TO CITY SECRETARY CONTRACT
WHEREAS, the City of Fort Worth (CITY) and Neel--Schaffer,
Inc . , (ENGINEER) made and entered into City Secretary Contract
No. 41351, (the CONTRACT) which was authorized by M&C C-24655 on
the 14th day of December, 2010 in the amount of $114 , 512 . 00; and
WHEREAS, the CONTRACT was subsequently revised by: Amendment
Number 1 in the amount of $24, 600 . 00 which was administratively
authorized on May 26, 2011 and Amendment Number 2 in the amount
of $1, 750 . 00 which was administratively authorized on February
15, 2012; and
WHEREAS, the CONTRACT involves engineering services for the
following project:
Sanitary Sewer Rehabilitation, Contract 81; and
WHEREAS, it has become necessary to execute Amendment No. 3
to the CONTRACT to include an increased scope of work and revised
fee.
NOW THEREFORE, CITY and ENGINEER, acting herein by and
through their duly authorized representatives, enter into the
following agreement, which amends the CONTRACT:

Article I of the CONTRACT is amended to include the
additional engineering services specified in a proposal dated
April 15 , 2012, a copy of which is attached hereto and
incorporated herein. The cost to City for the additional design
services to be performed by Engineer totals $5 , 890 . 00 .
2 .
Article II of the CONTRACT is amended to provide for an
increase in the fee to be paid to Engineer for all work and
services performed under the Contract, as amended, so that the
total fee paid by the City for all work and services shall be an
amount of $146, 751 . 15 .

All other provisions of the Contract, which are not
expressly amended herein, shall remain in full force and effect .
EXECUTED and EFFECTIVE as of the date last written by a

Reference: Sanitary Sewer Rehabilitation Contract LXI (81) Contract Amendment 3 for
Additional Services
Dear Mr. Sauceda:
Neel-Schaffer, Inc. (NSI) offers additional Professional Engineering Services for this project.
These additional services are required due to the need to make requested changes to the water
and sewer locations in Erath Street as discussed April 17, 2012. This Engineering Services
Contract Amendment includes a detailed scope of additional services attached to this letter.Neel-
Schaffer, Inc, proposes to provide the attached scope of additional services for the lump sum
total of $5,890.00. All additional services proposed will be performed in accordance with the
original contract dated January 14, 2011.
This Amendment, consisting of one page; and the attached Additional Scope of Services,
consisting of one page, represent the entire amendment to the original agreement between the
City of Fort Worth and Feel-Schaffer, Inc. If the terms of this Amendment are acceptable,please
indicate by acknowledging acceptance of the terms of this document.

Additional Scope of Services
Amendment 3
Sanitary Sewer Rehabilitation Contract LXXXI(S1)
Additional work is necessary for the sewer rehabilitation of the Erath Street sanitary sewer lateral
(L-3613)and the replacement of the water line in Erath Street from Howard Street to Knox Street.
The additional work will require revisions to two double view plan sheets for the wateRdine and
revisions to four plan and profile sheets for the sewer line.
Upon receipt of an executed amendment,the Engineer will perform the following tasks:
• Switch the location of the proposed sewer line with the location of the proposed water line and
make revisions as required.
• Switch the location of the proposed water line with the location of the proposed sewer line and
make connection revisions as required.
