That hereafter, all persons operkting carriages, Laoks, 0=1buses, automobiles or other vG'tA- cles within the City of Fort ort14 for the purpose oi` oarrying passengers for hire *hall first obtain a permit from the Hoard of Commissioners so to do, and shall make application to the Roaxd of Qoxmissioners In writing therefor, stating in such supplication the name, street address, age -of the operator, period o�ienoe of such operator in the oporstion of pubi- 13a emebOAM, the Character of business engaged in by euoh applicMt, whether such person is addicted to the use of in- toxioating liquors or the use of morphine, cocaine, or opium ' or any other lr� calculated to of feet the physical etrer gth or wind of the operator, and shall also state whether such appli- cant is deaf or partially deaf, or to nearsighted, or is U. borin# under any other physioal infirmity or afflicted with any disease of any kin4, and that acid application shall also state rhether tho aai4 applioant has ever been oonvieted of any viola- tion of the traffic ordinance of the city of port •'orth, and sbai1 also state the plus pr residence of tts appl•ioant tar a period of at least five years prior to the date of =Lking such appl for ties, and the oharaoter of business engaged In by such xpplic8arst. Seatj=_& . Any person or persons violating any of the provisime of this ordinance shall be guilty of a mis-de- meanor and upon oonviotian,punished by fine not less than $5.00 nor more than 010.00.
