Complete each Table as applicable to your Proposed Project. Costs must be broken out in
Tables to a degree that is sufficient to determine if costs are reasonable, allowable, and
necessary for the successful performance of the grant project. Costs will be reviewed for
compliance with UGMS and federal grant guidance found in 2 CFR Part 200, Uniform

Veteran, their dependents, and surviving spouses that receive grant-funded service(s).

cumulative totals, will be reported to the FVA quarterly.

CAP currently uses Shah Software, The New Gen System Database , which is a client tracking system
for data collection and performance measure. The Home Improvement and Construction Section will
use Happy Software for client tracking, work write-up and construction management.

New Gen system is capable of generating reports to ensure the numbers are unduplicated. A separate
component can be added to the system for the Healthy Homes for Heroes Program. The Happy software

Describe the Proposed Project. The answers should be brief but specific.

Through the Healthy Homes for Heroes Program, the City of Fort Worth will provide home modification
assistance to low income (80% of AMI) and/or disabled veterans residing in Tarrant County. There are
no fees assessed for the home modifications. Home Modifications to ensure healthy homes will include
accessibility modifications such as ramps, grab bars, bathroom modifications, door widening,
handrails, as well as health and safety items such as heating and air, water and sewer, unstable rotten
flooring, electrical system failures, rotten/moldy siding replacement and leaky roofs. Other health and
safety modifications no listed will be reviewed and approved by management on a case-by-case basis.
The Program will be a partnership between different sections of the Neighborhood Services Department
of the City of Fort Worth and other community partners such as Six Stones (a local non-profit agency),
Tarrant County, Habitat for Humanity, etc. Habitat for Humanity is a procured contractor, we refer to
them if there is something we cannot handle. The Community Action Partners section of the
Neighborhood Services department will provide case management and collect eligibility documentation.
The Home Improvement and Construction section will perform inspections, prepare scopes of work and
oversee rehabilitation construction contractors to ensure quality of work.

Program services will be posted on the City of Fort Worth's website along with contact information,
listed with United Way's 2-1-1 assistance service and marketed to local social services providers.
Beneficiaries can access the program by calling CAP's Customer Care Center at 817-392-5790 24 hours
a day, 7 days a week. Beneficiaries will be screened for preliminary eligibility and then will be directed
to either applying via mail, applying online or setting up an appointment. Veterans have one dedicated
day per week that is only for veterans, but can make an appointment any day that best suits their needs.
If a beneficiary makes an appointment they will be assigned to one of the CAP Centers mentioned under
the location section. Beneficiaries will meet with a Case Worker to ensure they meet program eligibility
requirements and to assess the beneficiary's needs.

What is the community need(s) or existing service gap(s)that the Proposed Project will address? (Maximum
allowable characters = 1500)
According to Census data, 23,146 of Tarrant County veterans have service connected disabilities. Out
of these, 13,270 have a disability rating of 30—70% or above, making it more likely that these veterans
are in need of accessibility modifications for their homes.There is no waiting list, however there is one
day per week dedicated for only veterans. However, veterans can make an appointment based on their
availability.
Additionally, 41,751 of veterans are senior citizens (65 and older)who frequently need accessibility or
health and safety modifications for their homes.
A Needs Assessment for Veterans in the Dallas-Fort Worth (DFW) Region states that housing accounts

accessibility modifications for Fort Worth residents and residents throughout Tarrant County.

The Healthy Homes for Heroes Program will assist veterans with needed home modifications.
According to the Census Bureau, 39.1% of the owner-occupied housing in Tarrant County was built
prior to 1979,this is a relatively high percentage of older homes, which become in need of repair or
replacement of most major systems once the housing is 30 years old or older. The goal of the Healthy
Homes for Heroes Program is to provide assistance to these homeowners. The Program will assist with
filling in some of these gaps by providing the health and safety repairs that beneficiary homes are in
desperate need of, in addition to accessibility improvements.

The program is unique because of the internal and external partnerships. Six Stones and other community
partners will provide assistance with repairing exterior of homes as well as other repairs that are not
covered by the grant. These partnerships create an opportunity for more comprehensive services while
leveraging funds from different funding streams. The Community Action Partners and the Home
Improvement and Construction sections of the Neighborhood Services Department provide an array of
services, including job training, case management, weatherization repairs, lead abatement, health and
safety related repairs among other benefits. Program beneficiaries will have access to the services
provided by both sections of the department if they qualify, giving them a unique opportunity for additional
services and leveraging funding to provide more comprehensive services. Additionally, the program will
provide services to all veterans regardless of era, branch of service, duty status and specific character of
discharge.

