COUNTY OF TARRANT s
WHEREAS, the City of Fort Worth (City) and Burgess & Niple,
Inc., (Engineer) made and entered into City Secretary Contract No.
C-24395, (the Contract) which was executed by the City Manager on
the Sixth day of August 2010; and
WHEREAS, the Contract involves engineering services for the
following project:
Crooked Lane Drainage Improvements, City Project No. 01335
WHEREAS, it has become necessary to execute Amendment No. 1
to said Contract to include an increased scope of work and revised
maximum fee;
NOW THEREFORE, City and Engineer, acting herein by and
through their duly authorized representatives, enter into the
following agreement which amends the Contract.
1.
Article I of the Contract is mended to include the
additional engineering services specified in a proposal letter
dated November 22, 2010 a copy of which is attached hereto and
incorporated herein. The cost to City for the additional design
services to be performed by Engineer total $6,650.00
2.
Article II of the Contract is amended to provide for an
increase in the maximum fee to be paid to Engineer for all work
and services performed under the Contract, as amended, so that the
total fee paid by the City for all work and services shall not
exceed the sum of $95,611.00

Additional Services Request for Surveying
Dr. Pandey,
This letter is our formal request for Authorization for additional surveying services on this project.
Justification
In the Preliminary Drainage Report, we determined that two additional areas need to be surveyed
outside of the original scope of work. The two areas are circled in red on Figure 1(enclosed1. The
driveway between the two lots at the low point on Crooked Lane is an existing 20' Drainage and Utility
Easement. We need survey data of the entire easement as well as survey along Crooked Lane where the
existing driveway ties into the road. Additionally, an additional inlet will likely be required on the
upstream side of High Vista Court, so additional survey data will be needed along Brentwood Stair from
Rock View Court to 20' West of Druid Lane/Druid Ct.
I have also enclosed the Surveyor's proposal letter to me for this additional effort.

IMPROVEMENTS ADDITIONAL SURVEY & SUE
Dear Mr. Mullins:
Gorrondona &Associates, Inc. (G&AI) is pleased to submit this proposal for professional
land surveying services for the above referenced project. The following itemized
surveying services are to be provided by Gorrondona & Associates, Inc.:
1.) Research property owners and obtain copies of subdivision plats and ownership
deeds.
2.) Locate property corners and establish property lines and existing right -of --way
lines. Prepare a property map of the existing right -of --way showing lots numbers,
ownership with recording information and all of the parcels of land affected by
the project. Deliverables include a property map to overlay onto the topographic
survey prepared in an AutoCad 2009 file format, along with copies of deeds,
subdivision plats and tax ownership information.
3.) Extend existing horizontal and vertical project control. Establish at least two
benchmarks or every 500' for each project. The horizontal control shall be
NAD83, north central zone with a detailed description being given for each
control point.
4.) Provide a topographic survey of the proposed drainage improvements as show in
an exhibit provided by engineer dated 11/08/2010, with the limits being down
Brentwood Stair Road from Rockview Court to the west approximately 570
linear feet and from the existing storm drainage system in Crooked Lane east
between the house to the rear lot lines approximately 130 linear feet. The survey
along Brentwood Stair Road shall include 50-foot cross -sections extending from
right -of --way to right -of --way, and at all grade breaks along the existing centerline.
The survey between the houses shall include 50' cross-section and extend from
house to house. The topographic survey shall include, but not limited to, locating
all existing features such as water valves, water meters, curb & gutter, asphalt,
sidewalks, medians, fences, driveways, storm & sewer manholes, tree lines or

trees 6" and larger, tops and toes of slope, power poles, mailboxes, signs,
telephone risers and other visible features.
6.) Deliverables shall include an AutoCad 2009 drawing file showing all %J the
features located, ground elevations, a TIN and TIN file, along with one -foot
interval contours. Provide copies of the survey field notes, a hardcopy of the
coordinates and an ASCII file of the coordinates for the points located.
�.) SUBSURFACE UTILITY ENGINEERING QL"B" — G&AI will perform this
work in general accordance with the recommended practices and procedures
described in ASCE Publication CUASCE 38-02 (Standard Guideline for the
Collection and Depiction of Existing Subsurface Utility Data). G&AI will utilize
geophysical prospecting equipment to designate the horizontal position of
existing underground utilities that are within the existing ROW. This level of
work includes acquiring "as -built" documentation from utility companies and
making the initial contact with their representatives. Based on our site visit and
information already obtained, we anticipate encountering buried electric, cable
television, telephone and storm drains behind the houses and buried electric and
sanitary sewer along the south side of Brentwood Stair Road. The length of
project is 700 linear feet and utilities will be mapped to the same limits as
defined in the Topographic Survey section at a rate of $4.50 per linear foot or
$3,150.00.
Gorrondona &Associates, Inc. can provide the above professional land surveying
services for a Lump Sum fee of $3,500.00 and a lump sum fee of $3,150.00 for SUE
services. The total amount of this proposal is for $6,650.00. If you have any questions or
