TO CITY SECRETARY CONTRACT No. 3S8 - A LI
WHEREAS, the City of Fort Worth (CITY) and AECOM USA, Inc. (formerly
TCB, Inc. ) , (ENGINEER) made and entered into City Secretary Contract No.
35857, (the CONTRACT) which was authorized by M&C C-22390 on the 18th day
of September, 2007 in the amount of $ 37, 400 . 00; and
WHEREAS, the CONTRACT was subsequently revised by: Amendment Number
1 in the amount of $10, 600 . 00 which was administratively authorized on
February 27, 2008, Amendment Number 2 in the amount of $99, 770 . 00 which
was authorized by M&C #C-24228 on May 18, 2010, Amendment Number 3 in the
amount of $22, 565 . 00 which was administratively authorized on October 19 ,
2011; and
WHEREAS, the CONTRACT involves engineering services for the
following project :
Provine Drainage Improvements, Project No. 00703 ; and
WHEREAS, it has become necessary to execute Amendment No. 4 to the
CONTRACT to include an increased scope of work and revised fee.
NOW THEREFORE, CITY and ENGINEER, acting herein by and through their
duly authorized representatives, enter into the following agreement,
which amends the CONTRACT:

Article I of the CONTRACT is amended to include the additional
engineering services specified in a proposal dated December 14, 2012, a
copy of which is attached hereto and incorporated herein. The cost to
City for the additional design services to be performed by Engineer
totals $12, 760 . 00 . (See Attached Funding Breakdown Sheet, Page -3-)
2 .
Article II of the CONTRACT is amended to provide for an increase in
the fee to be paid to Engineer for all work and services performed under
the Contract, as amended, so that the total fee paid by the City for all
work and services shall be an amount of $183 , 095 . 00 .

All other provisions of the Contract, which are not expressly
amended herein, shall remain in full force and effect.

Provine Outfall Drainage Improvements
Supplemental Services Request#4
Dear Mr. Simmons:
At the 90%design review meeting on May 29, 2012, it was determined that additional services
outside the current scope of work are needed to be performed on this project. The following services
are required to complete the design of this project:
1.0 GEOTECHNICAL INVESTIGATION
AECOM will contract with Mas-Tek Engineering&Associates, Inc. (MTE)to conduct a geotechnical
investigation and present a geotechnical report with recommendations. MTE's proposal is included
as Attachment 1. Below is a summary of the scope of the geotechnical investigation:
• Utility clearance and traffic control plan;
• Two (2) borings to 35'depth—one located in Provine ROW, one located west of Ederville
Road ROW on City-owned park property;
• Groundwater observations;
• Laboratory testing, including Atterberg Limits, moisture content, unconfined compression,
swell, sieve analysis, unit weight, and sulfates; and
• Report with recommendations for open cut construction and trench shoring,tunneling
considerations, and construction considerations related to tunnel adjacent to existing
residences.
ASSUMPTIONS
• The City will provide access to the park property located west of Ederville Road.
• No lane closures on Ederville Road will be required.
AECOM will evaluate the recommendations provided in the geotechnical investigation and
incorporate necessary changes into final construction documents.
2.0 TECHNICAL SPECIFICATIONS
AECOM will prepare technical specifications for vibration monitoring and rock excavation to address
project-specific requirements for installing the proposed storm drain in the vicinity of an existing single
family residence. These specifications will be incorporated into the contract documents and
referenced on the project plans in the final submittal to the City.

COMPENSATION
Compensation for the scope of services described above will be provided on a Lump Sum basis in
accordance with the terms and conditions of the agreement dated 2 October 2007. A summary of
fee is provided below. Exhibit A includes a fee breakdown for the requested supplemental services.

Transmitted herewith is our proposal for a geotechnical investigation at the site referenced
above.
Project Requirements
• 42 inch storm drain pipe for discharge into lake
• Invert depths: 12'to 15'
• Open-Cut Construction Proposed
Scope of Work
• Traffic Control Plan
• Site Reconnaissance
• Staking of borings
• To prevent rutting to park property, either plywood sheeting will be used or the boring will
be drilled on Ederville Road requiring lane closure, traffic control and flagmen. The
actual boring locations will depend upon existing overhead lines and underground utility
lines.
• Two(2) Pavement Penetrations/Patching
• Mobilization of truck Mounted Rig
• Two (2) borings to 20'with truck mounted rig. Rock will be augered and evaluated using
cone penetration tests.
• Groundwater observations upon completion of each boring and end of day
• Laboratory testing (Atterberg Limits (Pis), moisture content, unconfined compression,
swell, sieve analysis, unit weight, sulfates)
• Report with recommendations for open cut construction and trench shoring, and
construction considerations related to open cut construction adjacent to existing
residence.
