ADVANCE FUNDING AGREEMENT
AMENDMENT O
THIS AMENDMENT IS MADE BY AND BETWEEN the State of Texas, acting through the Texas Department of
Transportation, hereinafter called the State, and the City of Fort Worth, acting by and through its City Council, hereinafter
called the Local Government.
WITNESSETH
WHEREAS, the State and the Local Government executed a contract on November 19, 1996 to effectuate their agreement
to construction intersection improvements at Hulen Street at IH 20; and,
WHEREAS, Supplementa!Agreement No. 1 was executed on August 4, 1999 and increased the total funding on the
project from $240,000 to$492,000; and,
WHEREAS, Supplemental Agreement No. 2 was executed on January 26, 2000 and it changed the funding percentages
on the project to 64% federal, 3.9% state, and 32.1% local; and,
WHEREAS, Transportation Improvement Program Modification No. 2004-157 was approved on April 8, 2004. It changed
the scope of work of the project and increased the funding on the project by$852,100 to$1,344,100; and,
WHEREAS, it has become necessary to amend that contract to recognize the change in the scope of work, increase in
cost and the addition of an on-system CSJ for that portion of the project within the right of way of IH 20;
NOW THEREFORE, in consideration of the premises and of the mutual covenants and agreements of the parties hereto,
the State and the Local Government do agree as follows:

Article 1. Description of Amended Items
Page 2 of 13, Article 2 Scope of Work—The scope of work for this project consists of the construction of:
• dual left turn lanes on northbound and southbound Hulen Street at IH 20.
• dual left turn lanes on northbound and southbound Hulen Street at Barwick.
• right turn lane from IH 20 eastbound frontage road to Hulen Mall entrance.
• sidewalks and sidewalk ramps will be included at the intersections.
• traffic signals will be modified to accommodate the new turning lanes.

Article 2. Signatory Warranty
The signatories to this amendment warrant that each has the authority to enter into this agreement on behalf of the
organization they represent.
IN WITNESS WHEREOF, THE STATE AND THE LOCAL GOVERNMENT have executed duplicate counterparts to
effectuate this agreement.
