HomeMy WebLinkAboutContract 37520STATE OF TEXAS COUNTY OF TARRANT InntY SF!CRE'TARY AMENDMENT NO. 1 § CITY SECRETARY CONTRACT NO. 34044 (No M&C Needed) WHEREAS, the City of Fort Worth (City) and Carter & Burgess, Inc., (Engineer) made and entered into City Secretary Contract No. 34044 (the Contract) which was authorized by the City Council by M&C G21593 on the 3rd day of September, 2006, and WHEREAS, the Contract involves engineering services for the following project: Stonegate Boulevard and an unnamed Connector to SH-121(Southwest Parkway) Capital Project No. 00107; DOE NO. 5994 WHEREAS, it has become necessary to execute Amendment No. 1 to said Contract to include an increased scope of work and revised maximum fee; NOW THEREFORE, City and Engineer, acting herein by and through their duly -authorized representatives, enter into the following agreement which amends the Contract. 1. Article I of the Contract is amended to include the additional engineering services as specified in a proposal letter dated August 19th1 2008, a copy of which is attached hereto and incorporated herein. The cost to City for the additional services to be performed by Engineer total $23,208, 2. Article II of the Contract is amended to provide for an increase in the maximum fee to be paid to Engineer for all work and services performed under the Contract, as amended, so that the total fee paid by the City for all work and services shall not exceed the sum of $ 526,290 + $ 23,208 (Amendment No. 1) _ $ 549,4989 OFFICIAL RECORD CITY SECRETARY FT. C&B Nevada, Inc. The following additional services for City Secretary Contract No. 34044 shall be added: 1. Bridge Shop Drawing Review $59000.00 COFW requested Carter & Burgess to review the bridge shop drawings from Orval Hall. The contract does not include any construction surveying, site visits, or shop drawing review, instructions to contractor, differing site condition sketches and record rawing preparation as stated in Part C — Pre Construction Assistance Paragraph C Assistance During Construction. 2. Pavement Width Plan Revisions $79608.00 COFW requested the pavement width for Edwards Ranch Road (Stonegate) be revised to 25 feet back to back. The construction plans were approved and the pre -construction meeting had already been completed. Additionally at the pre -construction meeting the city surveyor and Mr. Wilson requested additional information on the approved construction plans, including top of curb information along the main curt lines and at the median for the PS, PT, PRS and noses. 3. Reimbursement for Reproduction $1,000.00 The contract was to provide one complete set of construction documents, specifications and bid package. Due to the job being split in two phases, the reproduction costs have increased. To minimize and expedite the requests for additional prints, Carter & Burgess will print the requested check prints and intern submittals. To ensure the MWBE requirement and contracted amount is used, the final submittals will be provided utilizing the MWBE. 4. Geotechnical Services —Pavement Design $9,000.00 Carter &Burgess will conduct three borings along Edwards Ranch Road and five borings along Clearfork Main exploring sub -surface conditions. A pavement recommendation meeting the requirements of the City of Fort Worth Pavement Design Standards will be provided. Survey staking the boring locations is included. Due to the project being split in two construction sets, the geotech only provided pavement recommendations and subsurface exploration for the bridge and first construction phase. 5. Shop Drawing —Review —Sidewalk Cover Plates COFW requested Carter &Burgess to review shop drawings for sidewalk cover plates submitted by Ashlar. TOTAL $235208.00
