COUNTY OF TARRANT S
WHEREAS, the City of Fort Worth (City) and Freese and Nichols,
Inc. (Engineer) made and entered into City Secretary Contract No.
35765, (the Contract) which was authorized by the City Council by
M&C C-22323 on the 21st day of August 2007; and
WHEREAS, the Contract involves engineering services for the
following project:
Engineering Design Services: Meacham Boulevard/Mark IV Parkway
Drainage Improvements, Project No. 00666; DOE No. 5689
WHEREAS, it has become necessary to execute Amendment No. 2 to
said Contract to include an increased scope of work and revised
maximum fee;
NOW THEREFORE, City and Engineer, acting herein by and through
their duly authorized representatives, enter into the following
agreement which amends the Contract:
1.
Article I of the Contract is amended to include the additional
engineering services specified in a proposal letter dated November
16, 2009, a copy of which is attached hereto and incorporated
herein. The cost to City for the additional design services to be
performed by Engineer total $16,170.00.

Article II of the Contract is amended to provide for an
increase in the maximum fee to be paid to Engineer for all work and
services performed under the Contract, as amended, so that the
total fee paid by the City for all work and services shall not
exceed the sum of $255,045.00.

not expressly amended herein shall remain in full force and effect.

Amendment No. 2
Dear Mr. Owen:
Additional services have been requested for the Future Storm Drain and Channel
Improvements at Meacham Boulevard, Mark IV Parkway and Franklin Drive (City
Project No. 00666, DOE No. 5689). The additional services include Level A SUE utility
location along Meacham Boulevard to determine exact locations of existing utilities that
may conflict with proposed storm drain improvements. The scope of work includes
approximately 14 borings to locate existing gas, electric, water, telephone and fiber optic
utilities. The total for the proposed services is $16,170 including administration fees.
This brings the total contract amount to $255,045. Please find attached proposal for

Test Holes
Dear Mr. Berman:
Gorrondona and Associates,Inc. (G&AI)is pleased to submit a cost proposal for Subsurface
Utility Engineering (SUE) required for the above referenced project. This proposal is based
on information contained in your email to Truett Wilson dated October 1,2009.
Introduction
G&AI will perform the SUE work required for this project in general accordance with the
recommended practices and procedures described in ASCE Publication CIlASCE 38-02
(Standard Guidelines for the Collection and Depiction of Existing Subsurface Utility Data).
As described in the publication, four levels have been established to describe the quality of
utility location apd attribute information used on plans. The four quality levels are as
follows:
• Quality Level D (QL"D")—Information derived from existing utility records;
• Quality Level C (QL"C') - QL"D" information supplemented with information
obtained by surveying visible above-ground utility features such as valves, hydrants,
meters,manhole covers,etc.
• Quality Level B (QU B")—Two-dimensional (x,y) information obtained through the
application and interpretation of non-destructive surface geophysical methods. Also
known as "designating" this quality level provides the horizontal position of
subsurface utilities within approximately one foot.
• Quality Level A (QL"A') — Three dimensional (x,y,z) utility information obtained
utilizing non-destructive vacuum excavation equipment to expose utilities at critical
points which are then tied down by surveying. Also known as `locating", this
quality level provides precise horizontal and vertical positioning of utilities within
approximately 0.05 feet.
It is the responsibility of the SUE provider to perform due-diligence with regard to records
research (QL "D'I and acquisition of available utility records. The due-diligence provided
for this project will consist of visually inspecting the work area for evidence of utilities and
reviewing the available utility record information. Utilities that are not identified through

these efforts will be here forth referred to as "unknown" utilities. G&AI personnel will scan
the defined work area using electronic prospecting equipment to search for "unknown"
utilities. However, G&AI is not responsible for designating and locating these "unknown"
utilities.
Scone of Work
Based on information provided by Freese and Nichols, Inc. (F&N), G&AI has developed a
preliminary scope for the SUE work required for this project. The scope of work may be
modified, with F&N's concurrence, during the performance of the SUE fieldwork if
warranted by actual field findings.
The scope of this proposal includes Quality Level "A" test holes on utilities located along
Meacham Boulevard. Five test holes will be excavated on a 30-inch diameter water line
running along the south side of Meacham Boulevard. The purpose of the test holes is to
locate the low point in the water line where it crosses under the existing stone drain culverts.
A sixth test hole will be excavated on a 16-inch diameter gas line that is also located on the
south side of Meacham Boulevard. The test hole locations are depicted in an exhibit
provided by F&N.
The test holes for this project will be surveyed by G&AI personnel utilizing survey control
provided by F&N.
Locating Procedures
G&AI will utilize its utility designating marks and the proposed utility alignments to layout
the test hole locations. GAI will establish routine/ordinary traffic control (signs, cones)
wheneN er required as part of our standard pricing. If non-routine traffic control measures
are required (flagman, arrowboard. barricades, etc.), these services will be considered extra
and will be invoiced at our cost.
G&AI will attempt to place the test holes outside of paved areas wherever possible. G&AI
will utilize non-destructive vacuum excavation equipment to excavate test holes at the
required locations. Once each utility is located. GAI will record the utility type, size,
material, depth to top, and general direction. Each test hole will be assigned a unique ID
number and will be marked with rebar/cap, nail disk, or chiseled X, as appropriate. The test
hole 1D number and other pertinent utility information will be placed at each test hole
location. The excavations will be backfilled with appropriate material and the original
surface will be restored. The backfill will be compacted in lifts by mechanical means to
prevent future settlement.
Deliverables
G&AI will prepare a test holes summary sheet along with the horizontal and vertical survey
data.
Schedule
Field work can CDmmence within approximately five (5) days after receipt of Notice To
Proceed (NTP) from F&N. G&AI estimates that the test holes can be completed in four (4)
days. Test hole summary sheet along with the survey data can be prepared and delivered
within five(5) days after completion of the field work.
Estimated Fees
G&AI proposes to perform the work as described herein, on a Unit Rate basis as follows:
Test Holes:
0—5 feet= $ 850.00 (assume 5 test hole)
5— 8 feet= $1,050.00 (assume 4 test holes)
8— 12 feet = $1,250.00 (assume 5 test holes)
The estimated cost to complete the 14 requested test holes at the depths indicated on the
provided profile sheet is fourteen thousand, seven hundred & 00/100 dollars
($14,700.00).
