FIRST AMENDMENT TO CITY SECTRETARY CONTRACT 1N0.39247
WHEREAS on September 30, 2009, the City of Fort Worth ("City") and
SafeHaven of Tarrant County ("Contractor") made and entered into City Secretary
Contract No. 39247 (the "Contract");
WHEREAS, the Contract provided funding to Contractor pursuant to a grant
received by City from the United States Department of Housing and Urban Development
("HUD") under Title XII of the American Recovery and Reinvestment Act of 2009,
("Recovery Act") for utilization in connection with its Homelessness Prevention and
Rapid Re -Housing Program ("HPRP");
WHEREAS, the term of the Contract is from September 1, 2009 to August 31,
2011; and
WHEREAS, City and Contractor desire to exercise its option to extend the
Contract until March 31, 2012 as authorized by M&C C-23715; and
WHEREAS, Contractor desires to reallocate One Thousand Three Hundred Forty
Seven Dollars and Seventeen Cents ($1,347.17) by decreasing Salaries, Medicare, and
Unemployment and increasing FICA, Health Insurance, and Electric/Gas/Water Utilities
but leaving the total grant award at Two Hundred Fifty One Thousand Six Hundred Fifty
Four Dollars and Fifteen Cents ($251,654.15); and
WHEREAS, City and Contractor desire to further amend provisions in the
Contract regarding the budget in order to better meet program goals;
NOW, THEREFORE, City and Contractor hereby agree:
I.
The term of the Contract is hereby extended to March 31, 2012;
II.
Section 2.c. is hereby deleted and replaced with the following:
"HPRP Funds will be reimbursed in accordance with Exhibit D —
Program Operating Budget. During the Term, Contractor may submit
written proposals to increase or decrease line -item amounts in the Budget;
such approvals are subject to approval by the director of the City's
Housing and Economic Development Department ("Director"). If
Director gives written approval of Contractor's proposed Budget
amendment (as approved, the "Amended Budget"), then the Amended
Budget will take effect on the first day of the month following the month

in which it was approved by Director. No Amended Budgets will be
approved after February 15, 2012."
The following language is added to EXHIBIT D —OPERATING BUDGET immediately
after the words "BUDGET DETAIL".
"The following tables are purely informational and were created solely for purposes
of preparing, negotiating, and determining the reasonableness of the overall line item
budget in this EXHIBIT D — OPERATING BUDGET and is not to be considered part
of the terms and conditions of this Contract. Contractor may make changes to these
tables without the City's consent so long as the total amounts charged to the grant do
not exceed the line items on the first page of the EXHIBIT D — OPERATING
BUDGET."
IV.
The first page ofEXHIBIT D —OPERATING BUDGET is hereby deleted and replaced
with the attached EXHIBIT D — OPERATING BUDGET REVISED II.
V.
All terms amended herein shall be effective as of September 1, 2009. All other terms and
conditions of the Contract not amended herein remain unaffected and in full force and
effect, are binding on the Parties and are hereby ratified by the Parties. Terms used but
not defined have the same meaning as the Contract.
