HomeMy WebLinkAboutContract 53829-A1 CSC No.53829-A1 AMENDMENT 1 TO CITY OF FORT WORTH SUBGRANT AGREEMENT 53829 BETWEEN THE CITY OF FORT WORTH AND TARRANT COUNTY HOMELESS COALITION FOR RENTAL ASSISTANCE AND HOUSING SERVICES This Amendment (the "Amendment") is made and entered into by and between the City of Fort Worth,a home-rule municipality of the State of Texas (hereinafter referred to as the"City"),acting by and through Fernando Costa, its duly authorized Assistant City Manager, and TARRANT COUNTY HOMELESS COALITION (hereinafter referred to as the "Agency"), acting by and through Lauren King its duly authorized interim Executive Director, for rental assistance and housing services. Each party may be individually referred to herein as a"Party" and collectively as "Parties." RECITALS WHEREAS, the City of Fort Worth received an award of$140,243.00 from the Texas Department of Housing and Community Affairs ("TDHCA") through its Homeless Housing and Services Program ("HHSP")Youth Set-Aside Grant ("Grant"); WHEREAS, on December 12, 2019, the City entered into a contract with TDHCA, City Secretary Office (CSO) Contract No. 53208; WHEREAS, on April 24, 2020, the City entered into a Subgrant Agreement with the Agency to provide services more specifically described in the agreement, Contract No. 53829, (the "Contract"); and WHEREAS, on August 17, 2020 the City and TDHCA amended Contract No. 53208 to extend the term of the contract to February 28, 2021 and modify the budget, Contract No. 53208- A1; WHEREAS, the Parties agree to amend the contract term and budget of the Contract to reflect the City's contract amendment with TDHCA, Contract No. 53208-A1; WHEREAS, the Parties agree to add Exhibit "G" to reflect TDHCA performance measures; NOW THEREFORE City and Agency do hereby agree to the following: II. AMENDMENT TO AGREEMENT Replace Section 2 with the following: OFFICIAL RECORD 2. Add attached Exhibit"G" Performance. III. This amendment is effective as of the Effective Date of this Amendment. IV. All terms and conditions of the Contract not amended herein remain unaffected and in full force and effect, are binding on the Parties and are hereby ratified by the Parties. Capitalized terms not defined herein shall have the meanings assigned to them in the Contract. This report shall itemize each monthly expense requested for reimbursement by the Agency and shall be included in Budget submitted. In order for this report to be complete the following must be submitted: For payroll expenses, timesheets signed and dated by employees and approved by supervisor for all payroll expenses listed with the code of time being billed to City/Directions Home. Paystub must include pay period, date paid, amount and expenses (salary, FICA, benefits etc). If pay stubs are unavailable, payroll registries with applicable expenses highlighted and labeled will suffice. Agency may not submit payroll expenses dated 60 calendar days prior to the date of the Reimbursement Request with the exception of the first Reimbursement Request which may include items from the Effective Date of the Agreement to the end of the reporting month. For non-payroll expenses, invoices for each expense listed. Agency may not submit invoices dated 60 calendar days prior to the date of the Reimbursement Request with the exception of the first Reimbursement Request which may include items from the Effective Date of the Agreement to the end of the reporting month. Proof that each expense was paid by the Agency, which proof can be satisfied by cancelled checks. If a cancelled check is not possible,a bank statement with the expense highlighted and labeled will suffice. If allocations percentages are used, all documentation must be submitted with the first month's invoice. For the audit, bank statements showing payments, Form 941 s and allocation documentation will be reviewed. Costs associated with the purchase or licensing of HMIS or an HMIS-comparable databases are eligible administrative costs. **Case management costs include staff salaries related to assessing, arranging, coordinating and monitoring the delivery of services related to finding or maintaining housing. Costs include, but are not limited to, Household eligibility determination, counseling, coordinating services and obtaining mainstream benefits for Program Participants,monitoring Program Participant progress, providing safety planning for persons under VAWA, developing a housing and service plan, and entry into HMIS or an HMIS-comparable database. ***Essential services costs are associated with finding and maintaining stable housing, and include,but are not limited to, costs for: (1) out-patient medical services; (2) child care; (3) education services; (4) legal services; (5)mental health services; (6) local transportation assistance; (7) drug and alcohol rehabilitation; and (8)job training. Amendment to CSO Contract No.53829 Page 5 of 7 CoFW and TARRANT COUNTY HOMELESS COALITION ""Homelessness prevention and homelessness assistance costs are associated with housing relocation, stabilization and assistance costs. Staff time entering information into HMIS or HMIS- comparable database related to homelessness prevention and homeless assistance is also an eligible cost. Homeless prevention and homelessness assistance costs include, but are not limited to,hotel or motel costs;transitional housing;rental and utility assistance;rental arrears;utility reconnection fees; reasonable and customary security and utility deposits; and moving costs.
