(No M&C Needed) COUNTY OF TARRANT § WHEREAS, the City of Fort Worth (City) and Dannenbaum Engineering Corporation (Engineer) made and entered into City Secretary Contract No. 38950 (the Contract) which was authorized by the City Council by M&C C- 23658 on the 13th of August, 2009; and which was subsequently amended by Amendment #1, administratively authorized on the 12th of May, 20110 and as amended by Amendment #2 administratively authorized on the 23rd of September, 2011 and as amended by Amendment #3 administratively authorized on the 20th of February, 2012 and WHEREAS, the Contract involves engineering services for the following project: 2008 Capital Improvements Program Arterial Street Project — Ray White Road Project No. 01294 WHEREAS, it has become necessary to execute Amendment No. 4 to said Contract to include an increased scope of work and revised maximum fee; NOW THEREFORE, City and Engineer, acting herein by and through their duly -authorized representatives, enter into the following agreement which amends the Contract: 1. Article I of the Contract is amended to include the additional engineering services specified in a proposal letter dated NovemberZT; 2012, a copy of which is attached hereto and incorporated herein. The cost to City for the additional services to be performed by Engineer total $8,100.00. 2. TPW Department City of Fort Worth 1000 Throckmorton Street Fort Worth, Texas 761024311 ItE; Supplemental Agreement No. 4 Ray White Road Paving Improvements City Project Nos. 01294 Dear Ms. Smith: We have prepared and are submitting for your review the attachments to Supplemental Agreement No. 4 to modify the construction plans to convert the single left bound turn lane into a double turn lane on the south bound approach of Ray White at North Tarrant Parkway. This work will generally consist of modifying Sheets 12 and 13 of 50, to show the new geometry in plan view, sheets 20 and 21, to modify the striping, and sheet 27 to modify the luminaires and corresponding conduit. Traffic signal modification shall also be incorporated and base cad drawings shall be updated in accordance with an email between Bryan Sherrieb, Wilma Smith, and Homero Beltran dated Monday, November 19'h, 2012. It does not appear that the traffic control plan will need to be changed as phasing will not have to change. If you are in agreement with the attachments for Supplemental Agreement No. 4, our authorized compensation will increase by.58,100.00. The $216,731.00 original contract amount along with the amount of $19,676.00 Supplemental Agreement No. 1, and the amount of $21,910.00 Supplemental Agreement No. 2, and the amount of $3,050.00 Supplemental No. 3 all previously authorized will be increased to $269,467.00.
