��y25 `�p19 CITY SECRETARY CONTRACT NO. 419 0 R-h3 c`�y�Ec�ETP� AMENDMENT No . 3 TO CITY SECRETARY CONTRACT WHEREAS, the City of Fort Worth (CITY) and DeOtte, Inc. , (ENGINEER) made and entered into City Secretary Contract No. 41902 , (the CONTRACT) which was authorized by M&C C-24860 on the 12th day of April, 2011 in the amount of $84, 904 .00; and WHEREAS, the contract was subsequently revised by Amendment No. 1 in the amount of $26,745.00 which was administratively authorized September 28, 2015 and Amendment No. 2 in the amount of $19, 946. 00 which was administratively authorized on June 20, 2016; and WHEREAS, the CONTRACT involves engineering services for the following project: Water Improvements on Ridglea Country Club Drive; and WHEREAS, it has become necessary to execute Amendment No.3 to the CONTRACT to include an increased scope of work and revised fee. NOW THEREFORE, CITY and ENGINEER, acting herein by and through their duly authorized representatives, enter into the following agreement, which amends the CONTRACT: 1 . Article I of the CONTRACT is amended to include the additional engineering services specified in a proposal dated June 18, 2019 a copy of which is attached hereto and incorporated herein. The cost to City for the additional design services to be performed by Engineer totals $12 ,060 .00 . 2 . Article II of the CONTRACT is amended to provide for an increase in the fee to be paid to Engineer for all work and services performed under the Contract, as amended, so that the total fee paid by the City for all work and services shall be an amount of $143,205.00 OFFICIAL RECORD CITY SECRETARY Page -1- FT. (DI) and the City of Fort Worth(the City) dated June 22, 2011 and amended September 15, 2015 and August 25, 2016, herein referred to as the "Contract." This letter requests an amendment revising the previously approved subject project design plans and contract documents. Specifically, this amendment includes the provision of additional work associated with the following items; 1. One new additional Temporary Construction Easement. 2. One new additional Permanent Access Easement 3. Revisions to convert two Temporary Construction Easements to Permanent Access Easements. 4. Additional Revisions to the Final Plans and Specification to prepare them for bidding. Additional Survey The original contract work as amended did note include easements consistent with current policy regarding temporary easements so new permanent easements and revisions to two temporary easements are needed to make them permanent. 1. Survey revisions to two easements (Sub-consultant) $1,000 2. Prepare 2 new Permanent Access Easements (Sub-Cons.) $2,200 Engineering Plan and Specification Revisions Due to extensive inactivity on the project, various revisions were needed to the plans and specifications to conform to current policy. The Plans are also to be revised to accommodate the abandonment of a portion of the water line which ruptured recently. 1.
