FOR ARCHITECTURAL DESIGN SERVICES

COUNTY OF TARRANT
WHEREAS, The City of Fort Worth and GRW Engineers, Inc. (Architect) made and entered into City
Secretary Contract No. 35474 (The Contract) which was authorized by the City Council by M&C C-22190,
the 19th day of June, 2007; and
WHEREAS, further amendment is necessary to reimburse the Architect for additional design services
relative to preparation of FAA form 7480-1 for the helipad at the new Police Air Support Helicopter Facility
and designing to FAA Standards; and
NOW THEREFORE, City and Architect, acting herein by and through their duly authorized
representatives, enter into the following agreement which amends the Contract:
1.
Article 1.1, SECTION I —SERVICES OF THE ARCHITECT, a new paragraph is added to read as follows:
"Provide additional design services to prepare a FAA Form 7480-1 for the helipad at the new
Police Air Support Helicopter Facility at Meacham Airport and in addition, to design the helipad
implementing the FAA Standards. The cost of this additional service is $6,760.00. The scope of
services is described more fully in the GRW Engineers, Inc.'s letter dated February 21, 2008,
subject: FAA Requirements for Helipad Police Helicopter Facility, Project No. 03480-02 which is
attached hereto as Attachment"A" and made a part of this Agreement."

Paragraph 1 of SECTION V- COMPENSATION TO ARCHITECT, is amended as follows:
"The total compensation for all of the assignments to be performed by Architect as described in
CHARACTER of ARCHITECT'S SERVICES (Section III hereof) shall be $130,205.00, hereinafter
referred to as the"total fee", plus up to$3,200.00 in reimbursable expenses."

All other provisions of this Contract which are not expressly amended herein shall remain in full force and
effect.

RE: FAA Requirements for Helipad
Police Helicopter Facility
Alfonso,
The decision to prepare FAA form 7480-1 for the helipad at the Police Helicopter Facility during
our last meeting makes it necessary to design to FAA standards. The FAA standards will require
the design of a lighting system for the TLOF (Touchdown and Lift Off Area)and the FATO (Final
Approach and Take Off Area)as well as the addition of a lighted wind cone to the project.
The initial project did not include any of these considerations or preparing form 7480-1 and while
we are fully capable of meeting these standards they are beyond our original scope for the project
As such; it will require additional time and effort on our part as well as our subcontractors.
Therefore we are requesting an amendment to our contract in the amount of$6760.00 for
additional services relating to the design of the helipad to FAA standards.

In addition any specialized lighting costs were not considered in the project and will add
approximately$23,000-$30,000 to the preliminary cost estimate.
