CONTRACT NO. 3 �SS�-�2
TO CITY SECRETARY CONTRACT
WHEREAS, the City of Fort Worth (CITY) and Neel -Shaffer Inc,
(ENGINEER) made and entered into City Secretary Contract No. 38655, (the
CONTRACT) which was authorized by M&C C-23509 on the 12 day of May, 2009
in the amount of $133,109.47; and
WHEREAS, the CONTRACT was subsequently revised by: Amendment No 1 in
the amount of $ 14,763.00 which was administratively authorized on Feb
3rd 2010; and
WHEREAS, the CONTRACT involves engineering services for the
following project:
Contract 2B, 2008 CIP (Project 01263) Pavement, Water & Sewer
replacement for:
NW 23rd Street (Hanna Avenue -Belle Avenue),
NW 24th Street (Belle Avenue- Dodge Avenue),
Be11e Avenue (NW 22nd Street -NW 23rd Street) ,
Macie Avenue (NW 24th Street -Azle Avenue) ; and
WHEREAS, it has become necessary to execute Amendment No. 2 to the
CONTRACT to include an increased scope of work and revised fee.
NOW THEREFORE, CITY and ENGINEER, acting herein by and through their
duly authorized representatives, enter into the following agreement, ..
which amends the CONTRACT:

Reference: Amendment No. 2 - Contract 2B - 2008 CIP Neighborhood Street Project
NW 23rd Street, 24th Street, Belle Avenue and Macie Avenue - City Project No. 01263
Dear Mr. Jaafari:
Neel -Schaffer, Inc. is submitting this fee Amendment to the above referenced contract based on the City's
request for additional design services to extend the project limits from its original scope; for monthly
M/WBE reports extending beyond the original contract time limits and for modifications to plans and
contract documents required due to City requesting implementation of new City adopted Standards and
Specifications. A brief description and cost for the additional services follows:
Extension of Project Limits for Contract 2B
The City has requested design services for extending 24th Street from it's originally scoped terminus by
reconstructing the asphalt roadway section between Belle Avenue to the concrete section of 24th Street
adjoining Hanna Avenue, including adding adjoining sidewalk along both sides of the roadway and the
addition of barrier free ramps at the southwest and southeast quadrants of the Belle Avenue / 24th Street
intersection and barrier free ramps at the junction of 24th Street and Hanna Avenue. This will require
removal and replacement of some existing retaining wall along the southwesterly segment of 24th Street
and possibly the installation of some retaining wall along the northeasterly side of the roadway.
Replacement of the existing water main in this segment with an eight inch PVC line and connecting
services is also included. It will include replacement of one half of the existing concrete roadway segment
of 24th Street (about eighty five feet) which joins Hanna Avenue. This will require the addition of two
new sheets, relevant revisions to fourteen other sheets and renumbering of all of the sheets in the plan set
along with revisions due to the quantities changes.
Extension of Project Reporting
Only a twelve month reporting period was allowed on the original contract which was to have had a
construction advertisement date in December 2009. Monthly M/WBE reports are still being prepared and
additional compensation for this task is requested.
Implementation of New City Standards and Specifications
Since the original contract was executed, many changes have been made to the City's standards and
specifications for contract documents. Per the City's request we have implemented the new standards and
specifications for this project. This resulted in a sheet total increase to the project manual and revisions to
the plans to meet the new city standards.

For your information only this Amendment includes approximately $2,900 for survey, $600 for additional
reporting; $10,635 for update of plan set and contract documents to new City Standards and
Specifications; $10,470 for additional design for lengthening of project and $295 for additional costs of
printing of plan sheets and contract documents.
