COUNTY OF TARRANT §
WHEREAS, the City of Fort Worth (City) and Chiang, Patel & Yerby, Inc.,
(Engineer) made and entered into City Secretary Contract No. 27951 (the Contract)
which was authorized by the City Council by M&C C-19197 on the 20th day of August,
2002; and
WHEREAS, the Contract involves engineering services for the following project:
Engineering Contract for Holly Pressure Plane Water Transmission Main
Improvement.
WHEREAS, it has become necessary to execute Amendment No. 2 to said
Contract to include an increased scope of work and revised maximum fee;
NOW THEREFORE, City and Engineer, acting herein by and through their duly
authorized representatives, enter into the following agreement, which amends the
Contract:
1.
Article I of the Contract is amended to include the additional engineering services
specified in a proposal letter dated August J. 2008, a copy of which is attached hereto and
incorporated herein. The cost to City for the additional services to be performed by
Engineer total $24,266.00
2.
Article II of the Contract is amended to provide for an increase in the maximum
fee to be paid to Engineer for all work and services performed under the Contract, as
amended, so that the total fee paid by the City for all work and services shall not exceed
the sum of $593,547.69.
3.
All other provisions of the contract, which are not expressly amended herein, shall
remain in full force and effect.

Dear Mr. Sahu:
The purpose of this letter is to request a Change Order for additional services related to the

ppeline design. We respectfully submit the following items for your consideration.
Additional Subsurface Utility Engineering (SUE)
The previous contract for this project only included Quality Level (QL)"B" SUE services. QL"B"
services provide two-dimensional information (x,y) of underground utilities. Now that the
preferred pipeline alignment has been determined, additional information, specifically the depth
of the utilities, needs to be ascertained. Therefore, CP&Y recommends that a Quality Level "A"
SUE be performed at approximately 11 locations identified along this preferred alignment. A QL
°A" SUE provides three-dimensional (x,y,z) information for the concerned utilities. Due to the
possibility that the alignment may still change slightly, CP&Y has requested Gorrondona &
Associates, Inc. (GAI) to provide us with a proposal for services on a "Unit Rate" basis with a
total of 14 locations. The fee for these services is $13,660.
Additional Surveying
Due to preferences by the City of Lake Worth, the alignment of the waterline through the city
park will not parallel the TxDOT right-of-way along Loop 820 as originally anticipated. Instead,
the preferred alignment will cross sections of the park between the baseball fields and a parking
lot. This new alignment is outside of the scope of work performed in the original survey and the
area will need to be surveyed in order to complete an accurate design. Therefore, CP&Y has
requested additional land surveying services from GAI to incorporate the limits of the new
alignment. The fee for these services is not to exceed $8,400.
Project Schedule
Once CP&Y is given approval by the City of Fort Worth, we will give Notice to Proceed to GAI.
It will take approximately 3 weeks for GAI to complete the work and provide the data to CP&Y.

Quality Level "A" (Test Holes)
Fort Worth - NW Water Transmission Main
Dear Mr. Cochill:
Gorrondona and Associates, Inc. (GA}, is pleased to submit a cost proposal for Subsurface
Utility Engineering (SUE) required for the above referenced project.
Introduction
GA will perform the SUE work required for this project in general accordance with the recommended
practices and procedures described in ASCE Publication CUASCE 38-02 (Standard Guideline for the
Collection and Depiction of Existing Subsurface Utility Data). As described in the mentioned ASCE
publication, four levels have been established to describe the quality of utility location and attribute
information used on plans. The four quality levels are as follows:
• Quality Level D (QL"D") —Information derived from existing records;
• Quality Level C (QL"C") - QL"D" information supplemented with information obtained by
surveying visible above -ground utility features such as valves, hydrants, meters, manhole
covers, etc.
• Quality Level B (QL"B") — Two-dimensional (x,y) information obtained through the
application and interpretation of non-destructive surface geophysical methods. Also known as
"designating" this quality level provides the approximate horizontal position of subsurface
utilities within approximately one foot.
• Quality Level A (QL"A") — Also known as "locating", this quality level provides precise three
dimensional (x,y,z) information at critical locations by exposing specific utilities. Non-
destructive vacuum excavation equipment is used to expose the utilities at specific points
which are then tied down by survey.
It is the responsibility of the SUE provider to perform due -diligence with regard to records research
(QL "D") and acquisition of available utility records. The due -diligence provided for this project will
consist of reviewing the "As -Built" documents previously compiled by others. Utilities that are not
identified through these efforts will be here forth referred to as "unknown" utilities. GA personnel
will scan the defined work area using electronic prospecting equipment to search for "unknown"
utilities. However, GA is not responsible for designating and locating these "unknown" utilities.

