The Demo/Loaner instruments are provided free -of -charge for a one -week (seven consecutive days) period from the date of delivery. Failure to return Demo/Loaner equipment on site to Scantek, Inc. by the due date without prior written approval from Scantek, Inc. will result in the automatic conversion of the Demo/Loaner agreement to a penalty rental agreement effective the first day (week day, weekend, or holiday) after the equipment is due on site at Scantek, Inc. The rental will be commensurate with the current rental fee schedule and will include a $35 per day late charge per instrument bearing a serial number. Payment terms are net 30 days from date of invoice. EQUIPMENT ON LOAN WHILE CUSTOMERS UNIT IS BEING REPAIRED IS NOT SUBJECT TO RENTAL FEE. 2. The lessee is responsible for the equipment return shipping cost to Scantek, Inc. The lessee is responsible for delays caused by their shipping department or by personnel failing to return equipment on time but is not responsible for delays due to the carrier. 3. The lessee is responsible for the cost of repair or replacement of the equipment due to theft, damage, or loss during the use or shipment of the equipment. Equipment covered by this condition includes all instruments, all associated literature, instruction manuals, operational support peripheral equipment, tools, and appropriate shipping containers. 4. The lessee is responsible for the cost of all repair parts plus a labor rate of $75/hr for the repair of any damaged equipment. 5. All equipment must be returned clean and unmarked. If equipment is not returned in the condition it was delivered, a charge of $50 per piece of equipment bearing a serial number will be charged for cleaning. 6. The lessee assumes full responsibility for the use of equipment. All equipment is checked at Scantek, Inc. before it is shipped. Scantek, Inc will not be responsible for any loss or damage resulting from equipment use or misuse, or equipment failure. 7. The lessee must notify Scantek, Inc. immediately when equipment malfunctions to receive proper credit. If not so notified, Scantek, Inc. will assume equipment operated properly and as appropriate charge for the full period. 8. If a purchase order is issued for a comparable unit, either new or used, during the period the lessee is evaluating the instruments, the lessee may keep the demo unit, rent free from the time the purchase order is issued until delivery of the ordered unit. The demo unit must be returned within two business days of delivery of the ordered unit.
