exploratory excavation, or upgrade to Quality Level A data.
6. As an alternative to the physical marking of lines, the ENGINEER may, with CITY's
approval, utilize other means of data collection, storage, retrieval, and reduction, that
enables the correlation of surface geophysical data to the project's survey control.
Level A
1. Expose and locate utilities at specific locations.
2. Tie horizontal and vertical location of utility to survey control.
3. Provide utility size and configuration.
4. Provide paving thickness and type, where applicable.
5. Provide general soil type and site conditions and such other pertinent information as is
reasonably ascertainable from each test hole site.

Additional Services for R®W/Easement Services
ENGINEER will support and engage a Registered Professional Land Surveyor to perform
actives related to drainage easements as outlined below, per direction and guidance from
the CITY's Project Manager.
Research
The ENGINEER shall determine easement needs for construction of the project.
Required temporary and permanent easements will be identified based on available
information and recommendations will be made for approval by the CITY.
Easement Preparation and Submittal
The ENGINEER shall prepare documents to be used to obtain permanent and/or
temporary easements required to construct the improvements.
Corners will not be set for easements.
The documentation shall be provided in conformance with the checklists and
templates available on the C ITY's Buzzsaw site.
Temporary Right of Entry Preparation and Submittal
Prior to construction, the ENGINEER shall prepare, mail and obtain Temporary Right
of Entries from landowners. It is assumed that letters will only be required for land
owners adjacent to temporary construction easements or who are directly affected
by the project and no easement is required to enter their property.

Additional Services for Storm Drain Outfall Re -Design
Based on direction received from the Storm Water Department, plan revisions will be made
to redesign the outfall structure from Quail Run to the channel outfall along the west side of
Burton Hill. The outfall will be designed as a 24-inch bore from the channel bank to the
northeast curb line of quail run. The line will run in an easement located between 5320 and
5324 Quail Run. The outfall structure will be designed to limit the potential for erosion in the
creek bed. The junction box structure at the upstream end will be designed as a deep
structure to allow minimal grade on the outfall pipe to prevent high velocities. Plan and
profile views of the new outfall will be re -developed based on supplem entary topographic
survey data. City details will be used in addition to special details for the junction box if
needed. Approximately one plan and profile sheet and one detail sheet will be required.
In addition to the outfall re -design, hydraulics will also include an evaluation of the existing
line that collects runoff from Bradford Park at a headwall structure located at the rear of Lot
5. The line also collects water from two on -grade inlets in Burton Hill and discharges in the
existing channel behind 5320 & 5324 Quail Run. The purpose of this analysis is to confirm
that water does not bypass the headwall structure and surcharge the Quail Run s ystem.
