APPENDIX B-4
In accordance with the Professional Services Agreement signed between Sentari
Technologies, Inc. ("Contractor") and the City of Fort Worth ("Client") on June 22, 2005,
City Secretary Contract (CSC) No. 32037, as amended by CSC No. 37231, collectively
the "Professional Services Agreement," Appendix B-4, which is incorporated into the
Professional Services Agreement is hereby amended to read as follows:
1. The term of this work order shall be extended to end when all services have
been completed under this work order as contemplated herein, unless
otherwise terminated in accordance with CSC No. 32037, as amended.
2. This Work Order incorporates by reference all of the terms and conditions of
the Professional Services Agreement, CSC No. 32037, as amended, and
specifically the services to be provided under Section 1 of said agreement.
3. The services to be performed under this Work Order shall be as follows:
Programmer for Front Range's Service Management/ CRM Application
Programmer shall provide programming and consulting services to correct issues
with ITSM interfaces, business rules used to automatically escalate aged issues,
provide maintenance fixes needed to archive old data, and repair a bug in the Self
Service module.
The following are specific action items to be addressed:
1. ITSM losing IT Tickets when entered thru ITSM Self Service module. —
Troubleshoot the ITSM Self Service module and determine the root cause for the
loss of tickets when saving a new incident. Suggest correct measures. Develop, test
and implement solution.
2. ITSM/CodeOps API Interface - Troubleshoot API interface errors. Determine the
cause of lost information and suggest corrective measure. Develop, test and
implement solution.
3. Open Requests— Create and/or modify miscellaneous reports, screen/scheme
customizations, quick actions, business rules, and provide consultation on patching
and upgrades.
4. System documentation (business rules, quick actions, triggers, etc....)- Create
system documentation that describes the current h/w-s/w environment, system
licensing, configuration of business rules, quick actions and triggers.
5. Develop a database purge process- Consult with customer's designated—
technical staff to review their database retention needs, performance) % rRD
