TO CITY SECRETARY CONTRACT No. 39765
WHEREAS, the City of Fort Worth (CITY) and Criado & Associates, Inc.
(ENGINEER) made and entered into City Secretary Contract No. 39765, (the
CONTRACT) which was authorized by M&C C-24017 on the 12 day of January,
2010 in the amount of $ 175,075,00; and;
WHEREAS, the CONTRACT was subsequently revised by: Amendment Number 1

n the amount of $26,810.00 which was administratively authorized on
10/25/2012; and
WHEREAS, the CONTRACT involves engineering services for the
following project:
THOMPSON ROAD, City Pro3ect#:01368, North Riverside Drive to
Flowertree Drive; and
WHEREAS, it has become necessary to execute Amendment No. 2 to the
CONTRACT to include an increased scope of work and revised fee.
NOW THEREFORE, CITY and ENGINEER, acting herein by and through their
duly authorized representatives, enter into the following agreement,
which amends the CONTRACT:

Article I of the CONTRACT is amended to include the additional
engineering services specified in a proposal dated March 4, 2013, a copy
of which is attached hereto and incorporated herein. The cost to City
for the additional design services to be performed by Engineer totals
$111891.25.

Article II of the CONTRACT is amended to provide for an increase in
the fee to be paid to Engineer for all work and services performed under
the Contract, as amended, so that the total fee paid by the Cit for all
work and services shall be an amount of $213,776.25.

Re: Thompson Road, from North Riverside Drive to Flowertree Drive —Amendment No.2
Dear Mr. Buckley:
Criado &Associates, Inc. (CRIADO) is pleased to present this Amendment No.2 to the Engineering Services
Contract for City Project Number 01368 (City Secretary Contract No, 39765) "Thompson Road, from North
Riverside Drive to Flowertree Drive". This amendment includes two separate parts, as described below:
PART 1: ADDITIONAL SIDEWALK DESIGN TO HERITAGE ELEMENTARY SCHOOL, AND UPDATED
PAVING PLANS
CRIADO prepared the design of additional 560 linear feet of 4-ft and 6-ft wide sidewalk to provide pedestrian
access to Heritage Elementary School along the north parkway, from the end of full pavement reconstruction (Sta.
22+60.25) to Appleyard Circle east driveway (Sta. 28+20.00). This new sidewalk design also included the
installation of 4 new barrier free ramps and the removal and reconstruction of Appleyard Circle west driveway.
Two plan sheets were added to the Thompson Road plan set identifying these improvements. A Temporary
Right -of -Entry letter was prepared for the Contractor to obtain permission to access Heritage Elementary School
property as necessary to construct new sidewalk. CRIADO updated paving plans to reflect the pedestrian
facilities and driveways added/revised along the project area. Seven paving sheets were updated identifying
these revisions/additions. CRIADO spent $3,035.00 to complete this task.
PART 2: NEW STRIPING PLAN ALONG NORTH RIVERSIDE DRIVE AND THOMPSON ROAD, AND
REVISED STRIPING PLANS FOR THOMPSON ROAD BETWEEN NORTH RIVERSIDE DRIVE TO NORTH
BEACH STREET
CRIADO prepared the design of a new right turn lane along the northbound lane of North Riverside Drive at
Thompson Road. A preliminary color exhibit was initially prepared for City's review and comments. TP&W and
Traffic Department provided CRIADO comments and these were incorporated into the final design. One new plan
sheet was added to the Thompson Road plan set. CRIADO prepared revisions to Final Striping plans
incorporating TP&W and Traffic Department comments following a meeting with the Assistant Principal of
Heritage Elementary School, Mrs. Lilia Vasquez and a meeting with Fossil Ridge High School Principal David
Hadley. Striping plan aerial strip maps were prepared and submitted for the City's review (three different designs
were prepared and submitted). Final pavement striping plans were submitted for construction, incorporating
TP&W and Traffic Department comments. Seven pavement striping sheets were revised identifying the proposed
markings, signs, pavement lane widths, and all associated work. CRIADO spent $8,856,25 to complete this task.

This project is at its final construction phase (pavement striping), and is being completed successfully thanks to
the combined effort and collaboration between City staff (office and field), School Officials (Heritage Elementary
School Assistant Principal and Fossil Ridge High School Principal), JLB Contracting (Paving Contractor), and
CRIADO. We are asking the City of Fort Worth to approve this Amendment No.2 in the amount of $11,891,25,
incurred to perform Items 1 and 2 above,
In accordance with the City of Fort Worth Mayor and Council Communication, Reference Number C-24017 of
January 12, 2010, CRIADO's MWBE participation for this project is 26 percent. In order to achieve the MWBE
participation commitment, CRIADO proposes to increase A.N.A. Consultants original contract by having them
perform the task of preparing "As -Built" plans for a total effort of $4,900,00 and will reduce CRIADO's original
contract by the same amount. This task includes CADD Operator's time to provide CADD updates to the plans
utilizing the Contractor's As -Built Mark-ups. The breakdowns of the "Original Contract" and the "Original Contract
with Amendment No.2" are included in Attachment 1 and 2. For ease of evaluation, the values of the MWBE firm
tasks are shown in red in these tables. With Amendment No.2 and the As -Built task revision under the original
contract between CRIADO and A.N.A. Consultants, CRIADO's MWBE participation for the project remains at
26%.
We look forward to working with the City of Fort Worth Transportation and Public Works Department on the next
project. With the approval of this amendment, the total contract will be $213,776.25 with a minority participation of
26% M/WBE.
