A Subscription Form under NewsBank License Agreement known as Contract No.CSC No.40795(the"Contract")
Renewal Term No.3:July 1,2019 through June 30,2020
The above referenced Contract covering the subscription renewal for America's News, Genealogy Bank, and Historical Newspapers
expires on June 30,2019. Pursuant to the Contract,subscription renewals are at the mutual agreement of both parties. This letter is to
inform you that the City desires to renew the subscriptions to America's News, Genealogy Bank, and Historical Newspapers for an
additional one year period, which will begin immediately upon the expiration of the current term and will end on June 30, 2020. All
other terms and conditions of CSC No.40795 remain unchanged.Please return a signed acknowledgement letter,along with a copy
of your current insurance certificate,to the address Fort Worth Library,Attn: Aaron Cummins,500 W.3rd St.,Fort Worth,
TX 76102, acknowledging receipt of the Notice of Subscription Renewal. Notwithstanding the foregoing, this Contract may be
executed in multiple counterparts,each of which shall be deemed an original,but all of which shall constitute one and the same Contract.
Facsimile signatures and/or electronically scanned signatures that are transmitted by e-mail shall be as binding on the parties as original
signatures.Once fully executed,a copy of this subscription renewal notice will be sent to NewsBank in order to generate an invoice.

company information is correct and up-to-date.
