HomeMy WebLinkAboutContract 43862-A3 (2)AMENDMENT No 0 3 , isa urir mn ` LEZ TO CITY SECRETARY CONTRACT WHEREAS, the City of Fort Worth (CITY) and Shield Engineering Group, PLLC., (ENGINEER) made and entered Into City Secretary Contract No. 43862, (the CONTRACT) which was authorized by M&C C 2 5 817 on the 11th day of September, 2012 in the amount of $131,291.00; and WHEREAS, the CONTRACT was subsequently revised by: Amendment Number 1 in the amount of $28,369.00 which was administratively authorized on August 23, 2013 and Amendment No. 2 in the amount of $13,068.00 which was administratively authorized on October 23, 2013; and WHEREAS, the CONTRACT involves engineering services for the following project: Sanitary Sewer Rehabilitation, Contract 86. WHT+.REAS, it has become necessary to execute Amendment No . 3 to the CONTRACT to include an increased scope of work and revised fee. NOW THEREFORE, CITY and ENGINEER, acting herein by and through their duly authorized representatives, enter into the following agreement, which amends the CONTRACT: 1. Article I of the CONTRACT is amended to include the additional engineering services specified in a proposal dated January 22, 2014, a copy of which is attached hereto and incorporated herein. The cost to City for the additional design services to be performed by Engineer totals $43,738.00 (Water: $25,742.00; Sewer: $17,996.00). 2. Article II of the CONTRACT is amended to provide for an increase in the fee to be paid to Engineer for all work and services performed under the Contract, as amended, so that the total fee paid by the City for all work and services shall be an amount of $216,466.00. City Sec • Page o2 M&C: Date: N/ A N/A OFFICIAL RECORD CITY SECRETAR Ft yjoRTH TX January 22, 2014 Soon Wong Project Manager City of Fort Worth Water Department 1000 Throckmorton Street Fort Worth, TX 76102 Re: Sanitary Sewer Rehab Contract 86 - CPN 01964 Dear Soon, Shield Engineering Group (SEG) is pleased to submit the following as amended services to the original contract shown above: • Trench repair/rehabilitation plans and coordination for Eastwood Avenue (approximately 979 LF) Trench repair was added to this project as requested by the COFW Water Dept (Tony Sholola) due to recent citizen complaints and observed failures Effort includes initial site visit onsite field meeting with City officials from TPW and the Water Department trench repair plans, field trips to other trench repair locations to observe the effectiveness of other trench repair, coordination with TPW on the selected trench repair detail and geotextile materials. • Incorporate the 107 Temporary Right of Entry exhibits into the final plan construction set (approximately 26 sheets). These sheets will provide the contractor with detailed information of the private sanitary sewer relocation effort on each of the lots. They include coordinate locations of proposed cleanouts as well as removal limits of driveways, sod, trees, borings limits, etc, and items which will need to remain in place. • Prepare a stand alone plan set to replace and lower the existing waterline along Hillcrest between Camp Bowie and El Campo (approximately 3000 LF). This effort includes preliminary opinion of probable costs and survey to locate the existing meters, valves etc. We anticipate that any necessary supplemental information will be obtained from the existing plans along Hillcrest (i.e. pavement elevation, utility crossings, etc) in order to limit design survey effort. • Additional construction phase services anticipated for regular construction phase meetings This task includes weekly construction meetings and coordination with contractor and residents for service line re -alignments Total change/addendum to the contract Original contract Amendment 1 Amendment 2 Amendment 3 Total contract $131,291 $28,369 $13,068 $43.738 $216,466 The new contract amount will be $216,466.
