TERM: September 01, 2006 THRU: August 31, 2007
SECTION I. STATEMENT OF WORK:
Contractor shall design, implement, and evaluate a Community Diabetes Services Project
(CDSP). This project shall provide community-based diabetes prevention and management
interventions that implement population-based and evidence-based programs and strategies at the
community level and inform and educate community leaders about evidence-based community
systems and environmental changes that support and impact diabetes primary and secondary
preventive measures.
For the purpose of this Program Attachment, CDSP shall target adults 18-64 years old and
families in high-risk populations - racial and ethnic minorities - who have disproportionate rates
and burdens of diabetes and limited access to health care.
Contractor shall meet the following objectives through the CDSP and shall provide evidence that
the CDSP is meeting the following objectives:
• Increase opportunities for implementing positive behavior and lifestyle changes (e.g.,
increased and continuing physical activity and healthy eating) in people who already
have diabetes and those who are at risk of developing diabetes;
• Increase community, environmental, and systems changes in various community
sectors (i.e., schools, businesses, faith communities, neighborhoods, health systems
and food industry) that facilitate ongoing, increased physical activity and healthy
eating among the general population, especially those with diabetes and "pre-
diabetes";
• Institute project strategies or community policy and environmental changes
conducive to risk reduction;
• Increase public and provider knowledge of the symptoms, risk factors and target
goals of diabetes, pre-diabetes and gestational diabetes and the importance of physical
activity and healthy eating in preventing and delaying or managing the disease and its
complications; and,
• Increase health care providers', payers', and their patients' knowledge and
implementation of the Texas Diabetes Council's (TDC) Minimum Standards for
Diabetes Care in Texas (flow sheet and algorithms).

All activities shall be performed in accordance with DSHS's Request for Proposal (RFP),
Community Diabetes Services Projects, RFP D02 0001.1 issued April 15, 2003, Renewal
Application D02 0001.2 issued April 30, 2004, and D02 0001.3 issued April 25, 2005,
Contractor's objectives, activities, work plan, timeline, detailed budget and revisions, if any, as
adopted by DSHS and transmitted in writing to Contractor. These documents are incorporated
by reference and made a part of this Program Attachment.
DSHS may renew this Program Attachment for 1 (one) additional year term at DSHS's sole
discretion.
Contractor shall establish and maintain a broad-based community diabetes coalition that meets at
least quarterly for the duration of this project to perform the project requirements outlined in
Section F. of the DSHS's RFP dated April 15, 2003.
Contractor shall have and maintain procedures and tracking tools in a consistent, timely manner
to document activities, services, and persons served that are measured in reports. Contractor also
shall fulfill evaluation requirements as specified by DSHS.
Contractor shall continue to be listed in the Texas Information Referral Network (TIRN) and
continue to identify other diabetes resources in their service area that are not listed with TIRN,
encouraging these agencies/programs to add their resources to the database.
Contractor shall provide evidence through its yearly formative evaluations and its end-of-project
summative evaluation that Contractor is meeting project work plan's goals outlined in project's
impact and process objectives.
Contractor shall submit a sustainability report in a format specified by DSHS that describes how
the project outcomes will be sustained after funding ends.
Contractor shall align any health care system components with the TDC's Minimum Standards
of Care (flow sheet and algorithms). All project interventions shall be consistent with the TDC's
strategic plan and DSHS priorities for the health of Texans: fitness through physical activity and
nutrition, immunizations, and eliminating health disparities.
Contractor shall comply with all applicable federal and state laws, rules, regulations, standards,
and guidelines including, but not limited to the following:
• Health and Safety Code, §12.012 and Chapter 103;
• Healthy People 2010, United States (U.S.) Department of Health and Human
Services, November, 2000; and
• Diabetes Today. A Course on How to Build Skills to Plan and Implement Community
Based Programs for Persons with Diabetes, U.S. Department of Health and Human
Services, September 2000.
This Contract is contingent upon the continued availability of funding. If funds become
unavailable through lack of appropriations, budget cuts, transfers of funds between programs or

