Amendment 1 Request
Contract No. 35341
Dear Mr. Weiss,
I am writing this letter to request an amendment to our contract. This amendment will not impact
compensation; it will only reflect scope modifications that were negotiated but weren't revised in the
executed contract. The below scope revisions are requested to be reflected in our contract.
1. Public Involvement—meetings were reduced from 4 (four) meetings to 1 (one) meeting
2. Subsurface Utility Engineering - Quality Level of Service A was reduced from 7 (seven)
locations to 2 (two) locations.
3. Subsurface Utility Engineering - Mapping of existing overhead utilities was eliminated.
4. Subsurface Utility Engineering - instead of a lump sum fee; this work will be invoiced
based on time and materials not to exceed $73,575.00.
Please fell free to contact me with any questions you may have.
