This Agreement is made and entered into by and between the City of Fort Worth ("City") and the County of Tarrant, acting through the Tarrant County Public Health Department ("Department"), of Tarrant County, State of Texas, witnessed: WHEREAS, the Department is a local Health Department under Chapter 121 of the Health and Safety Code; WHEREAS, the Department has received a grant from the Texas Department of Health to make preparations for public health emergencies; WHEREAS, the Department will need the City serve as closed Point of Dispensing ("POD") site in an Event, as defined below; WHEREAS, this POD sites will enable the City of Fort Worth to dispense medications and vaccines to its employees and the employees' household members; WHEREAS, the Department has concluded that the City possesses the facilities and personnel necessary to be a Closed POD site; and WHEREAS, this Agreement serves a public service. I POD DESIGNATION In the event of a public health emergency, the City agrees to provide a closed POD site(s), to staff the site, and to dispense the prescribed prophylaxis to its employees and household members. The City agrees to make its facilities and personnel available for mass immunization or medication of its employees and their household members as a closed POD site(s). Vaccine or medications will come from available supply sources. II PUBLIC HEALTH EMERGENCY This Agreement becomes operational in an Event, defined as: a. A public health emergency is legally declared by the applicable local, state or national health officials with such authority; and _ I OFFICIAL RECORD CITY SECRETAR FT. WORTH, TX 1 b. The local health authority declares that large scale immunization or treatment is necessary. III OBLIGATIONS OF THE DEPARTMENT (a) The Department will provide site screening at City's POD site to determine immunization participation suitability. (b) The Department will assist the City with its site dispensing plan, including but not limited to supply lists, POD layouts, fact sheets, dispensing algorithms. (c) The Department will arrange for all equipment, vaccine, and medicine, if available, to the extent necessary to administer the vaccine or medication in accordance with the City's Mass Prophylaxis Plan. (d) The Department will coordinate with the Department of State Health Services for transporting the City's allotment of medication from the Department of State Health Services storage site to the City POD Site. (e) During an Event, as defined in Section II above, provide consultation and assistance as able. (f) Provide to the City information updates as necessary and as requested by City. (g) The Department will train City employees to operating dispensing sites under the County's Medical Reserve Corps Program. (h) The Department will collect any unused medication and medical documentation, including specific immunization recipient screening information (i) The Department will provide after -action consultation to City. IV OBLIGATIONS OF THE CITY OF FORT WORTH (a) City will serve as a Closed POD and dispense medications to only employees and their household members. (b) The City will write a dispensing plan for the facility serving as a dispensing site. (c) The City will supply and arrange for all equipment and personnel necessary for staffing, security, crowd control and other tasks necessary to dispense the prescribed prophylaxis to employees and their household members. (d) The City shall designate the following individuals: 1. An administrator who will serve as the primary point of contact for the Department. 2. A medical staff point of contact for the Department. 3. A security point of contact that will interact with the Department regarding security plans. 4. A Medical Reserve Corps Liaison who will coordinate training and coordination of corporate Medical Reserve Corps members. (e) The City will provide the Department with a list containing the number of employees and their household members. (f) The City will designate one or more representatives that will have the authority to sign for receipt of medication from the Department (g) The City will provide security during transport of the medication from the Department distribution site to the City's point of dispensing. (h) The City will be responsible for disposal of medical waste, and may engage a contractor to do so. (i) The City will be responsible for inventory management during an Event V TERM This Agreement becomes effective when approved by the governing body of the County. It may be canceled by either party by giving thirty (30) days notice to the other party, otherwise it remains in effect for five (5) years and may be renewed by mutual agreement.