health and human services agencies, amendment to the Appropriations Act, health and human
services agencies consolidation, or any other disruptions of current appropriated funding for this
Contract,DSHS may restrict,reduce, or terminate funding under this Contract.
Within thirty (30) days of receipt of an amended standard(s) or guideline(s), Contractor shall
inform DSHS Program, in writing, if it will not continue performance under this Program
Attachment in compliance with the amended standard(s) or guideline(s). DSHS may terminate
the Program Attachment immediately or within a reasonable period of time as determined by
DSHS.
DSHS reserves the right, where allowed by legal authority, to redirect funds in the event of
financial shortfalls. DSHS Program will monitor Contractor's expenditures on a quarterly basis.
If expenditures are below that projected in Contractor's total contract amount as shown in
SECTION III. BUDGET, Contractor's budget may be subject to a decrease for the remainder of
the Program Attachment term. Vacant positions existing after ninety (90) days may result in a
decrease in funds.
PERFORMANCE MEASURES
The following performance measures will be used to assess, in part, Contractor's effectiveness in
providing services described in this Program Attachment, without waiving the enforceability of
any of the other terms of the Contract. Contractor shall complete each performance measure as
defined in this Program Attachment under SECTION I. STATEMENT OF WORK.
Contractor shall provide the following:
• Conduct at least 8 organizational activities leading to changes in community systems,
policy, and/or environments that reduce the burden of diabetes;
• Provide a minimum of 0 organizational changes impacting community systems,
policy, and/or environments in different sectors of the community (e.g., health care,
work site/businesses, faith-based communities, neighborhoods, recreations, schools,
non-profits, and food industry)to reduce the burden of diabetes;
• Conduct at least 46 self-management series of classes (e.g., the Centers for Disease
Control and Prevention's diabetes self management program — (Diabetes Education
and Empowerment Program), or other DSHS approved models) or activities to reduce
the burden of diabetes reaching 560 people;
• Provide a minimum of 24 physical activity events and ongoing activity groups to
reduce the burden of diabetes reaching 45 people;
• Conduct at least 0 support groups to reduce the burden of diabetes reaching 0
people;
• Conduct at least 12 series of food and nutrition classes to reduce the burden of
diabetes reaching 45 people;
• Provider education (risk reduction, TDC's Minimum Standards of Care [flow sheet
and algorithms], etc.) about diabetes reaching 80 providers;
• Publish a minimum of 100 diabetes-related articles (e.g., newspapers, church
bulletins,business inserts or newsletters);

• Provide a minimum of 80 diabetes-related radio activities;
• Provide a minimum of 50 diabetes-related television activities;
• Distribute at least 3,500 pieces of diabetes material (e.g., National Diabetes
Education Program and Texas Diabetes Program/Council [TDP/C] literature such as
foot posters);
• Submit a sustainability report in a format designated by the Texas Diabetes Program
(TDP)that describes how project outcomes will be sustained after funding ends. This
report is due by November 30, 2006;
• Submit summative evaluation report covering the four year contract period
(evaluation data and analysis format to be specified by TDP);
• Continue to identify 7 diabetes resources in their service area to submit profile into
the database; and
• Conduct at least 0 Diabetes Empowerment Education Program (DEEP) training for
trainers reaching 0 people.
The numbers above indicate the minimum for each performance measure. This value is subject
to change upon written agreement between DSHS and Contractor. Work plan activities and
deadlines shall not be changed without prior approval from DSHS.
Contractor shall submit monthly performance reports that are prepared in the format approved by
DSHS Program within ten (10) calendar days after the end of each month. Contractor shall also
submit quarterly performance reports that are prepared in the format approved by DSHS
Program within twenty (20) calendar days after the end of each quarter. Contractor shall also
submit a year-end summary, prepared in the format approved by DSHS Program, within thirty
(30) calendar days after the end of the Program Attachment term. Contractor shall submit a
project summative evaluation report covering the four year project within thirty (30) calendar
days after the end of the contract term. Contractor shall maintain the documentation used to
calculate key outcome performance measures.
Contractor shall notify DSHS Program Director, Diabetes Program, of any changes in project
staff or contact information within fourteen(14) calendar days of such change(s).
Contractor's project coordinator and support staff shall attend all scheduled trainings in their
entirety held by DSHS unless prior approval is obtained from authorized DSHS's Texas Diabetes
Program staff for absences.
Contractor shall request prior approval from DSHS on any proposed educational media
campaigns for the public or health care professionals. This requirement does not apply to
promotional campaigns for local events. Contractor shall use National Diabetes Education
Program (NDEP) materials whenever possible. Contractor shall include the following credit on
all printed, audio or visual materials developed as part of this program: "Funding provided by the
Texas Diabetes Program, Department of State Health Services."

