COUNTY OF TARRANT S
WHEREAS, the City of Fort Worth (CITY) and Gorrondona & Associates,
Inc. , (CONSULTANT) made and entered into City Secretary Contract No.
32378, (the CONTRACT) which was authorized by the City Council by M&C C-
22048 on the loth day of April, 2007;
WHEREAS, the Contract involves surveying services for the following
project:
Dirks Road from Bryant Irvin Road to Granbury Road
WHEREAS, it has become necessary to execute Amendment No. 1 to said
Contract to include an increased scope of work and revised maximum fee;
NOW THEREFORE, CITY and ENGINEER, acting herein by and through their
duly authorized representatives, enter into the following agreement,
which amends the Contract:

Article I of the Contract is amended to include the additional
surveying services specified in the proposal letter dated May 8, 2007, a
copy of which is attached hereto and incorporated herein. The cost to
City for the additional services to be performed by CONSULTANT total
$22,250.00.

Article II of the Contract is amended to provide for an increase in
the fee to be paid to CONSULTANT for all work and services performed
under the Contract, as amended, so that the total fee paid by the City
for all work and services shall be the sum of $68, 150.
3 .
All other provisions of the Contract, which are not expressly amended

herein, shall remain in full force and effect.

Dear Mr.Leuschen:
Gorrondona & Associates, Inc. (GA ) is pleased to submit this proposal for professional land
surveying services for the above referenced project. The following itemized surveying services
are to be provided by Gorrondona&Assoc.,Inc.:
1) TOPOGRAPHIC SURVEY - Provide additional topographic survey of Alta Mesa Blvd. as
detail in an attachment in an e-mail from Dustin McKinney of Carter&Burgess,dated April
23'd, 2007. The topographic survey shall include, but not limited to, locating all existing
features such as water valves, traffic signal equipment, culverts, curb & gutter, asphalt,
sidewalks,medians,driveways, storm&sewer manholes,tops and toes of slope,power poles,
telephone risers and other visible features.
2) DELIVERABLES - Prepare a digital topographic design file showing all features located
along with 1-foot interval contours and the digital terrain model of the survey merged with
the original topographic survey. Provide copies of the survey field notes, a hardcopy of the
coordinates,the digital design file in Microstation and an ASCII file of the coordinates for the
points located.
3) BORE HOLES — Stake approximately twenty-five (25) bore holes for the geotechnical
engineer.Deliverables shall include an X,Y&Z format in an ASCII point file format.
AMOUNT $ 11,000.00
3) SUBSURFACE UTILITY ENGINEERING(SUE)
GENERAL SUE SCOPE-SUE is in essence a two-phase process,designating and locating as
defined in ASCE Publication CUASCE 38-02 (Standard Guideline for the Collection and
Depiction of Existing Subsurface Utility Data). The first phase,designating(QL"B),provides
the horizontal (x, y) information necessary to overlay existing utilities on plan sheets, and is
completed by means of electronic geophysical prospecting. The second phase, locating (QL
"A"), provides vertical (z) information by means of non-destructive vacuum excavation
methods.
It is our responsibility to provide due-diligence in regards to records research (QL "D") and
acquisition of available utility records. The due-diligence provided will consist of contacting
the applicable"one-call"agency and acquiring/reviewing available utility records from utility
owners, including local municipalities. Utilities that are not part of the"one call" system, or
not shown in the available utility records will be here forth referred to as"unkno

Gorrondona & Associates, Inc. will scan the defined work area using electronic prospecting
equipment to search for"unknown'utilities. However,Gorrondona&Associates,Inc. is not
responsible for designating and locating these"unknown'utilities.
A.)PROJECT SPECIFIC SCOPE-Based on the plan sheet provided defining the additional
areas where SUE is being requested, there is an additional 2,300 linear feet where
QL"B"activities will be completed.
AMOUNT $ 8,000.00
Additionally, we have included four (4) test holes so that vertical information can be
obtained at requested locations. We have assumed that all test hole locations are
accessible to tuck-mounted vacuum excavation equipment.
AMOUNT $ 3,250.00
Following designating, we will prepare field sketches depicting the designated utilities.
These sketches will be used as a reference during surveying and during review the survey
information.
Gorrondona&Associates, Inc. can provide the above professional land surveying services for a
