Home My WebLink AboutContract 36690-A3 City Secretary Contract No. AMENDMENT NO. 3 TO CITY SECRETARY CONTRACT NO. 36690 PROFESSIONAL SERVICES AGREEMENT WHEREAS, the City of Fort Worth ("City") and MCM Technology, Inc., ("Consultant') a Delaware corporation, collectively referred to as the "parties," made and entered into City of Fort Worth City Secretary Contract ("CSC") No. 36690 on or about March 3, 2008 ("Agreement'); and WHEREAS, the Agreement involves implementation of efficient business work flow processes for asset and work order management responsibilities for the City; and WHEREAS, the parties previously amended the Professional Services Agreement via CSC No. 36690-A1 to extend the term and modify the scope to include platform services for the Fort Worth Fire Department; CSC No. 36690-A2 to modify the scope of services to include the current CommSHOP 360 platform to include services for the Fort Worth Police Department; CSC No. 36690, 36990A-1 and 36990 A-2 are incorporated herein by reference and collectively referred to as the "Agreement'; and WHEREAS, the parties now wish to amend the Agreement to add five additional seat licenses and one year of maintenance for the Police Department. NOW THEREFORE, City and Consultant, acting herein by and through their duly authorized representatives, enter into the following agreement: 1. Exhibit A-1 to the Agreement, Pricing, shall be amended to add pricing for Data Conversion and User Training, and five (5) additional seat licenses for CommSHOP 360, as set forth in the "Professional Service Pricing" document attached hereto and incorporated into to the Agreement for all purposes. Exhibit A-3 to the Agreement, Statement of Work, shall be amended to add software maintenance and support for CommSHOP 360 for one year. 3. Section 3, Compensation, is hereby amended to increase the contract amount by $10,750.30, thereby increasing the total contract amount to $46,348.30, inclusive of all services and expenses. 4. The Term of the Agreement shall be extended to effective February 13, 2012 and expire September 30, 2013. 5. All other provisions of the Agreement that are not expressly amended herein shall remain in full force and effect. Executed in multiple originals on this�ay of 12012.
