CONTRACT FOR PROFESSIONAL SERVICES
This Amendment No. 3 t ntract for P s i S rvices (the "Amendment")
is made and entered into this — day of 2007 by and between
the City of Fort Worth ("City"), a home-rule municipal corporation situated in Tarrant,
Denton and Wise counties, Texas, acting by and through its duly authorized Assistant
City Manager, and Concentra Health Services, Inc., a Nevada corporation, d/b/a/
Concentra Medical Centers ("Concentra").
WITNESSETH:
WHEREAS, City and Concentra have entered into a certain Contract for
Professional Services, effective as of November 9, 2004 ("Effective Date"), same being
City of Fort Worth City Secretary Contract ("CSC") No. 30880; and
WHEREAS, City and Concentra subsequently amended CSC No. 30880 by
entering into Amendment No. 1, which is styled as CSC No. 33302 to modify the
payment schedule and certain services; and
WHEREAS, City and Concentra subsequently amended CSC No. 30880 by
entering into Amendment No. 2, which is styled as CSC No. 35356 to modify the
payment schedule; and
WHEREAS, CSC No. 30880, CSC No. 33302 and CSC No. 35356 are hereby
collectively referred to as the "Agreement."
NOW, THEREFORE, in consideration of the foregoing and mutual covenants and
agreements contained herein and for other good and valuable consideration, the receipt
and sufficiency of which are hereby acknowledged, City and Concentra hereby agree to
further amend the Agreement as follows:
Section 1.
Section 3 of the Agreement is hereby amended to include the following:
The City hereby exercises the third of four options to renew this Agreement for
one additional year. The term of this renewal shall commence on October 1, 2007 and
expire on September 30, 2008.
Section 2.
Appendix"B,"which is attached to the Agreement as Schedule for Compensation
is hereby amended to read as follows:

*90% of Texas 'Fee schedule until a Workers' Compensation Healthcare Network is
implemented by the City at which time the discount rate may be negotiated directly
between the City or its third party Workers' Compensation Healthcare Administrator
and Concentra.
Section 3.
All other terms and conditions of City of Fort Worth Contract No. 30880, as
amended that are not ame ded herein� continue in full force and effect.

Authorization to Execute the Third Renewal of the Agreement with Concentra Medical Centers as
the City's Preferred Provider for Occupational Health Care Services
RECOMMENDATION:
It is recommended that the City Council authorize the City Manager to execute the third renewal of the
Agreement with Concentra Medical Centers as the City's preferred provider for occupational health care
screener.
DISCUSSION:
On September 14, 2004, (M&C C-200266) the City Council authorized the City Manager to execute a
contract with Concentra to provide occupational health care services to the City of Fort Worth to treat
employees injured on the job. The City's current contract with Concentra began October 1, 2006, and
expires September 30, 2007. The third of four one-year options will begin October 1, 2007 and expire
September 30, 2008.
Concentra is one of the nation's largest occupational health care providers and provides health care
services to the City for both occupational injuries and non-occupational services that include drug screens,
vaccinations, Department of Transportation physicals, and pre-employment physicals.
Concentra's occupational health care services for City employees are invoiced to and paid on behalf of the
City by Sedgwick CMS/CMI Barron Risk Management, the City's contracted Third Party Claims
Administrator. It is anticipated that Concentra's cost for non-occupational health care services for Fiscal
Year 2007-2008 will be approximately $235,000.
Concentra is in compliance with the City's M/WBE Ordinance by committing to 10 percent M/WBE
participation.
FISCAL INFORMATION/CERTIFICATION:
The Finance Director certifies that funds will be included in the proposed Fiscal Year 2007-2008 budget of
the Workers' Compensation Insurance Fund.