Outreach is performed by Public Education Specialists throughout the county at Health Fair events,
Community events and Job Fairs. Clients are also recruited through direct advertising (flyers) sent to
churches, apartment complexes and other venues or through partner agencies such as United Way 211.
Social media will also be used, i.e. Facebook and twitter. Additionally, veteran services organizations
will be contacted directly to market the program and generate referrals as applicable. Information and of
flyers will also be provided at 21 local community centers.

Marketing and outreach activities are conducted monthly.We attend the bi-monthly resource round-up
CLC, Inc. hosts. We also attend the IWAG (inter-Agency Workforce Group) monthly meetings. The CAP

provided at all of our CAP offices as well as taken to our partnering agencies. Our Communications
Department also posts the flyers on the NextDoor app as well as make announcements on their
Community Engagement Bulletin.

repairs (Priority Repair Program), however, the cost limit is $5,000 and the income limits are 60%
instead of 80%. The City also funds a non-profit agency (REACH) that provides accessibility
modifications, however, they do not widen doors.Additionally, both of these programs are open to the
general public and not specific to veterans and their households.

The City received a Housing for Texas Heroes Grant for the 2018-2019 grant period. To date,we have
inspected 8 homes that are in need of repair including a veteran whose home needed extensive (over
$20,000) repairs. He was referred to our Weatherization program who will assist with repairing the
heating system, Tarrant County will assist with replacing the roof, a CAP repair program will assist with
gas line repairs and the Texas for Heroes Grant will assist with replacing rotten/moldy siding. The client
was receiving Code Compliance warnings and was afraid of losing his home, without the grant the
client would have no hope of bringing his home up to code and living in safe conditions, the maximum
amount was reached for each of the different programs helping him due to the extent of repairs that
were needed. Our goal is to receive the Housing for Texas Heroes so that we can continue to provide
urgently needed home modifications and repairs toTarrant County's low-income/disabled veteran
homeowners.

RECOMMENDATION:
It is recommended that the City Council:
1. Authorize the application to the Texas Veterans Commission for two Fund
for Veterans' Assistance Grants,a General Assistance Grant in the amount
of$300,000.00 and a Housing for Texas Heroes Grant in the amount of
$300,000.00;
2. Authorize the execution of all applicable contracts;
3. Waive indirect costs; and
4. Adopt the attached appropriation ordinance increasing estimated receipts
and appropriations in the Grants Operating State Fund in the amount of
$600,000.00 upon notification of award.
DISCUSSION:
Two grant applications to the Texas Veterans Commission are being submitted
for activities that will benefit veterans in Fort Worth and Tarrant County.
The Forward Home Veteran Assistance Program funded by the General
Assistance Grant will provide emergency financial assistance such as
deposits,rental/mortgage and utility payments that are short-term and
temporary in nature. The grant will be administered by the Community Action
Partners (CAP) section of the Neighborhood Services Department. The
increased funding will enable CAP to provide additional needed services to
area veterans.
The Housing for Texas Heroes Grant will be utilized in partnership with 6
Stones to provide needed accessibility or safety modifications to veteran
homes in the Tarrant County area.
A waiver by the City of indirect costs will maximize program benefits to
veterans and surviving spouses. During 2017-2018, the Forward Home grant
was able to assist 175 veterans/dependents and surviving spouses. This
year,through September 2018,the program has served 50 households for a
benefit average of$758 per household. These funds were used to stabilize
veteran households,preventing homelessness. Waiver of the indirect cost
would allow the program to serve an additional 13 veteran households.
The estimated indirect costs that are being asked to be waived is
$10,000.00. If awarded,both grants will begin on July 1,2019 and end on
June 30,2020.
This contract will be with a governmental entity, state agency or public
institution of higher education: Texas Veterans Commission.
FISCAL INFORMATION/CERTIFICATION:
The Director of Finance certifies that,upon award of the above
recommendations,receipt of the grant funding and adoption of the attached
appropriation ordinance, funds will be available in the Grant Operating
State Fund, as appropriated.
This is a reimbursement grant. The Neighborhood Services Department will be
responsible for the verification and submission of documents for these
funds. Prior to expenditure being made, the Neighborhood Services
Department has the responsibility to validate the availability of funds.