Scope of Work
Based on information provided by CPY, GA has developed a preliminary scope for the SUE work
required for this project. The scope of work may be modified, with CPYs' concurrence, during the
performance of the SUE fieldwork if warranted by actual field findings.
The scope of this proposal includes Quality level "A" along NW Loop 820 from 800 feet northeast of
Quebec Street to 3,000 feet northeast of Azle Avenue along the southeast ROW line. Based on the
requested test hole locations which include gas — 2 each; sanitary sewer — 4 each; water — 2 each;
storm drain — 3 each; for a total of 1 I test holes. For purposes of this proposal we are including an
additional 3 test hoes in case additional test holes will be required.
It is G&A's understanding that surveying is apart of this scope and will be completed by G&A
personnel.
Locating Procedures
GA will utilize its utility designating marks and assistance from CPY personnel to layout the test -hole
locations. GA will attempt to place the test holes outside of paved areas wherever possible. If non -
routine traffic control measures are required, these services will be considered extra and will be
invoiced at cost.
GA will utilize non-destructive vacuum excavation equipment to excavate test holes at the required
locations. Once each utility is located, GA will record the utility type, size, material, depth to top, and
general direction. Each test hole will be assigned a unique ID number and will be marked with
rebar/cap, nail/disk, or chiseled X, as appropriate. The test -hole ID number and other pertinent utility
information will be placed at each test -hole location. Test -hole excavations will be backfilled with
appropriate material and the original surface will be restored. The backfill will be compacted in lifts
by mechanical means to prevent future settlement.
Test -hole locations will be added to the previously mentioned field sketches for use during surveying
activities.
Deliverables
GA will produce SUE plans depicting the type and horizontal location of the designated utilities. The
size of each utility will also be presented on the SUE plans where test -hales have been completed.
GA will produce a summary sheet containing test hole field notes. A test hole data form will be
prepared for each test hole following receipt of coordinate and elevation information from the
surveyor. SUE plans depicting designated utility information will be revised to include test hole
locations. GA will provide completed electronic file and appropriate hard copy files to CPY.
Schedule
GA estimates that the QL"A" test holes can be completed in four k4) workdays with a deliverable
being completed five days after the survey has been collected and processed.

Estimated Fees
GA proposes to perform the work on a "Unit Rate" basis. Based on the unit rate schedule below, the
cost for designating, QL"A" is thirteen thousand, six hundred dollars and 001100 cents
($13,660.00) including the survey cost to collect the SUE data.

Gorrondona &Associates, Inc. (GAI) is pleased to submit this proposal for professional land surveying
services for the above referenced project. The following itemized surveying services for each alignment to
be provided by Gorrondona & Assoc., Inc. are as follows:
ALIGNMENT
The route of the additional surveying services starts at the south property line of the City of Lake Worth
Park property and extend approximately 2,100 linear feet to the north line of Roberts'Cut-Off.
TASK 1. — TOPOGRAPHIC/DESIGN SERVICE5
1.) PROJECT CONTROL — Extend existing project control for new waterline alignment.
2.) TOPOGRAPHIC/DESIGN SURVEY —The topographic design survey shall include locating all
existing features within 150 feet of the eastern limits of the existing topographic survey. Cross -sections
shall be taken at approximate 100-foot intervals or as needed to generate an accurate Digital Terrain
Model. Features located shall include, but not limited to, property corners, water valves, manholes,
power poles, storm drain structures, road and parking lot pavement, sidewalks, buildings, trees (tree
lines in highly condensed areas, individual trees 6" or larger including all landscape trees), fences, curb
& gutter, ball fields, railroad rails and tops & toes of slope.
3.) DELIVERABLES — Prepare a final digital topographic/design survey drawing in AutoCAD Civil 3D
format at a scale of 1 "= 20'. The drawing shall show all features located, horizontal and vertical
control point and existing visible utilities along with merging the new topographic survey into the
existing Digital Terrain Model with one -foot interval contours. Other deliverables shall include a copy
of all field notes and field sketches, a hard copy coordinate list of points located in the field and a
digital ASCII point list.
Gorrondona &Associates, Inc. can complete the above -itemized professional land surveying tasks for an
hourly not -to -exceed fee of $8,400.00 utilizing the attached fee schedule. If you have any questions or need