Authorize Execution of a Grant Contract with the Texas Department of State Health Services
Chronic Disease Prevention Branch for the Texas Diabetes Program's Community Diabetes Service
Projects from September 1, 2006 to August 31, 2007, Adopt Appropriation Ordinance and Inter-
Fund Loan
RECOMMENDATION:
It is recommended that the City Council:
1. Authorize the City Manager to apply for and accept, if awarded, a grant contract with the Texas
Department of State Health Services (DSHS), Chronic Disease Prevention Branch for the Texas Diabetes
Program's Community Diabetes Service Projects (CDSP) in the amount of $80,000 to be used to provide
community-based diabetes prevention and management programs from September 1, 2006 to August 31,
2007;
2. Adopt the attached appropriation ordinance increasing estimated receipts and appropriation in the
Grants Fund by $80,000;
3. Waive indirect costs of 17.59% of related salary and fringe benefits which are the most recent rates
indicated by the Finance Department; and
4. Authorize a non-interest bearing interfund loan from the General Fund to the Grants Fund in the
amount of $25,000 for the interim financing for this grant pending contract execution to secure program
continuation from one grant funding period to the next.
DISCUSSION:
Diabetes is an epidemic and a public health crisis in Texas. During the 2003 City of Fort Worth Public
Health Department Community Needs Assessment, six out of the 12 Neighborhood Police Districts
surveyed ranked diabetes number one as their greatest household health problem.
Poor eating habits, being overweight and lack of physical activity attribute to 90% of diabetes
diagnoses. More than 60% of American adults do not get enough physical activity and more than 25% are
not active at all. The City of Fort Worth is no exception. According to the 2003 City of Fort Worth Public
Health Community Needs Assessment 21.3% to 40% surveyed identified not exercising. Along with 82.5%
to 90.8% surveyed identified eating fewer than five servings of fruits and vegetables a day.
The City of Fort Worth Public Health Department Diabetes Program was awarded funding in September
2003 for a four-year program renewal to provide the following but not limited to:
. Organizational activities leading to changes in community systems, policy and/or
environments,
. Organizational changes impacting community systems, policy, and/or environments

in different community sectors (e.g. health care, worksites/businesses, faith-based communities,
neighborhoods, recreation, non-profits, food industry, etc.),
. Lifestyle classes in English (e.g., CDC Diabetes self management program or other DSHS
approved models) or similar activities,
. Lifestyle classes in Spanish (e.g., CDC Diabetes self management program or other DSHS
approved models) or similar activities physical activity ongoing groups and activity events,
. Physical activity events and ongoing activity groups,
. Support groups - English,
. Support groups - Spanish,
. A series of food and nutrition classes,
. Provider Education (risk reduction, standards of care, etc.),
. Public health information activities (e.g., television, radio, print media, outdoor mobile
advertising, websites, kiosks, displays, etc.),
. Material distribution (e.g. brochures, posters, etc. from National Diabetes Education Program,
Texas Diabetes Council/DSHS and other sources)
. Resources in Texas Information Referral Network (by Jan. 2006)
The City of Fort Worth Public Health Department provides community-based diabetes programs targeting
individuals with diabetes or those at risk and their families, those uninsured/underinsured and communities
in Fort Worth with high prevalence and burdens of diabetes, including mortality.
FISCAL INFORMATION/CERTIFICATION:
The Finance Director certifies that upon approval of the above recommendations and adoption of the
attached appropriation ordinance, funds will be available in the current operating budget, as appropriated, of
the Grants Fund. Upon execution of the grant contract, the interim inter-fund loan will be returned to the
General Fund.
