April, 2011 in the amount of $214 , 706 . 00 ; and W 1 AO, the CONTRACT was subsequently revised by Amendment No. 1 in the amount of $1O1,659OO which was authorized by MC C 25553 . WHIEMEAO, the CONTRACT involves engineering services for the following project: Sanitary Sewer Rehabilitation, Contract 75. VVHEREAS, it has become necessary to execute Amendment No. 2 to the CONTRACT to include an increased scope of work re v Wised NOW DTI llL Y0401 CITY and ) NGINEER, acting herein by and through thei rr duly authorized representatives, enter into the following agreement, which amends the CONTRACT 1. Article I of the CONTRACT is amended to include the additional engineering services specified in a proposal dated March 29, 2013, a copy o2 which is attached hereto and incorporated herein. The cost to City for the additional design services to be performed by Engineer totals $90,715.00. 2. A LEO A DALY COMPANY March 29, 2013 Mr. Roberto Sauceda Project Manager City of Fort Worth Water Department 1000 Throckmorton Street Fort Worth, Texas 76102 RE: 2010 Sanitary Sewer Rehabilitation Contract 75, Contract Amendment No. 2 Request for Engineering Design and Surveying Project No. 1480 Mr. Sauceda, As requested and for the convenience of the City, LAN hereby submits this request for additional services. The services listed below were not part of the original contract scope and are necessary to complete the project. Included within the scope of services described below are design and surveying for new sewers lines in the Stratford Park area The additional bid documents, referred to as Part 3, will include designs prepared by other consultants. AMENDMENTS TO THE PROiECT SCOPE OF SERVICES 1, Design for Sanitary Sewers - LAN will provide the engineering design for the sanitary sewer lines shown in the attached Exhibit. The exhibit indicates the additional 8" sewer lines and new 4" service lines which are required. Additional plan and profile drawings will be required and details for layout and grading of all new sewer services. A revised cover sheet, location map and General Notes Sheet will be required, in addition to revised Storm Water Pollution Prevention Plans (SW3P). The SW3P plins will be prepared by an MWBE subcontractor. Construction Specifications and cost estimates will also be provided. 2. Design for Water Line In Medford Rd. - LAN will provide the engineering design for the water distribution Tine in Medford Road and Stratford Park Road, shown in the attached Exhibit. The new water line will generally parallel the new sewer line and will extend from Chelsea Street to an Existing water line in Stratford Park Road, a distance of approximately 1,600 If The design will also include approximately 200 If of water line in Medford Road to serve 3 additional homes. 3. Design Services to Include Water Imes Designed by Another Consultant - LAN will include the design drawings and coordination for new water lines in Stratford Court and Belford Ave. LAN will utilize the topographic survey provided by this consultant to design the new sewer lines. Additional survey will be required to supplement to topographic survey information provided by to us to ensure that new service lines operate properly. Design Coordination with Existing Pavement Project - LAN will provide the engineering design for a new sewer line in Avalon Ct as shown on the Exhibit. We will coordinate our sewer line design with the existing des'gn for new pavement for Avalon LAN will utilize the topographic survey provided by the consultant for the paving project to design the new sewer lines. Additional survey will be required to supplement to topographic survey information provided by to us to ensure that new service lines operate properly. 5. Revise the Design for Stratford Ct. Sewer - LAN will revise the current design of the sewer main in Stratford Ct. as well as the details for the connections of the new sewer services. 6. Provide Surveying for New Sewer Mains - Additional topographic surveying is required to complete the design of new sewers. The survey will be conducted in Meadowbrook Drive and Stratford Park, a public park. Portions of these new lines will extend in the wooded areas within the public park. The length of survey is estimated to 350 feet in the wooded at a cost of $7.00/ft and 830 feet in Meadowbrook Drive at a cost of $5 00/ft. 7. Provide Surveying to Relocate Private Services - As part of the new sewer relocation in the Stratford Park area, private services will be rerouted from the rear of the homes to the new sewers in the streets at the front of the lots. There are 34 new services. Th's relocation eliminates the need for the city to maintain portions of the services from the rear of the Tots to connections to the mains in the steep park land with limited accessibility. The additional survey is required within the backyards of the lots along Belford Avenue Avalon Ct. and Meadowbrook Drive. These are developed lots with fences and limited access. The length of survey is estimated to 4,050 feet at a cost of $4.66/ft 8. Provide Surveying Services for Easement Documents. The sewer line locations require the preparation of additional permanent and temporary construction easements on private property. These types of easements, called Park Dedications, are also required in Stratford Park. COMPENSATION Based upon the work performed as described above, a lump sum fee addition of $90,715.00 is hereby requested. This increase will adjust the total engineering fee from $396,365to $487,080. DISCUSSION: On April 12, 2011, (M&C C-24858) the City Council authorized an Engineering Agreement with Lockwood, Andrews & Newnam, Inc., in the amount of $214,706.00 to prepare plans and specifications for Sanitary Sewer Rehabilitation Contract 75. The Agreement was subsequently revised by one amendment in the amount of $181,659.00 that was approved on April 10, 2012 (M&C C-25553). Amendment No. 2 will provide compensation for the following: Design for replacement of the deteriorated water mains on Medford Road; Design for the replacement of additional deteriorated sanitary sewer mains within the Stratford Park Area; Additional design survey; and Additional easement preparation. Lockwood, Andrews & Newnam, Inc., proposes to provide the additional requested services for a lump sum fee in the amount of $90,715.00. Staff considers this fee to be fair and reasonable for the scope of services provided. M/WBE Office - Lockwood, Andrews & Newnam, Inc., is in compliance with the City's M/WBE Ordinance by committing to an additional 36 percent M/WBE participation on this Amendment No. 2. The City's goal on this amendment is 33 percent. The project is located in COUNCIL DISTRICT 8.
