HomeMy WebLinkAboutContract 36748 AMENDMENT NO. 3 STATE OF TEXAS S CITY SECRETARY CONTRACT NO. � (No M&C Needed) COUNTY OF TARRANT S WHEREAS, the City of Fort Worth (City) and Dunaway Associates, L.P. , (Engineer) made and entered into City Secretary Contract No. 34093, (the Contract) which was authorized by the City Council by M&C C-21712 on the 26th day of September 2006; and WHEREAS, the Contract involves engineering services for the following project: Engineering Design Services: Street Reconstruction, Storm Drain Design on College Avenue (Page St to W. Capps St) , Orange Street (Stanley Ave to Townsend Dr) , Lipscomb Street (W. Capps St to W. Berry St) and Wayside Avenue (N. Dead End to W. Cantey St) , 2004 CIP Contract Year 4 & 5, Contract 53, Project No. 00455; DOE No. 5411 WHEREAS, it has become necessary to execute Amendment No. 3 to said Contract to include an increased scope of work and revised maximum fee; NOW THEREFORE, City and Engineer, acting herein by and through their duly authorized representatives, enter into the following agreement which amends the Contract: 1 . Article I of the Contract is amended to include the additional engineering services specified in a proposal letter dated October 31 , 2007 , a copy of which is attached hereto and incorporated herein. The cost to City for the additional design services to be performed by Engineer total $10,570. 00. 2 . Article II of the Contract is amended to provide for an increase in the maximum fee to be paid to Engineer for all work and services performed under the Contract, as amended, so that the total fee paid by the City for all work and services exceed the sum of $238,837 .00 . Program Manager City of Fort Worth Department of Engineering Fort Worth, Texas 76102 Reference: Amendment No. 3 2004 CIP Project Yrs 4 & 5 - College Ave., Orange St, Lipscomb St and Wayside Ave. Contract.53 Project No. 00455 Dear Ms. Johnson: Dunaway Associates, L.P. is pleased to present this Amendment No. 3'for additional water line engineering design services that are beyond our original scope of services.The general scope of these services is as follows, Per water department 60°Io submittal review meeting held on Wednesday October 17, 2007, and confirmation email received from David Cooper on Thursday October 18, 2007, it was determined that the, City would like to loop the water lines between Lipscomb Ave and College Ave via an 8 water line in W.Capps St. Additionally, because the land use adjacent to Lipscomb is commercial/industrial north of Cantey St,the City feels it maybe necessary to upsize that portion of the line from the proposed 8"to 12". This would require preparation of a profile for that length of line. Because our contract and budget does not provide for the design and preparation of construction documents for this additional water line design, we propose to supplement our scope of services to add to following tasks. • 625 LF of re-design to upsize the proposed water line from 8" to 12" (plan and profile preparation required per City's standard) on Lipscomb St between Cantey St and W. Capps St. • 300 LF of additional design and survey for 8" water line in W.
