TO CITY SECRETARY CONTRACT r15
WHEREAS, the City of Fort Worth (CITY) and Jacobs Engineering Group,
Inc, (ENGINEER) made and entered into City Secretary Contract No. 38733,
(the CONTRACT) which was authorized by M&C-23511 on the 12th day of May,
2009 in the amount of $251,818.00; and
WHEREAS, the CONTRACT was subsequently revised by: Amendment Number
1 in the amount of $48,553.00 authorized by the City Council by M&C C-
24033 on January 26, 2010, Amendment Number 2 in the amount of $19,969.00
administratively authorized by the City on May 26, 2010, Amendment Number
3 in the amount of $37,616.00 administratively authorized by the City
Council on September 30, 2010, and Amendment Number 4 in the amount of
$0.00 administratively authorized by the City on June 27, 2011; and
WHEREAS, the CONTRACT involves engineering services for the
following project:
North Beach Street from Vista Meadows Drive to south of Shiver Road;
WHEREAS, it has become necessary to execute Amendment No. 5 to the
CONTRACT to include an increased scope of work and revised fee.
NOW THEREFORE, CITY and ENGINEER, acting herein by and through their
duly authorized representatives, enter into the following agreement,
which amends the CONTRACT:
1.
Article I of the CONTRACT is amended to include the additional
engineering services specified in a proposal dated July 17, 2012, a copy
of which is attached hereto and incorporated herein. The cost to City
for the additional design services to be performed by Engineer totals
$24,475.00.
2.
Article II of the CONTRACT is amended to provide for an increase in
the fee to be paid to Engineer for all work and services performed under
the Contract, as amended, so that the total fee paid by the City for all
work and services shall be an amount of $382,431.00,

3.
All other provisions of the Contract, which are not expressly
amended herein, shall remain in full force and effect.

SCOPE OF SERVICES
This supplement to the design services contract provides the property/right-of-way services for
revisions to the ROW and permanent easement preparation. In addition, the supplement provides
for revisions to the Paving and Drainage plans to incorporate the ROW changes and allow for
Bewley Drive,
I. PROJECT MANAGEMENT AND DESIGN SERVICES

b. Revise TCP, Paving, and Drainage sheets as necessary to allow for new ROW at
Bewley Drive.
c. Add intersection layout sheet (and median openings) as required for Bewley Drive,
3. SPECIFICATION AND BID DOCUMENT REVISIONS
a. Revise specifications to incorporate the recently completed design manual and
specifications.
b. Revise Bid Item List format using BidTools to improve accuracy and enhance
appearance.
II. PRE -CONSTRUCTION SERVICES
1. Bidding Assistance —The Engineer shall assist the City during this phase including
preparation and delivery of any additional addenda prior to bid opening to plan holders
and respond to questions submitted to the City by prospective bidders. The Engineer
shall upload all addenda onto Buzzsaw.
2. Recommendation of Award — The Engineer shall assist in reviewing the bids for
completeness and accuracy. The Engineer shall attend the project bid opening, develop
bid tabulations in hard copy and electronic format and return four (4) copies of the bid
tabulation, along with the contract documents to the City within three (3) working days
after bid openings.
3. Pre -bid Conference — The Engineer shall attend the pre -bid conference and the bid
opening, prepare bid tabulation sheets and provide assistance to the owner in evaluating
bids or proposals and in assembling and awarding contracts for construction, materials,
equipment, and services.
4. Pre -Construction Public Meeting — After the contractor has been selected, the Engineer
shall prepare project exhibits, provide an agenda, and attend a public meeting to help
explain the proposed project to residents. The City shall select a suitable location and
mail the invitation letters to the affected customers. The City shall be responsible for any
fee assessed by the public meeting facility.

III. RIGHT-OF-WAY SERVICES
1. PROPERTY SURVEY — Provide field survey for the new Heritage Addition to locate the
subdivision boundary and ROW of Bewley Drive. Analyze and revise orginal property lines
to adjust for the new Heritage Addition boundary.
2. ROW ACQUISITION DOCUMENT PREPARATION — Prepare one (1) ROW acquisition
document that includes a legal description with exhibit to CoFW standards. An additional
fee of $1800.00 will be required for additional ROW acquisition document preparation.
3. PERMANENT EASEMENT DOCUMENT PREPARATION — Prepare two (2) permanent
slope easement documents that include a legal description with exhibit to CoFW
standards. An additional fee of $1000.00 will be required for additional permanent slope
easement document preparation.

ASSUMPTIONS
1. Any additional work not specifically included in the above scope of services will be accomplished
as Additional Services, after approval by the Client.
2. If additional meetings or additional meeting duration are required, they wilt be performed as
additional services.

Dear Mr. Yates:
Gorrondona &Associates, Inc. (G&AI) is pleased to submit this proposal for professional
land surveying services for the above referenced project. The following itemized
surveying services are to be provided by G&AI:
PROPERTY/RIGHT-OF-WAY SERVICES
1.) PROPERTY SURVEY —Provide field survey for the new Heritage Addition to

required for additional right-of-way acquisition document preparation.

3.) PERMANENT EASEMENT DOCUMENT PREPARATION -Prepare two (2)

exhibit to City of Fort Worth standards. An additional fee of $ 1,000.00 per
document will be required for additional permanent slope easement document
preparation.

RECOMMENDATION:
It is recommended that the City Council authorize execution of Amendment No. 5 to City Secretary
Contract No. 38733, an Engineering Services Agreement with Jacobs Engineering Group, Inc., in the
amount of $24,475.00 for the design of North Beach Street from Vista Meadows Drive to south of Shiver
Road, thereby increasing the total contract cost to $382,431.00.
DISCUSSION:
The 2008 Capital Improvement Program (CIP) includes funding for the design and construction of this
project. This contract amendment is needed to revise the ROW and easement documents and design
plans due to new development along North Beach Street, incorporate the new City standard specifications
and contract documents, add bidding services, and add a public meeting for the project.
City Council authorized the City Manager to execute Engineering Services Contract No. 38733 with
Jacobs Engineering Group, Inc., for the design of North Beach Street from Vista Meadows Drive to south
of Shiver Road as a four lane divided roadway. The following table summarizes previous contract action
and amendments.

Jacobs Engineering Group, Inc., proposes to complete Amendment No. 5 for a lump sum fee of
$24,475.00, for a total contract amount not to exceed $382,431.00. Staff considers this fee to be fair and
reasonable for the scope of services to be performed, including revisions to the right-of-way documents
and design plans due to new development along North Beach Street, incorporation of the new City
standard specifications and contract documents, and the addition of bidding services and attendance at an
additional public meeting for the project.
Jacobs Engineering Group, Inc., is in compliance with the City's M/WBE Ordinance by committing to an
additional 27 percent M/WBE participation on this Amendment No. 5. The City's goal on this Amendment
is 27 percent.
This project is located in COUNCIL DISTRICT 2, Mapsco 22S, 22W and 3bA.
FISCAL INFORMATION /CERTIFICATION:
The Financial Management Services Director certifies that funds are available in the current capital
budget, as appropriated, of the Street Improvements 2008 fund.
