AMENDMENT NO. 1

WHEREAS, the City of Fort Worth (City)
inc., (Engineer) made and entered into City
38059, (the Contract) which was authorized
MC C-23162 on the 11th day of November 2008;

WHEREAS, the Contract involves engineering services for the
following project.
Storm Water Utility Project - Westlake, Project No. 01073; DOE No.
60010
WHEREAS, it has become necessary to execute Amendment No. 1 to
said Contract to include an increased scope of work and revised
maximum fee;
NOW THEREFORE, City and Engineer, acting herein by and through
their duly authorized representatives, enter into the following
agreement which amends the Contract:
1.
Article I of the Contract is amended to include the additional
engineering services specified in a proposal letter dated October
23, 2009, a copy of which is attached hereto and incorporated
herein. The cost to City for the additional design services to be
performed by Engineer total $24,921.25.
2.
Article II of the Contract is amended to provide for an
increase in the maximum fee to be paid to Engineer for all work and
services performed under the Contract, as amended, so that the
total fee paid by the City for all work and services shall not
exceed the sum of $49,864.50.

All other provisions of the Contract which are not expressly amended
herein shall remain in full force and effect.

Westlake Drive Drainage Improvements Study
City Project No. 01073
Dear Mr. Owen:
Per your request, this amendment revises the previously approved Westlake Drive Drainage
Improvements Study, City Project No. 01073, scope of work and contract documents. Specifically, this
amendment includes the following additional services:
1. Geotechnical Engineering
Soil investigations, including field and laboratory tests, borings, related engineering analysis and
recommendations for determining soil conditions will be made. In addition to the above
investigations, borings and appropriate field and laboratory analyses will be made at reasonable
intervals along the project alignment for the Contractor's use in determining soil conditions for
preparing bids and a trench safety plan.
The Engineer shall prepare a detailed geotechnical engineering study. The study shall include
recommendations regarding utility trenching and identifying existing groundwater elevation at each
boring.
Geotechnical Engineering Assumptions:
Four borings to 12 feet in roadways and one boring near houses to a depth of 25 feet will be drilled.
The deeper boring between the houses is planned to provide information for potential requirement of
underpinning of the houses. The deeper boring will be drilled in the street if underground utilities or
trees preclude placing the boring in one of the yards. Soils will be sampled continuously to 12 feet,
then at 5 foot intervals. Rock will be tested in place with TxDOT cone penetrometer. Water level
observations will be made during and upon completion of drilling.
Street pavement will be patched in accordance with City of Fort Worth requirements. Plyboard will
be used to support drilling equipment accessing the boring on home owner's yard to reduce the risk
of rutting and damage to sprinkler lines. The yard boring will be backfilled with soil cuttings.
Excess cutting will be removed. Should an irrigation line or sprinkler head be damaged due to the
drilling operations, the damage will be repaired. Laboratory tests will be performed to classify the
soils and to measure soil strengths for determining soil conditions for trenching and storm drain
construction. These tests will include Liquid and Plastic Limits, grain size, moisture contents, and
unconfined compressive strengths.
Geotechnical Engineering Study:
The geotechnical engineering study will include recommendations for utility trenching within close
proximity to residential structures. The geotechnical report will include the following:
• Boring site plan, logs of borings, water level observations, and laboratory test results.
• Classification of soil and rock to allow contractor to develop and implement trench safety
plans.
• Presence of groundwater and recommendations to control groundwater and to protect
stability of trench bottom.
• Evaluation of the potential effects of open cuts for the 36 inch or 66 inch storm drain
between two residential structures to include ground settlements and vibrations.
Recommendations will be provided for protection of the structures by trench shoring,
building underpinning and trench backfill requirements.
• Condition surveys for houses located at 4808 and 4900 Westlake Drive prior to construction
to catalog and document distress to exterior finishes, and if permission received from home
owners, interior condition surveys. A brick line survey will be performed in conjunction
with the exterior finish survey. Findings will be presented in narrative, photographic and
graphic report.
• Cost estimates will be provided for underpinning and special trench backfill requirements if
required for project.
• 4 copies of the geotechnical report and condition survey reports will be delivered to the
CITY.
2. Design Surveying
• Research property owners and obtain copies of subdivision plats and ownership deeds for
Lots 14 & 15 of Block 105 of Westcliff addition.
• Locate property corners and establish property lines, existing right -of --way lines and
easement lines. Prepare aright -of --way map of the existing right -of --way showing lot numbers,
ownership with recording information and all of the parcels of land affected by the project.
Deliverables include a property map to overlay onto the topographic survey prepared in an
AutoCAD file format, along with copies of deeds, subdivision plats and tax ownership
information.
• Establish vertical control for the project from existing City of Fort Worth control. Establish
benchmarks every 500' for construction. The horizontal control shall be NAD83, North
Central Zone with a detailed description being given for each control point.
• Provide a limited topographic survey of the proposed drainage project with the limits for the
project being down the common lot line of Lots 14 & 15 of Block 105 of Westcliff Addition.
The survey shall locate the existing fence, finish floor elevations of each house, closest
building comer of each house. The survey shall also stake on the ground the existing
drainage easement along the lot line.
• Locate proposed bore holes and provide X, Y, & Z coordinates for each location.
• Deliverables shall include an AutoCAD drawing isle showing all of the features located,
ground elevations, borehole information. Provide copies of the survey field notes, a hardcopy
of the coordinates and an ASCII file of the coordinates for the points located.
• Prepare (2) Right -of --Entry letters to City of Fort Worth standards. The Right -of --Entry letters
will be on a per document fee of $125.00.
3. Engineering Services
A. Public Involvement:
The Engineer shall prepare project exhibits, and attend one public meeting to help explain
the proposed project to residents. The City shall select a suitable meeting location and mail
the invitation letters to the affected customers.
Public Involvement Assumptions:
• 4 full size (22" x 345) black and white exhibits will be prepared for the public meeting.
B. Update Final Concept Study:
The Engineer shall update and resubmit the Final Westlalce Drive Drainage Improvements
Study to include the findings and recommendations resulting from the geotechnical
engineering study.
Update Final Concept Study Assumptions:
• Additional hydrologic, hydraulic and alternatives analyses are not included as part of this
amendment.
Additional Services Fees:
1. Geotechnical Engineering
Drilling, testing and analysis $ 6,515.50
Cost estimates for underpining and flowable fill $ 630.00
Exterior catalog of distress and brickline survey with report $ 1,892.00
Interior catalog of distress and brickine survey with report $ 2,340.00

Right of Entry Letters $ 250.00
